Malignant ovarian germ cell tumors are rare malignancies primarily affecting adolescents and young adults. Management must balance curative intent with fertility preservation. This review highlights recent advances in malignant ovarian germ cell tumor management, focusing on risk-stratified approaches, surgical techniques, somatic transformation, and long-term surveillance. The bleomycin/etoposide/cisplatin regimen remains the first-line therapy. Fertility-sparing surgery, including unilateral salpingo-oophorectomy with minimal staging, is recommended for adolescent and young adult patients. C

INTRODUCTION: Testicular germ cell tumors (TGCT) are highly curable malignancies, with excellent survival rates largely attributable to advances in cancer treatment. Consequently, there is a growing population of long-term TGCT survivors whose life expectancy approaches that of the general population. However, these survivors may experience acute and late adverse effects of cancer treatment, with cardiovascular toxicity being among the most serious and potentially life-threatening.
METHODS: This narrative review synthesizes current evidence on cardiovascular toxicity in testicular cancer surv

Germ cell tumors are native to the testis, retroperitoneum, and mediastinum. Here, we report our experience on rare scenarios in this entity. The first patient presented with a primary mediastinal germ cell tumor with 2° hemophagocytosis. This dysregulated immune milieu portends a dismal outcome irrespective of treatment. Gastrointestinal involvement in germ cell tumor accounts for only 5% of cases. The Second case with duodenal involvement and hematemesis is believed to be due to contiguous infiltration of the retroperitoneal node. Another case with bleeding ileal intussusception was seen aft

This case report describes the management of a 30-year-old male patient with a history of an advanced nonseminomatous germ cell tumor, hip fracture complicated by extensive deep vein thrombosis and pulmonary embolism, and on apixaban presenting with asymptomatic intracardiac teratoma and abdominopelvic metastases. Multidisciplinary intervention, including successful surgical excision of the intracardiac mass, highlights the importance of coordinated care and vigilant follow-up in optimizing patient outcomes and preventing life-threatening complications.

BACKGROUND: Testicular mixed germ cell tumors (TMGCTs) are rare malignant tumors that are more common in men aged 20-40 years. TMGCTs comprise two or more types of germ cell tumors that primarily affect the testis. Their onset is undetectable; thus, early diagnosis is challenging. However, early recognition and diagnosis substantially improve patient prognosis.
CASE SUMMARY: We evaluated a rare case of TMGCT in a male patient presenting with recurrent fever and left supraclavicular lymphadenectasis instead of testicular enlargement and pain, which may easily lead to misdiagnosis. We report th

INTRODUCTION: Pineal region tumors (PRTs) are tumors arising from the pineal gland and the paraspinal structures. These tumors are rare and heterogeneous that account for 2.8-10.1% and 0.6-3.2% of tumors in children and in all ages, respectively. Almost all types and subtypes of CNS tumors may be diagnosed in this region. These tumors come from cells of the pineal gland (pinealocytes and neuroglial cells), ectopic primordial germ cells (PGC), and cells from adjacent structures. Hence, PRTs are consisted of pineal parenchyma tumors (PPTs), germ cell tumors (GCTs), neuroepithelial tumors (NETs),…

INTRODUCTION: Regressed germ cell tumors are a rare disease commonly diagnosed with metastatic symptoms without local symptoms in the testis.
CASE PRESENTATION: A 33-year-old man with azoospermia was referred to our hospital. His right testis was slightly swollen, and ultrasonography revealed hypoechogenicity of the right testis with decreased blood flow. Right high orchiectomy was performed. Pathologically, the seminiferous tubules were absent or highly atrophied with vitrification degeneration; however, no neoplastic lesion was confirmed. One-month post-surgery, the patient noticed a mass i

Testicular cancer is the most common solid neoplasm of adult men between the ages of 20 and 40 years. Germ cell tumors account for 95% of all testicular tumors. The assessment of staging is essential to guide further management of patients with testicular cancer and to prognosticate cancer-related outcomes. Postradical orchiectomy treatment options, including adjuvant therapy and active surveillance, vary based on the anatomical extent of disease, serum tumor markers, pathologic diagnosis, and imaging. This review provides an update on the germ cell tumor staging system adopted by the 8th edit

BACKGROUND: Intracranial germ cell tumors (IGCTs) are rare, highly curable neoplasms. KRAS is a gene in the KIT/RAS signaling pathway, and KRAS mutations have been reported in patients diagnosed with IGCTs.
OBJECTIVES: To describe the clinicopathologic and molecular features of KRAS mutation and the treatment outcome of children diagnosed with IGCTs.
METHODS: Patients diagnosed with IGCTs at the Department of Pediatrics, King Chulalongkorn Memorial Hospital from 2007 to 2016 were retrospectively reviewed. DNA was extracted from formalin-fixed, paraffin-embedded tissue and used for molecular

LESSONS LEARNED: Advanced germ cell tumors are aggressive and associated with poor prognosis. Pembrolizumab was overall well tolerated in 12 heavily pretreated patients. Three patients had radiographic stable disease that lasted for 10.9 months, 5.5 months, and 4.5 months, respectively. Published data of immunotherapeutic agents in patients with advanced germ cell tumors are confirmed. The limited antitumor activity of immunotherapy in germ cell tumors is, at least partially, attributed to tumor biology (low tumor mutational burden; low PD-1 expression) and other poor-risk features. The authors report on a case of a patient who presented to the emergency department (ED) and was ultimately diagnosed with stage IV testicular non-seminomatous germ cell tumor. The patient was cachectic with a tumor on the neck, abdomen, and scrotum. Germ cell tumors (GCTs) exhibit characteristic symptoms at different points in development. Appropriate treatment can cure most GCTs. While cancer may not be thought of as an ED diagnosis, it can often be the place where patients first present, even when advanced. Recognizing it is important for prompt treatment.

INTRODUCTION: Obesity at the time of diagnosis has been associated with better outcomes in some metastatic cancers such as renal-cell cancer. The association between body mass index (BMI) and germ-cell tumor (GCT) outcomes has not been reported. We sought to assess the association between BMI and outcomes in men with GCT in a large hospital registry.
PATIENTS AND METHODS: Electronic medical records for 1161 GCT patients treated at Dana-Farber Cancer Institute between 1997 and 2012 were reviewed. Information regarding BMI, histology, stage, treatment, and patient characteristics was obtained.

Germ cell tumors, because they contain immature and mature elements, can differentiate into different tissue types. They can exhibit unusual imaging features or manifest in a syndromic fashion. The authors describe these features and assign them to one of the following categories: (a) unusual manifestations of metastatic disease (growing teratoma syndrome, choriocarcinoma syndrome, ossified metastases, and gliomatosis peritonei); (b) autoimmune manifestations (sarcoidlike reaction and paraneoplastic syndromes); (c) endocrine syndromes (sex hormone production, struma ovarii, and struma carcinoi

A 37-year-old male presented with a traumatic injury to the scrotal region necessitating emergency surgery. Evacuation of a haematoma and bilateral orchidectomy were performed. A left sided nonseminomatous germ cell tumour (NSGCT), predominantly yolk sac, was identified. Microscopic margins were positive for tumour. Initial tumour markers revealed an AFP of 22,854 ng/mL, HCG of <1 mIU/mL, and LDH of 463 IU/L. Eight weeks after surgery, AFP levels remained elevated at 11,646 ng/mL. Computed tomography (CT) scanning demonstrated left inguinal adenopathy, 1.5 cm in max dimension. On review, exten

A 54-year-old man was admitted to our hospital for further examination following a chest radiographic screening test. He was asymptomatic but had an elevated serum concentration of alpha-fetoprotein. Chest computed tomography (CT) and magnetic resonance imaging showed a posterior mediastinal mass in the left thoracic paravertebral region. CT-guided percutaneous needle biopsy was performed, and the histological findings confirmed nonseminomatous germ cell tumor (NSGCT). Gonadal examination and F-18 fluorodeoxyglucose positron emission tomography findings indicated the posterior mediastinum as t

Extragonadal germ cell tumors are rare neoplasms most commonly diagnosed in the first and second decades of life. Intracranially they tend to arise in the midline in the pineal and hypothalamic/intrasellar regions. The clinical presentation in the central nervous system depends on the patient's age, tumor location, and tumor size. Because of the proximity of these tumors to the hypothalamus and pituitary gland, their presence often results in the development of various hormonal abnormalities. Patients most often present with diabetes insipidus, growth hormone insufficiency and precocious puber
