Principles for successful outcomes in microsurgical guided bone regeneration.

BACKGROUND: Guided bone regeneration (GBR) is essential for rehabilitating alveolar defects, with success relying on biological processes and surgical precision. Despite advances in biomaterials and surgical protocols, operator-dependent variability remains a challenge. Integrating microsurgical principles and surgical microscopy-with magnification (5-30×) and coaxial illumination-enhances submillimeter precision, potentially improving accuracy and predictability in GBR procedures. We demonstrated the clinical application of microscopic GBR in challenging periodontal scenarios, highlighting th

Macrophage Phenotype Is Associated With the Regenerative Response in Experimental Replacement of the Porcine Esophagus.

A porcine model for bridging circumferential defects in the intrathoracic esophagus has been developed in order to improve the treatment of children born with long-gap esophageal atresia. The aim of this study was to identify factors beneficial for tissue regeneration in the bridging area in this model and to describe the histological progression 20 days after replacement with a silicone-stented Biodesign mesh. Resection of 3 cm of intrathoracic esophagus and replacement with a bridging graft was performed in six newly weaned piglets. They were fed through a gastrostomy for 10 days, and then h

Decalcified and Decellularized Bone Conduits for Peripheral Nerve Regeneration: A Comparative In Vivo Study Using Rat Femur and Chicken Ulna Conduits.

PURPOSE: Peripheral nerve injuries (PNIs) pose a major clinical challenge, especially when nerve gaps exceed 5 mm and spontaneous regeneration is inadequate. This study aimed to evaluate the regenerative potential of novel bone-derived nerve guidance conduits (NGCs) fabricated from rat femoral diaphysis and chicken ulna as alternatives to conventional autologous and allogeneic grafts. METHODS: Demineralized and decellularized bone scaffolds were characterized for collagen preservation, luminal geometry, and mechanical and electrical properties. In vivo, the conduits were implanted to bridge 1

Utilizing biological responses to evaluate thein vivofate and biocompatibility of reduced graphene oxide-integrated nerve guidance conduits for peripheral nerve repair.

Reduced graphene oxide (rGO) has been widely incorporated into nerve guidance conduits (NGCs) to promote peripheral nerve regeneration owing to its physicochemical properties. However, some studies have reported potential biocompatibility risks of rGO and raised concerns about its possible adverse effects on nerve regeneration. In this study, anin vivochallenge model was established. Optical microscopy techniques were comprehensively employed to track the distribution and degradation behavior of rGO after implantation of rGO-incorporated NGCs, and biological evidence at both cellular and tissu

The accumulation of T cells within acellular nerve allografts is length-dependent and critical for nerve regeneration.

Repair of traumatic nerve injuries can require graft material to bridge the defect. The use of alternatives to bridge the defect, such as acellular nerve allografts (ANAs), is becoming more common and desired. Although ANAs support axon regeneration across short defects (<3 cm), axon regeneration across longer defects (>3 cm) is limited. It is unclear why alternatives, including ANAs, are functionally limited by length. After repairing Lewis rat nerve defects using short (2 cm) or long (4 cm) ANAs, we showed that long ANAs have severely reduced axon regeneration across the grafts and contain S

Role of platelet-rich plasma in soft tissue root-coverage procedures: a review.

The aim of this article was to review the current evidence on the role of platelet-rich plasma (PRP) in enhancing root-coverage techniques and discuss the rationale for its use in these applications. Sound biologic rationale and a multitude of basic science research support the use of PRP to promote soft tissue healing, although evidence of its role in enhancing periodontal applications, especially root coverage, is limited. Current scientific research has yet to elucidate all of the mechanisms by which PRP can affect soft tissue healing and assess its capacity to stimulate regeneration. Furth

Nerve regeneration using tubular scaffolds from biodegradable polyurethane.

INTRODUCTION: In severe nerve lesion, nerve defects and in brachial plexus reconstruction, autologous nerve grafting is the golden standard. Although, nerve grafting technique is the best available approach a major disadvantages exists: there is a limited source of autologous nerve grafts. This study presents data on the use of tubular scaffolds with uniaxial pore orientation from experimental biodegradable polyurethanes coated with fibrin sealant to regenerate a 8 mm resected segment of rat sciatic nerve. METHODS: Tubular scaffolds: prepared by extrusion of the polymer solution in DMF into w

[Root coverage techniques--a literature review and a case report].

The main goal of periodontal therapy is to improve periodontal health and by doing so to maintain the patient's dentition. However, aesthetic, preservation and improvement have become an integral part of today's periodontal treatment. The term "Periodontal Plastic Surgery" was defined by the American Academy of Periodontology in 1996 as "surgical procedures preformed to prevent or correct anatomical, development, traumatic or plaque disease-induced defects of the gingiva, alveolar mucosa, or bone". Gingival recessions present a functional and aesthetic problem which is a major indication for P

Repair of articular cartilage defects: review and perspectives.

Articular cartilage defects heal poorly and lead to catastrophic degenerative arthritis. Clinical experience has indicated that no existing medication substantially promotes the healing process and the cartilage defect requires surgical replacement, preferably with an autograft. However, there is a shortage of articular cartilage that can be donated for autografting. A review of previous unsuccessful experiences reveals the reason for the current strategy to graft cartilage defects with regenerated cartilage. Autologous cartilage regeneration is a cell-based therapy in which autogenous chondro

Perspectives on the Novel Multifunctional Nerve Guidance Conduits: From Specific Regenerative Procedures to Motor Function Rebuilding.

Peripheral nerve injury potentially destroys the quality of life by inducing functional movement disorders and sensory capacity loss, which results in severe disability and substantial psychological, social, and financial burdens. Autologous nerve grafting has been commonly used as treatment in the clinic; however, its rare donor availability limits its application. A series of artificial nerve guidance conduits (NGCs) with advanced architectures are also proposed to promote injured peripheral nerve regeneration, which is a complicated process from axon sprouting to targeted muscle reinnervati

Polymeric biomaterials for nerve regeneration: fabrication and implantation of a biodegradable nerve guide.

Optimizing the quantity, quality, and speed of axon regeneration is important in maximizing functional outcomes following peripheral nerve injury. When severed, injured nerves must be able to regenerate and reconnect to the structures they previously controlled within 12-18 months before sensation and motion are permanently lost. Nerve sprouts from the proximal stump will spontaneously migrate toward the distal stump in the event of a nerve transection. However, surgical intervention remains necessary to repair transection injuries. Regeneration becomes particularly troublesome with large gaps
