This report is on a severe case fo a HELLP-syndrome (H haemolysis, EL elevated liver enzymes, and LP low platelets). A 32-year old gravida developed severe preeclampsia with epigastric pain at 33 weeks' gestation. During a few hours post partum she showed disseminated intravascular coagulation (DIC) and required intensive care. The severe HELLP-syndrome was combined with a fast increasing acute respiratory distress syndrome (ARDS) and acute oligo-anuric renal failure. She was treated in the intensive-care unit for several days with artificial respiration, 10 acute haemodialyses, 4 plasma excha

Postpartum recovery was examined in 100 pregnancies complicated by severe pre-eclampsia, and in 15 pregnancies in which HELLP-syndrome as present. Albuminuria disappeared and diastolic blood pressure returned to normal (< 90 mmHg) in half of the cases within one week postpartum. Postpartum recovery in the cases with HELLP-syndrome did not differ from that seen in the pre-eclamptic patients. Thrombocytopenia showed spontaneous resolution within three days after delivery. In stepwise discriminant analysis, the incidence of IUGR predicted a slow postpartum recovery, but this did not apply to othe

43 cases with severe preeclampsia with HELLP syndrome were observed out of 14890 deliveries from 1980 to 1993 in the Department of Obstetrics and Gynecology, University of Mainz. In 17 cases there were grave complications (14 renal failure: 3 patients requiring dialysis; 6 bleedings: abdominal wall hematoma in 5 patients after caesarean section, 1 rupture of the liver; 3 pulmonary complications; 9 cerebral complications: 7 eclamptic convulsions, 1 amaurosis caused by partial infarction of the posterior cerebral artery, 1 sinus venous thrombosis). The plasmatic blood-clotting factors were in th…

We describe the use of thrombelastography in HELLP syndrome (Haemolysis, Elevated Liver Enzymes, Low Platelets). It differentiated between two possible causes of significant haemorrhage and revealed an accompanying underlying fibrinolysis. This allowed specific therapy to be directed at both abnormalities and, we believe, helped prevent this patient from undergoing radical surgery to curb blood loss.

150 cases of maternal death were registered in Bavaria from 1.1.1983-31.12.1992. 15 women (10%) died in the puerperal period due to preeclampsia. 14 of these patients either had single or complete laboratory findings in HELLP-Syndrom. Anamnesis, age, parity, nationality, way of delivery and causes of death are analysed. To improve the prognosis of patients with HELLP-Syndrom early diagnosis and immediate delivery are necessary. First signs of pre-eclampsia require regular liver-enzymes and platelet controls. In the occurrence of right upper abdominal pain in the second half of pregnancy precis

We report on a case of a 22-year old primigravida with HELLP syndrome in association with blindness and cerebral sinus thrombosis. The diagnosis of the cerebral sinus thrombosis was based on MRI 1h after delivery. Immediate caesarean section and intensive care treatment led to a complete recovery of the patient.

A rather unusual but dramatic form of EPH gestosis or pregnancy-induced hypertension is presented via the case of a 30-year old second gravida in the 30th week of pregnancy. This is a case of pre-eclampsia combined with haemolysis, elevated liver function tests and low platelet counts. Diagnostic features, pattern and pathophysiology are discussed with regard to international literature.

A very rare, but severe complication, occurring together with preeclampsia, is the so-called HELLP syndrome (H for haemolysis, EL for elevated liver enzymes and LP for low platelet counts). Perinatal mortality for this syndrome is estimated at 9.5% up to 60%, maternal mortality at 3.5%. We examined retrospectively 11 patients of the last 5 years, presenting not only hypertension, proteinuria and oedema, but also pathologically elevated data concerning transaminases, indirect bilirubin and thrombocytopenia; the mean value for GI in this group was 7.7 (3-11). Thrombocyte counts showed mean value…

Reversible posterior lekoencephalopathy syndrome (RPLS) is usually reversible. However, permanent cerebral damage may result if diagnosis is … es of postpartum RPLS without involvement of posterior brain regions after eclampsia complicated by HELLP syndrome are very rare. Patients with RPLS do not always show typical manifestations.

BACKGROUND: The hemolysis, elevated liver enzymes, low platelet count (HELLP) syndrome is a serious complication in pregnancy occurring in 0.5-0.9% of all pregnancies and in 10-20% of cases with severe pre-eclampsia. Previous studies described HELLP syndrome without hemolysis without any further details.
OBJECTIVES: This report represents the criteria for the diagnosis of Abdelazim and AbuFaza elevated liver enzymes, low platelet count (ELLP) syndrome as a variant of HELLP syndrome.
CASE REPORTS: A 39-year-old woman, pregnant 32 weeks' gestation, previous five cesarean sections, admitted wit…

Uterine torsion belongs to one of the most dangerous labor complications also because of its rarity. As it is not common for an obstetrician to come across this state, it is usually not at the very top of the differential diagnostics list when solving acute child delivery complications. However, it is serious enough to pose a lethal threat to both mother and child. In this case, the term gravidity was ended by acute cesarean section because of HELLP syndrome. During the operation, as well as after a complicated delivery and hysterotomy suture uterine torsion of 120 degrees to the right, the pa…

KEY CLINICAL MESSAGE: HELLP syndrome is a complicated disorder associated with many unknown complications, which ischemic colitis might be one of. Timely diagnosis and prompt management with multidisciplinary approach is the key for a favorable outcome.
ABSTRACT: HELLP syndrome is a triad of hemolysis (H), elevated liver enzymes (EL), and low platelet count (LP) which is considered a rare but serious pregnancy complication. HELLP syndrome is mainly associated with pre-eclampsia, but can also occur individually. It may cause maternal and fetal mortality and some life-threatening morbidity. The…

Subcapsular hematoma of the liver is a potentially fatal complication of pregnancy-related hemolysis, elevated liver enzymes, and low platelets (HELLP) syndrome. In turn, HELLP syndrome is one of the most critical complications of preeclampsia. We present a case of a 31-year-old woman who developed a subcapsular liver hematoma in the postpartum period secondary to HELLP syndrome. The diagnosis was made based on the clinical features, radiological investigation, and laboratory values. She was managed with fluids, intravenous steroids, and rigorous monitoring in the intensive care unit before be…

The present report describes the case of a 31-year-old primigravida, with dichorionic twins at 31 weeks. She presented with history of myalg … in patients with COVID-19 appears to be multifactorial, similar to what occurs in preeclampsia and HELLP syndrome. We assume that the synergism of these pathophysiological mechanisms could accelerate the compromise of maternal conditions and could be a warning to the obstetric practice.

HELLP syndrome is a combination of symptoms described as hemolysis, elevated liver enzymes and low platelets. HELLP is a common life-threatening complication of pregnancy thought to be a variant or complication of preeclampsia. In this case report, we aimed to present a woman with acute postpartum HELLP syndrome complicated by pulmonary edema after caesarean section following severe preeclampsia. Our experience suggests that early detection of HELLP syndrome and timely management will bring good outcomes.
