Recurrence of glomerulonephritis after kidney transplantation is especially in the long term an important cause of renal allograft failure. The exact frequency depends on the one hand how the diagnosis of recurrence was established, either on clinical grounds or histologically via a kidney transplant biopsy, and on the other hand on the type of the underlying or primary glomerular disease. The consequences of a relapse on allograft function and survival vary, depending on the primary disease. For example, recurrences after IgA nephropathy occur depending on the length of the observation period

A liver nodule in a patient transplanted for primary sclerosing cholangitis: an interdisciplinary diagnostic approach.

We report the case of a 53-year-old female patient who was transplanted with the liver of a 71-year-old male donor for advanced primary sclerosing cholangitis (PSC) and who additionally was diagnosed with a histologically non-classifiable colitis shortly before transplantation. Upon follow-up abdominal ultrasound 4 months after transplantation, a liver lesion measuring 16 × 23 mm was detected in the transplanted liver. This lesion had not been noticed immediately after transplantation and showed a pattern suspicious for malignancy in contrast-enhanced ultrasound. In line, a biopsy revealed the

Early Liver Transplantation in Acute Alcoholic Hepatitis.

Alcohol-related liver disease (ALD) is currently the leading indication for liver transplantation in the United States. Among patients with ALD, those with acute alcoholic hepatitis who do not respond to medical treatment have a 6-month mortality of 70% without transplantation. Despite the high mortality, the majority of patients will not be eligible for transplant, given that most centers follow the 6-month abstinence rule. A handful of centers in Europe and the United States perform early liver transplantation (< 6 months abstinence) in these patients, as it provides a substantial survival b

A Cadaver Study of the Feasibility of Multidigit Allotransplantation for Reconstruction of the Metacarpal Hand.

BACKGROUND: Amputation of all fingers (metacarpal hand) can be functionally equivalent to hand amputation. Multidigit allotransplantation can benefit patients who require a full complement of fingers to return to their preinjury activities. The authors investigated the feasibility of the transfer of four fingers and thumb as a single allograft. METHODS: Four fingers and the thumb were harvested from 16 hands as an allograft based on the radial and ulnar arteries. Dorsal digital veins were dissected until confluent in the major veins. The common digital nerves were divided at the origin. The f

Cellular therapies for prolongation of composite tissue allograft transplantation.

Complex musculoskeletal defects resulting from cancer, congenital absence, and trauma represent a unique reconstructive challenge. Autologous tissue is often unavailable to reconstruct these deformities. Composite tissue allograft transplantation represents a unique solution for these clinical problems. Face, hand, or limb transplants can be performed in a single procedure. However, the use of chronic nonspecific systemic immunosuppression can lead to side effects such as drug toxicity, opportunistic infections, and malignancies. This article explores various cell-based therapies that represen

A Unique Case of Heart Transplant and Toxoplasma gondii Infection in a Parasite-Seronegative Recipient: A Case Report.

INTRODUCTION: The protozoan Toxoplasma (T.) gondii is responsible for toxoplasmosis, and this parasitosis represents a high risk in solid organ transplant procedures. In heart transplant patients, T. gondii infection is usually fatal since immunosuppressive drug therapy is administered to recipients. CASE PRESENTATION: A 62-year-old woman with severe biventricular dysfunction and seronegative for T. gondii underwent a cardiac transplantation from a donor seropositive for anti-T. gondii IgG antibody. The recipient exhibited post-transplant complications, including acute renal failure and diffi

Anatomical Foundations and Clinical Utility of Personalized Free Ilioinguinal Zone Flap for Hand Skin and Soft Tissue Defect Reconstruction.

OBJECTIVE: This study aims to explore the anatomical foundations and clinical utility of personalized free ilioinguinal zone flaps for reconstructing skin and soft tissue defects in the hand. METHODS: Anatomical evaluations of the superficial circumflex iliac and the superficial epigastric arteries and veins were performed on 10 cadavers within the inguinal region. Forty-two patients received flap transplants from January 2017 to December 2022 to repair hand tissue defects. Flap areas ranged from 4.0 cm × 6.0 cm to 11.0 cm × 23.0 cm, with arterial anastomosis ensuring vascular connection to r

Severe influenza in a paediatric patient with GATA2 deficiency and Emberger syndrome.

A 9-year-old girl was admitted to the paediatric intensive care unit with acute respiratory failure due to influenza. Nine months earlier, she presented with unexplained lymphoedema of the lower extremities and monocytopenia. She had a history of occasional finger warts and onychomycoses. During hospitalisation, the patient was diagnosed with Emberger syndrome caused by GATA2 deficiency. The admission was complicated by thromboses in the right hand, leading to amputation of multiple fingers. From then on, the patient has been in good recovery, the function of her right hand was improving and a

[Results of Kirschner wire osteosynthesis and intramedullary bone peg in toe-transfers].

During 1995-2002 in General and Hand Surgery Department 21 digits were reconstructed by toe to hand transplantation performed in 16 patients. There were 10 cases thumb was reconstruction with 4 great toe and 6 second toe transfers. Rest 11 fingers there were index, middle and ring were restored by II and III toe. Twelve osteosynthesis were performed by Kirschner wire and nine by bone peg. All fixations were sufficient stable to start early rehabilitation. In one case after Kirschner fixation occurred ostitis and malunion. This patient was cured by autovaccine. After bone peg we do not recorded…

CMV-specific T cell therapy.

Human cytomegalovirus (CMV) infection continues to be one of the most important and life threatening complications after allogeneic stem cell transplantation (SCT). The reconstitution of CMV-specific T cell responses after SCT has been demonstrated to be protective against the development of CMV disease. To improve T cell immunity against CMV in bone marrow transplant patients, different strategies were explored. On one hand, CMV-specific T cells can be selected from the donor, and can be transferred to the patient without any further in vitro expansion. On the other hand, CMV-specific T cells

Vascularized Composite Allotransplantation of the Hand: A Systematic Review of Eligibility Criteria.

BACKGROUND: Hand transplantation (HT) has emerged as an intervention of last resort for those who endured amputation or irreparable loss of upper extremity function. However, because of the considerable effort required for allograft management and the risks of lifelong immunosuppression, patient eligibility is critical to treatment success. Thus, the objective of this article is to investigate the reported eligibility criteria of HT centers globally. METHODS: A systematic review of the HT literature was conducted according to Preferred Reporting Items for Systematic Review and Meta-Analyses g…

[Hand allografts: experience from Lyon team].

Hand allograft is a method in the stage of clinical experimentation, which is reserved in France for the treatment of bilateral traumatic amputees. This study reports the Lyon team experience, which is pioneer in this domain. Four patients (3 males and 1 female) underwent seven (one unilateral and three bilateral) hand transplantations from September 1998 to February 2007. The level of amputation was at the wrist or at the mid-forearm. Delay since hand loss ranged from 2.5 to 9 years. The surgical protocol was elaborated and planned case by case. All recipients received the same immunosuppress…

Toe to hand transfer.

Nine patients received a toe-to-finger transfer after accidental amputation and one after a congenital defect. One led to secondary amputation due to arterial thrombosis. Seven of the 9 patients would have the operation again if needed. Five transplants were great toe-to-thumb transfers which the authors feel superior to second toe to thumb transplantations. The method seems to be useful in reconstructive hand surgery.

Perifascial Areolar Tissue Transplantation for Covering Exposed Proximal Interphalangeal Joint After Electric Burn.

Soft tissue damage in electric burn injuries is often deep and may expose tendons, bones, or joints in the hand. Here, we present the case of a 76-year-old man treated using perifascial areolar tissue (PAT) transplantation for covering the proximal interphalangeal joint of the middle finger, which had been exposed because of an electric burn. Following ointment therapy, surgery was performed on day 34 postinjury when a deep ulcer with opening of the proximal interphalangeal joint was observed on the dorsum of the right middle finger. After resecting the cartilage of the proximal interphalangea

Distraction and microvascular epiphysis transfer for radial club hand.

This paper presents a different technique of treatment for Bayne type IV radial club hand using a microvascular joint transfer in order to reconstruct the absent half of the wrist joint, aiming for better movement and stability at the wrist joint with preservation of longitudinal growth. The method uses preoperative soft tissue distraction to obtain proper alignment of the hand on the ulna before the second metatarsophalangeal joint with the whole metatarsal bone is transplanted. The treatment takes about 4 months and the optimum period for surgery is during the second year of life. Pollicizat

Treating complex palmar-plantar wounds using a bilaminar 'trapdoor' technique: a case series.

OBJECTIVE: Due to the similarities of glabrous skin, the plantar region is an excellent donor area for covering complex palmar-plantar wounds. However, taking grafts from the plantar area often results in significant morbidity at the donor site or non-integration of the graft due to the greater thickness of the plantar corneal layer. METHOD: This is a prospective case series including patients with burns or wounds who have been treated with a dermal graft using the bilaminar 'trapdoor' technique. This procedure is used to remove a thin graft from the deep plantar dermis after the partial elev
