Infection following operative treatment of ankle fractures.

UNLABELLED: Information on the microbiology of infections after operative ankle fractures, on the details of a treatment protocol used when … ively were treated by débridement and either hardware retention (if implants were judged stable) or hardware removal (if implants were loose). All patients presenting more than 10 weeks postoperatively underwent débridement and hardware removal, with the exception of one patient who underwent below knee amputation. Staphylococcus aureus was identified in 17 patien …

Posterior olecranon plating: biomechanical and clinical evaluation of a new operative technique.

The purpose of this investigation was to compare the biomechanical analysis of a new plating technique for olecranon fractures to tension band wiring, and review early clinical results. Six matched pairs of cadaveric ulnae were used for the biomechanical analysis. A transverse osteotomy of the mid part of the olecranon was made. One ulna of each pair was stabilized using a tension band and the other with a posterior hook plate. The ulnae were mounted and loaded, and displacement at the osteotomy site recorded. Twenty patients treated with this new technique (14 fractures and 6 osteotomies) wer

Management of ACL Reconstruction Complicated by MRSA Osteomyelitis Requiring a Gastrocnemius Perforator Flap: A Case Report.

CASE: A 21-year-old woman presented with sepsis from methicillin-resistant Staphylococcus aureus, patellar osteomyelitis, and 6 × 4 × 2-cm proximal anteromedial soft-tissue defect 1 week after bone-tendon-bone autograft anterior cruciate ligament reconstruction (ACL-R). She underwent multiple irrigation and debridement (I&Ds), hardware removal, bone grafting with antibiotic-loaded tricalcium phosphate beads, and medial gastrocnemius perforator flap with plastic surgery. At 18 months after her initial ACL-R, single-stage revision ACL-R with quadriceps autograft was performed. CONCLUSION: When …

Outcomes and complications of exposed Kirschner wire fixation for metacarpal fractures: a retrospective study.

AIM: Metacarpal fractures are common hand injuries, frequently treated with closed reduction and internal fixation (CRIF) using Kirschner wires (K-wires). While exposed K-wires allow for simple hardware removal and early mobilization, their use is traditionally associated with a higher risk of pin tract infections. There is a lack of studies focusing exclusively on exposed K-wires in metacarpal fractures. METHODS: It was retrospectively reviewed 134 patients who underwent CRIF with exposed K-wires for metacarpal fractures between January 2023 and March 2024. After applying inclusion and exclu…

Direct pedicle screw fixation in infected vertebrae for acute pyogenic spondylitis: short-term outcomes from a single-arm retrospective study.

BACKGROUND: The safety and efficacy of placing pedicle screws within infected vertebral bodies during surgery for acute pyogenic spondylitis remains controversial. The conventional "skip-level" fixation strategy avoids instrumentation at the infected site but may compromise spinal biomechanical stability and alignment. Conversely, fixation that includes the infected vertebrae can provide immediate stability, facilitate deformity correction, and promote bone fusion, although it carries potential risks of infection recurrence and implant failure. OBJECTIVE: To describe the short-term outcomes o

Initial Displacement Does Not Affect Loss of Reduction After Lisfranc Fracture Dislocations.

We sought to determine whether significantly displaced Lisfranc injuries had a higher rate of loss of reduction after fixation compared to those with low displacement. Forty-five patients who had fixation of an acute Lisfranc injury from 2005 to 2014 were retrospectively reviewed. All patients underwent subsequent hardware removal at 3 to 15 months after their index procedure. Radiographs were reviewed from the time of injury, 2 weeks, and 6 months postoperative. Lisfranc joint reduction was assessed using criteria previously described in the literature and graded as malreduced if displacement…

Deep Infections After Syndesmotic Fixation With a Suture Button Device.

Suture button devices such as the TightRope (Arthrex, Naples, Florida) have been increasingly used for syndesmotic fixation of ankle fractures. Despite proposed advantages, prior studies have shown equivalent outcomes, with a theoretical decreased need for removal of hardware. Complications of suture button fixation of syndesmotic instability may be underreported and include lateral suture knot inflammation with or without granuloma formation, infection, aseptic osteolysis with widening of the tibial drill tunnels, heterotopic ossification, and osteomyelitis. In this case series, the authors r

Hardware-preserving management of early soft-tissue breakdown over stable osteosynthesis: a retrospective single-centre experience.

This study evaluates whether a hardware-preservation strategy-restoration of soft tissue coverage over exposed but mechanically stable osteosynthesis combined with systemic antibiotic therapy aimed at limiting progression from contamination to established infection-can represent a viable alternative to hardware removal in selected patients with orthopaedic hardware exposure without overt clinical signs of infection and with negative superficial wound swabs. A retrospective analysis at Circolo and Macchi Foundation Hospital (January 2018-January 2023) examined all cases of soft tissue breakdown…

Retrograde headless compression screw fixation of olecranon stress fractures in throwing athletes: a novel technique.

BACKGROUND: An olecranon stress fracture (OSF) is a rare injury most commonly seen in high-level overhead throwing athletes with no clear consensus on surgical treatment. The most common surgical treatment described in the literature is cannulated screw fixation but there have been high rates of reported hardware irritation and need for subsequent hardware removal. HYPOTHESIS/PURPOSE: This study describes a novel surgical technique in the treatment of OSFs in high-level throwing athletes using retrograde headless compression screws. We hypothesized that patients would have excellent outcomes …

Streptococcus dysgalactiae-related intrathecal baclofen therapy infection: how to avoid withdrawal?

Intrathecal baclofen therapy is commonly used for neurologically spastic patients. One of the major complications is hardware infection, which generally requires urgent removal of the pump and the intrathecal catheter, with the risk of severe baclofen withdrawal. We have recently been facing this situation and propose another solution with adapted antibiotic therapy, removal with immediate replacement of the intrathecal catheter, initially connected to an implanted port to continue baclofen administration. A new pump was secondarily implanted, after successful treatment of acute bacterial meni

Long-term implant retention after impacted elastic stable intramedullary nailing in pediatric diaphyseal forearm fractures: a retrospective cohort study.

BACKGROUND AND PURPOSE: In elastic stable intramedullary nailing (ESIN) for forearm fractures, nail protrusion may cause irritation-related implant removal. The primary aim was to evaluate implant retention after nail impaction, and secondarily to assess secondary procedures, complications, and protrusion as a predictor of removal. METHODS: We analyzed a retrospective cohort of children with diaphyseal forearm fractures treated with impacted ESIN between 2017 and 2024. Implant retention was defined as no nail removal at record review. Secondary procedures were unplanned operations, while comp

Salvage of the infected Nuss bar: a protocol and institutional experience.

BACKGROUND: Surgical implant infections often necessitate early hardware removal, complicating patient care. Nuss bars are placed via minimally invasive surgery to correct pectus excavatum. While rare, Nuss bar infections may be salvaged with prolonged antibiotics. However, no standardized protocols exist to guide management and prevent premature removal. METHODS: This study describes the implementation of a Nuss bar infection salvage protocol. This protocol was developed and implemented for six adolescent patients who underwent minimally-invasive Nuss bar implantations for pectus excavatum an…

Treatment of subtrochanteric nonunion of the femur: whether to leave or to exchange the previous hardware.

OBJECTIVE: Our aim was to compare the clinical results of two different methods for treatment of subtrochanteric nonunion: an additional fixation to the previous hardware, and exchange of previous hardware. METHODS: We retrospectively analyzed 19 cases of subtrochanteric nonunion, and compared clinical results from 10 cases (Group 1) with exchange of the previous hardware, and 9 cases (Group 2) with retained previous hardware and an additional fixation. Autogenous bone grafting were performed for all cases. The mean age of the patients was 49.1 years in Group 1, and 48.2 years in Group 2. The
