Qualitative Exploration of Healthcare Providers' Perspectives on HIV Service Vulnerability During Funding Disruption in Nigeria.

Nigeria's 2025 PEPFAR funding suspension served as an unplanned natural experiment, uniquely demonstrating that behavioral instability among patients and providers precedes, and is in many respects more disruptive than, actual commodity shortages. This distinction, rarely documented in prior literature, has direct implications for how HIV program resilience should be designed and measured. We conducted in-depth key informant interviews with 15 healthcare providers at a major antiretroviral therapy (ART) facility in northern Nigeria, analyzed using Braun and Clarke's reflexive thematic analysis

A scoping review on the availability and utilization of essential opioid analgesics in Sub-Saharan Africa.

BACKGROUND: Management of acute, post-operative, and chronic pain requires access to and availability of opioids. While often oversupplied in high-income countries, significant shortages exist in low- and middle-income countries. We conducted a scoping review on availability and usage of opioids in Sub-Saharan Africa (SSA). METHODS: The five-stage approach of Arksey and O'Malley (2005) was used. MEDLINE via PubMed, EMBASE, and SCOPUS were search and results categorized into themes: 1) Local/regional availability and supply, 2) Consumption patterns, 3) Legislation and policy, 4) Costs and fina

Towards universal health coverage in Vietnam: a mixed-method case study of enrolling people with tuberculosis into social health insurance.

BACKGROUND: Vietnam's primary mechanism of achieving sustainable funding for universal health coverage (UHC) and financial protection has been through its social health insurance (SHI) scheme. Steady progress towards access has been made and by 2020, over 90% of the population were enrolled in SHI. In 2022, as part of a larger transition towards the increased domestic financing of healthcare, tuberculosis (TB) services were integrated into SHI. This change required people with TB to use SHI for treatment at district-level facilities or to pay out of pocket for services. This study was conducte

COVID-19 pandemic experiences of parents caring for children with oesophageal atresia/tracheo-oesophageal fistula.

PURPOSE: The COVID-19 pandemic has resulted in a global health crisis of unparalleled magnitude. The direct risk to the health of children is low. However, disease-containment measures have society-wide impacts. This study explored the pandemic experiences of parents of children with oesophageal atresia/tracheo-oesophageal fistula (OA/TOF) in the UK. DESIGN: A phenomenological approach underpinned use of an asynchronous online forum method, in collaboration with a patient support group. Data were evaluated using thematic analysis. RESULTS: The online forum ran between 7 November and 18 Decem

Transient gestational hypertension and pre-eclampsia: Two case reports and literature review on the need for stringent monitoring.

Transient gestation hypertension is a contributor to adverse pregnancy outcomes particularly when it progresses to pre-eclampsia (PE). This requires frequent monitoring. We illustrate the need for stringent monitoring of gestational hypertension, transient gestational hypertension (TGH) and PE without severe features and conducted a brief rapid review of the literature. Two cases are presented: Firstly, a 25-year-old primigravida at 30 gestational weeks who had an isolated TGH with high blood pressure (BP) of 141/87 mmHg, which was not investigated. Four weeks later, she presented with a BP of

What it Takes to Implement Population-Based Genomic Screening: A Multi-Site Qualitative Study of Implementation Determinants Across Health Systems.

BACKGROUND: Population-based genomic screening (PGS) holds promise for identifying individuals at elevated risk for hereditary conditions. However, the absence of implementation guidance limits the scalability and impact of these programs. This qualitative study aimed to identify facilitators and barriers to PGS implementation among healthcare settings at different phases of PGS implementation. METHODS: We conducted qualitative interviews with implementation team members, including genetic counselors, physicians, information technology and informatics personnel, study coordinators, and instit

Demographic and service-use profiles of individuals using the CarePayment program for hospital-related medical debt: results from a nationwide survey of guarantors.

BACKGROUND: Many Americans find themselves with problems paying medical bills, and medical debt can lead to numerous negative financial, social and access to healthcare outcomes. One potential market-based solution to these challenges is to provide financing options that have patient-friendly terms while complying with increasingly complex federal lending regulations. CarePayment (CP) is one entity that provides zero interest financing to individuals from participating medical facilities. An independent, initial outcome study was undertaken to understand the demographic and medical debt-relate

Longitudinal Prevalence of Financial Worry in a Cohort of Patients with Advanced Colorectal Cancer: A Secondary Observational Cohort Study.

IntroductionFinancial hardship during cancer treatment is common in privatised healthcare systems and has been extensively studied in cancer survivorship groups. The experience of financial concerns by people living with advanced, incurable cancer has been less frequently explored. This paper sought to describe the proportion of patients experiencing financial worry longitudinally, in a cohort with advanced colorectal cancer, in a publicly funded healthcare system.MethodsThis secondary analysis of a prospective, observational cohort study 'Palliative Care Early and Systematic (PaCES)' project,

Decision-Making Competence in Patients with Alzheimer's Disease: A Review of the Literature.

Decision-making competence (DMC) appears to be influenced by the congruency between the characteristics of the individual, the task and the context. Indeed, the ability to make decisions seems to be highly sensitive to cognitive changes as observed, in particular, in the healthy elderly. Few studies have investigated these relations in pathological ageing. In this review, we focus on DMC in patients with Alzheimer's disease (AD) and the links its impairment could have with deficits in episodic memory, working memory, and executive functions. Decision-making under risk and under ambiguity appea

Supporting Youth Aging Out of Foster Care.

Over 400,000 children are in foster care in the United States, and more than 100,000 of them are waiting to be adopted. Yet many will age out of foster care into adulthood without an adoptive family. Teens and young adults aging out of foster care, even those with preparation and training for the transition, often do not fare well in young adulthood. Many face challenges in areas of education, employment, homelessness, finances, the criminal justice system, and meeting health and mental healthcare needs. Research demonstrates what only makes sense: teens with tangible support from meaningful a

Perspectives on primary care management of chronic hepatitis B in refugees: findings from qualitative interviews with healthcare providers in Melbourne, Victoria.

BACKGROUND: In Australia, healthcare providers face significant challenges in meeting the niche health needs of refugees. Social determinants of health, including limited health literacy and finances, complicate refugees' access to treatment for conditions, such as chronic hepatitis B (CHB). Understanding the strategies utilised by a specialised general practice for refugees can offer guidance for treating this population. The aim of this study was to explore how primary care is currently managing CHB care tailored to refugees and to identify areas for potential improvements. METHODS: Between

Establishing an independent HTA agency in Ukraine: a conceptual framework for governance, operations, and long-term sustainability.

OBJECTIVES: Health technology assessment (HTA) has become an integral part of Ukraine's health system since its formal introduction into national legislation in 2017. By 2020, HTA was mandated for evaluating publicly funded medicines, laying the groundwork for more evidence-based healthcare decisions. Although the creation of an independent HTA agency was initially planned for 2022, implementation was delayed due to the COVID-19 pandemic and Russia's ongoing invasion. The relevant Cabinet Resolution calls for the establishment of an autonomous agency by January 2026. This commentary outlines a

The economic burden of hemodialysis and associated factors of among patients in private and public health facilities: a cross-sectional study in Addis Ababa, Ethiopia.

INTRODUCTION: The treatment of kidney disease, including hemodialysis, poses challenges in healthcare and finances. Despite limited data on hemodialysis costs and determinants in Ethiopia, existing literature indicates a paucity of evidence regarding the economic burden of hemodialysis. This study aims to evaluate the direct and indirect costs of hemodialysis among end-stage renal disease (ESRD) patients, alongside associated factors, among selected governmental and private institutions in Addis Ababa, Ethiopia. METHODS: An institutional-based cross-sectional study using a simple random sampl

Facilitators, barriers, and guidance to successful implementation of multidisciplinary transitional care interventions: A qualitative systematic review using the consolidated framework for implementation research.

BACKGROUND: Multidisciplinary transitional care interventions aim to improve the coordination and continuity of healthcare during hospitalization and after discharge for patients with complex care needs related to physical, nutritional, or psychosocial status. Implementing such interventions is complex as they involve many stakeholders across multiple settings. Numerous studies have evaluated patients', family members', and healthcare professionals' experiences with multidisciplinary transitional care interventions, which can provide insight into facilitators and barriers to their implementati

Epidemiology and Economic Burden of Puumala Virus Infection in Styria, Austria, 2003 to 2024.

BACKGROUND: Hantavirus infections cause hemorrhagic fever with renal syndrome and impose a potential burden on healthcare systems. However, comprehensive economic data remain scarce. We quantified the inpatient economic burden of hospitalized patients with Puumala virus (PUUV) infections in Styria, Austria's primary endemic region, to identify hospitalization costs and early admission predictors of costs. METHODS: This retrospective cost-of-illness study analyzed 1049 serologically confirmed PUUV hospitalizations across 20 public hospitals in Styria, Austria between September 2003 and March 2

Optimizing Intermittent Catheterization Following Spinal Cord Injury: A Narrative Review Using the WHO Health System Building Blocks Framework.

OBJECTIVES: This narrative review examines intermittent catheterization (IC) for individuals with spinal cord injury (SCI) from a health systems perspective. Although IC is considered the preferred standard of care for neurogenic bladder after SCI, its long-term effectiveness depends on healthcare systems' ability to provide integrated, sustainable, and patient-centered care. The review explores system-level challenges and strategies to optimize IC delivery using the World Health Organization (WHO) Health System Building Blocks framework. METHODS: A structured literature search was conducted

Dementia strategies in an aging society: Policies, care, and global insights from the Japanese experience.

Aging of the population has become a critical challenge globally. The proportion of individuals age 60 years and older is projected to increase from 12% in 2015 to 22% by 2050, representing more than 2.1 billion older adults globally. This demographic transition is advancing particularly rapidly in Japan, which has become the first nation to become a "super-aged society". Projections indicate that by 2060, the number of older adults living with dementia will reach approximately 6.45 million (more than 17% of the elderly population), making it one of the country's most urgent health and social
