Unscrambling hepatitis C virus-host interactions.

The human suffering exacted by the hepatitis C virus is enormous. Hundreds of thousands of people die each year from liver failure and cancer caused by this infection. There is no vaccine, and the available antiviral drugs are toxic, expensive and only partly effective. Progress has been hindered by the absence of cell culture and small-animal models of the infection. Nonetheless, recent advances have yielded several promising new antiviral drugs and enhanced the prospects of developing a vaccine. The recent development of a robust in vitro hepatitis C virus infection system will aid this sear…

[Prevention of hepatitis B and C].

Preventive measures are determined by epidemiological characteristics of HBV and HCV infections. A significant difference between these two infections is in the availability of effective vaccine for hepatitis B. Vaccine against hepatitis C is not available. The vaccination program for hepatitis B in Croatia was initiated in 1994, at first only for high risk groups. The vaccination program for children in the 6th grade of primary school started in 1999. Besides that, all pregnant women are obliged to undergo HBV testing. HBV infection is very rare but not unknown in early childhood. HBV infecti…

Hepatitis C virus and lichen planus.

Hepatitis C virus (HCV) is an important factor in the development of chronic liver disease and hepatocellular carcinoma. In recent years it has become known that HCV induces various extrahepatic manifestations including mixed cryoglobulinemia, membranoproliferative glomerulonephritis, Sjögren's syndrome, autoimmune thyroiditis, malignant lymphoma, porphyria cutanea tarda and lichen planus. Although the mechanisms of extrahepatic manifestations remain unclear, it is known that interferon (IFN) therapy and coadministration of IFN with ribavirin are effective in promoting the disappearance or all…

Chronic hepatitis C in Israeli children.

Natural history, epidemiology, and histopathological features of chronic hepatis C (CHC) are well established in adults. Data on histopathological findings of CHC in children are still limited and controversial. We aimed to evaluate the histopathological features of CHC in children in Israel. We reviewed, retrospectively, 20 liver specimens from 20 children with CHC for inflammation and fibrosis, hepatocyte necrosis, fatty changes, cholestasis, bile duct damage, sinusoidal lymphocytosis, and glycogen storage. The most common histological feature was portal inflammation (95%) and lobular inflam

Lymphoproliferative disorders in chronic hepatitis C.

Chronic hepatitis C virus (HCV) infection is associated with the development of lymphoproliferative disorders (LPDs). The aim of this investigation was to determine the prevalence and characterization of monoclonal gammopathy and benign and malignant LPDs in individuals with chronic hepatitis C. A total of 233 subjects diagnosed with chronic hepatitis C (male/female ratio: 131/102, median age; 49 years) were studied. Serum and urine were examined for the presence of a monoclonal gammopathy. A bone marrow aspirate and biopsy was obtained in individuals with a monoclonal gammopathy. Thirty-two p…

[Acute hepatitis C--an unresolved issue].

INTRODUCTION: The problem encountered on collecting a large number of cases of acute hepatitis C (AH-C) cases are the reduction in the incidence of new infections, clinically mild or completely asymptomatic disease, and absence of specific diagnostic tests to identify acute HCV infection and to distinguish it from acute exacerbation of chronic hepatitis C (CH-C). AIM, PATIENTS AND METHODS: The aim of this paper is to give an overview of 108 patients with AH-C hospitalized at Viral Hepatitis Unit, University Hospital of Infectious Diseases, during a 13-years period. Anti-HCV testing was starte…

Hepatitis C virus (HCV) and lymphomagenesis.

Hepatitis C virus (HCV) is the major cause for non-A, non-B hepatitis. Most HCV-infected individuals do not clear the virus resulting in a chronic infection that may potentially lead to liver cirrhosis and hepatocellular carcinoma. In addition to hepatic manifestations, HCV infection is associated with B cell lymphoproliferative disorders, including mixed cryoglobulinemia, usually a benign condition, and overt B cell lymphoma. A direct role of HCV infection in the genesis of these B cell lymphoproliferative disorders has been suggested initially by epidemiological studies and is supported by r…

Extrahepatic manifestations of hepatitis C infection.

Hepatitis C virus (HCV) affects 1.8% of the American population, and approximately 38% of patients with HCV will manifest symptoms of at least 1 extrahepatic manifestation during the illness. Renal disease, neuropathy, lymphoma, and Sjögren syndrome with or without mixed cryoglobulinemia are all strongly associated with HCV infection. Porphyria cutanea tarda and diabetes have also been linked to HCV. Most extrahepatic manifestations of chronic HCV infection are immunological, and the chronic infection seems to be necessary for their development. The molecular study of the unique way in which t…

Hepatitis C virus-associated tubulointerstitial injury.

BACKGROUND: Tubulointerstitial damage is recognized as a determinant of the prognosis of kidney disease. Various types of viral infection have been reported to induce tubulointerstitial lesions; however, that caused by hepatitis C virus (HCV) remains unclear, although glomerular lesions caused by this viral infection have been well documented. METHODS: To identify any association, we retrospectively investigated 320 patients who underwent renal biopsy and did not have extrarenal diseases causing tubulointerstitial nephritis. RESULTS: Of these patients, 13 patients had HCV infection and 307 p…

Antiviral treatment in acute hepatitis C.

BACKGROUND/AIMS: Several treatment schedules have been tried in acute hepatitis C, but due to the heterogeneity of included patients and to the non-practical interferon protocols, we assessed prospectively in a non-randomized pilot study, the efficacy of interferon 2b. METHODOLOGY: Five million units of interferon 2b were given three times weekly by subcutaneous route for 6 months as soon as diagnosis of acute hepatitis C was established. RESULTS: In 6 jaundiced patients, we have achieved a rate of 83% of sustained response, for at least one year, with normalization of ALT and clearance of H…

Chronic hepatitis C: updated Swedish consensus.

In 1999 a Swedish national expert panel published recommendations for the treatment of chronic hepatitis C (HCV) infection. Recently, pegylated interferon (peg-IFN) products have been introduced, and an increased knowledge concerning treatment of acute HCV and HCV-human immunodeficiency virus (HIV) coinfection has been gained. As a result of this, an update of the Swedish recommendations was developed following an expert meeting in October 2002. The panel now recommends the use of peg-IFN together with ribavirin as the standard treatment. Owing to the excellent response rates in HCV genotype 2…

Sarcoidosis-associated hepatitis C virus infection.

Although several reports of sarcoidosis have been reported in hepatitis C virus (HCV)-infected patients treated with interferon-alpha, this association has never been described in nontreated HCV patients. We report two cases of sarcoidosis associated with chronic hepatitis C infection. The patients developed multivisceral sarcoidosis (cutaneous, lungs, nodes) at two and at least six years after the presumed date of infection. One patient obtained remission of sarcoidosis with corticosteroid treatment but the other remained corticodependent. The levels of hepatic enzymes were not significantly …

Prevention of spread of hepatitis C.

Hepatitis C virus (HCV) is transmitted by percutaneous or permucosal exposure to infectious blood or blood-derived body fluids. Based on the results of cohort and acute case control studies, risk factors associated with acquiring HCV infection in the United States have included transfusion of blood and blood products and transplantation of solid organs from infected donors, injecting drug use, occupational exposure to blood (primarily contaminated needle sticks), birth to an infected mother, sex with an infected partner, and multiple heterosexual partners. Nosocomial and iatrogenic transmissio…

Hepatitis C virus related cryoglobulinemic glomerulonephritis.

A 46 years male presented with skin rash and acute nephritic syndrome. He had history of jaundice four months back. Rheumatoid factor and cryoglobulins were present in the serum. Although anti-HCV antibodies were negative, HCV RNA was detected by polymerase chain reaction. Kidney biopsy showed membranoproliferative glomerulonephritis and thrombi in the lumen of the glomerular capillary loops. His renal functions improved with steroids.
