A 56-year-old woman presented for evaluation of a right thyroid nodule causing dysphagia. Thyroid function tests were normal. Ultrasound showed an isoechoic right thyroid nodule measuring 3.6 × 2.1 × 3.5 cm. After fine needle biopsy demonstrated benign pathology, ultrasound-guided radiofrequency ablation (RFA) of the nodule was performed. We report an exemplary case of acquired Horner syndrome secondary to neuroblastoma in infancy. The patient presented with ptosis, miosis, and heterochromia. In reviewing the patient's laboratory and imaging workup, we highlight key etiologic differences between the pediatric and adult populations. Other important teaching points included in the discussion are a review of sympathetic neuroanatomy and oculosympathetic paresis, the appropriate and evidence-based diagnostic workup in infants and children, and a review of pharmacologic testing using cocaine and apraclonidine drops.

Horner syndrome is a rare but likely underdiagnosed complication of tonsillary surgery. The authors report the case of a 2.5-year-old boy who developed miosis and mild ptosis after radiofrequency tonsil ablation surgery. Pharmacologic testing with apraclonidine 0.5% eye drops confirmed left Horner syndrome. This case highlights the close anatomical relationship between the tonsils and cervical sympathetic trunk and draws attention to the occurrence of Horner syndrome after tonsillectomy. Reversal of anisocoria following instillation of apraclonidine 0.5% has been reported in Horner syndrome caused by lesions of the central and peripheral nervous system. Early disseminated Lyme disease can be difficult to diagnose because of atypical symptoms and physical findings. A clinical diagnosis must be made in the absence of confirmatory serologic testing to allow timely therapy. A 47-year-old woman presented with headache, acute monocular vision loss, and ipsilateral Horner syndrome. Apart from the optic neuropathy, all cranial nerve function was intact. Magnetic resonance imaging revealed an enlarged pituitary gland with compression of the orbital apex. The surgical specimen was consistent with pituitary apoplexy. The combination of headache, acute visual loss, and ipsilateral Horner syndrome without ophthalmoplegia, which may suggest carotid artery dissection, is evidently an unusual manifestation of pituitary apoplexy.

PURPOSE: To report a case of Horner syndrome that occurred after implantation of a vagus nerve stimulator.
METHODS: Case report.
RESULTS: A 6-year-old female with cerebral dysgenesis and intractable partial seizures presented with Horner syndrome after vagus nerve stimulator implantation.
CONCLUSION: Horner syndrome can occur as a result of the vagus nerve stimulator implant procedure and should be included as one of its possible surgical complications.

PURPOSE: To evaluate if an ocular sympathetic lesion may be a prerequisite for cluster headache to occur.
METHODS: Patients with a permanent Horner-like syndrome at the debut of cluster headache were identified from history and photographs.
RESULTS: Among 57 patients with cluster headache, seven patients (5 males and 2 females) exhibited a permanent Horner-like syndrome on the painful side when first seen at the outpatient department.
