Palliative needs for heart failure or chronic obstructive pulmonary disease: Results of a multicenter observational registry.

BACKGROUND: Heart failure (HF) and chronic obstructive pulmonary disease (COPD) share a common organ failure trajectory marked by prognostic uncertainty, which is a barrier to appropriate provision of palliative care. We describe in a prospective cohort from specialist hospital services the epidemiology and late clinical course of these chronic diseases to trace criteria for transition to palliative care in the community. METHODS AND RESULTS: Seven centers enrolled 267 patients with advanced HF (n=174) or COPD (n=93) using common (multiple hospitalizations or severely impaired functional stat

Beyond prescriptions: an analysis of patient journey, self-management adherence, and unmet needs in type 2 diabetes.

BACKGROUND: As a chronic disease, effective management of Type 2 Diabetes Mellitus (T2DM) heavily relies on people’s continuous self-management adherence. Patient journey mapping provides a powerful perspective for systematically understanding individuals’ complete experiences within the healthcare system, identifying key touchpoints, and uncovering unmet needs. However, research exploring the factors influencing adherence among people with T2DM from this perspective remains insufficient, particularly lacking integrated exploration from the perspective of pharmaceutical services to systematica

Changes in ECT services in India since the implementation of its mental health care act 2017: a clinician's perspective.

BACKGROUND: The large treatment gap in mental disorders in India also includes a lack of access to electroconvulsive therapy (ECT), a potentially lifesaving treatment. With the introduction of the Mental Health Care Act (MHCA) 2017 in India, an impact was speculated on the utilisation of ECT due to the ban on unmodified ECT and the regularisation of ECTs in children. METHODS: We planned this survey to identify the current status of ECT services in India as perceived by psychiatrists and to explore the barriers to providing ECT, particularly those influenced by the enactment of the MHCA 2017 i

Organizational characteristics of solid-organ donor hospitals and nondonor hospitals.

CONTEXT: Efforts to increase organ donation include serious attempts in hospital settings, where unrealized donation potential exists. Research on hospital donation must include understanding organizational as well as patient-specific influences on the donation process. OBJECTIVE: To identify organizational characteristics that distinguish hospitals producing organ donations from those that do not, and to estimate the number of nondonor hospitals with donor potential. DESIGN: Data from the American Hospital Association's 1992 annual survey of hospitals were matched to Organ Procurement and T

Patient Outcomes and Nurse Understaffing in For-Profit Hospitals.

BACKGROUND: For-profit hospital ownership has been associated with worse patient outcomes, but the mechanisms remain unclear. Underinvestment in nurse staffing is a plausible pathway linking for-profit ownership to performance. OBJECTIVES: To examine whether differences in nurse staffing mediate associations between for-profit hospital ownership and patient, nurse, and hospital outcomes. DESIGN: Cross-sectional study using 2024 data on hospital ownership, registered nurse survey responses, Medicare claims, and Hospital Compare measures. SUBJECTS: A total of 143 for-profit and 798 nonprofit

Ambulance location and relocation under budget constraints: investigating coverage-maximization models and ambulance sharing to improve emergency medical services performance.

Ambulance location in Emergency Medical Services (EMS) is a widely studied problem requiring efficient resource allocation within budgetary constraints. The literature has focused on enhancing EMS performance with limited attention given to their economic performance. This study addresses EMS performance with an emphasis on budget constraints by revising three coverage maximization models: the time-dependent Maximum Expected Coverage Location Problem (time-dependent MEXCLP), the multi-period Double Standard Model (mDSM), and the multi-period Queueing Maximal Availability Location Problem (Q-MA

Who is protected and who is not? Evaluating the effects of the long-term care insurance system and disparities in healthcare utilization and costs of the South Korean elderly population.

BACKGROUND: There is a growing global interest in investigating the substitutability between long-term care service (LTCS) and medical care use, particularly in countries where elderly medical care use is rapidly increasing. The use of an appropriate LTCS can reduce unnecessary medical utilization and the resulting costs such as those for hospital admission, delayed discharge, and emergency room visits. METHODS: This study evaluated whether the LTCS has a savings effect on medical use for older adults and whether the effect varies depending on income level, using longitudinal data obtained fr

Strengthening evidence-based practice culture in nursing: An ethnographic study in the central highlands of Vietnam.

BACKGROUND: Evidence-based practice (EBP) is a vital approach to achieve Quality Improvement (QI) in healthcare services by ensuring that nursing care is grounded in the best available evidence. However, the integration of EBP into nursing in Vietnam is often lacking, presenting a significant challenge for both healthcare leaders and individual practitioners. OBJECTIVES: This qualitative study aimed to understand the workplace culture regarding EBP implementation among Vietnamese nurses, exploring the shared beliefs, values, and contextual factors that influence EBP within a tertiary hospital

Prognostic factors associated with failure of total elbow replacement: a protocol for analysis of National Joint Registry data in England.

INTRODUCTION: Understanding the prognostic factors associated with the failure of total elbow replacement (TER) is crucial for informing patients about risks and enabling shared decision-making regarding TER as a definitive management option. This protocol outlines the planned analysis of National Joint Registry (NJR) data to investigate prognostic factors for TER failure. METHODS AND ANALYSIS: The primary analysis will use the NJR elbow dataset, including all eligible patients who underwent TER surgery between April 2012 and December 2023. To incorporate ethnicity and comorbidities as potent

What factors contribute to e-scooter crashes: A first look using a naturalistic riding approach.

INTRODUCTION: Shared dockless electric scooters (e-scooters) are a popular shared mobility service providing an accessible last-mile transportation option in urban and campus environments. However, city and campus stakeholders may hesitate to introduce these scooters due to safety concerns. While prior e-scooter safety studies have collected injury data from hospitals or riding data under controlled or naturalistic conditions, these datasets are limited and did not identify risk factors associated with e-scooter riding safety. To address this gap in e-scooter safety research, this study collec

Hospice and hematology - Barriers to hospice access for patients with hematologic malignancies.

Hospice care is known to enhance quality of life (QoL) for patients with advanced cancer, yet individuals with hematologic malignancies are disproportionately underrepresented in hospice settings at the end of life (EoL). To explore the reasons for this disparity, a systematic search across four databases identified 19 relevant publications, including qualitative, quantitative, and mixed-methods studies. Several key barriers to hospice referral were identified: unpredictable disease trajectories, strong patient-hematologist relationships, prognostic uncertainty, and widespread misunderstanding

AI co-pilot bronchoscope robot.

The unequal distribution of medical resources and scarcity of experienced practitioners confine access to bronchoscopy primarily to well-equipped hospitals in developed regions, contributing to the unavailability of bronchoscopic services in underdeveloped areas. Here, we present an artificial intelligence (AI) co-pilot bronchoscope robot that empowers novice doctors to conduct lung examinations as safely and adeptly as experienced colleagues. The system features a user-friendly, plug-and-play catheter, devised for robot-assisted steering, facilitating access to bronchi beyond the fifth genera

Perioperative Care of Children with Severe Neurological Impairment and Neuromuscular Scoliosis-A Practical Pathway to Optimize Peri-Operative Health and Guide Decision Making.

Neuromuscular scoliosis is a common feature in children with severe neurological impairment (SNI), including those with severe cerebral palsy. Surgical correction of scoliosis is the mainstay of treatment. This group of patients also have associated medical complexity. The complication rates post-surgery are high, although, for many, they are worth the risk. There are currently no published practice guidelines or care pathways for children with SNI who are undergoing scoliosis corrective surgery. In response to the high uptake of this surgery, coupled with the expected complication rates, our

Facilitators and barriers to patient and public involvement in developing learning health systems within community health services: ascoping review.

BACKGROUND AND AIMS: Learning Health Systems (LHS) integrate research, clinical expertise, and patient data to continuously improve health care. Although community engagement is central to LHS, existing literature has largely focused on data infrastructure, clinical improvement, and system learning, with limited attention to meaningful patient and public involvement (PPI) in community-based care. This review aimed to identify facilitators and barriers to such involvement in the development of LHS in community health services. METHODS: We conducted a scoping review in line with the JBI methodo

Patient-centered care and associated factors among outpatient attendees in primary hospitals in South Gondar Zone, Northwest Ethiopia: a mixed methods study.

BACKGROUND: According to the Picker Institute model, patient-centred care (PCC) includes respect for patients’ values, preferences, and expressed needs; information, communication, and education; access to care; emotional support; involvement of family and friends; continuity and transition; physical comfort; and coordination and integration of care. It is one of the six aims for quality improvement guiding today’s health care systems. However, PCC remains underutilized by both patients and professionals, and significant challenges persist in its implementation worldwide. Additionally, few stu

Skilled Nursing Facility Population Characteristics and Length of Stay: A Prospective Cohort Study.

OBJECTIVES: Skilled nursing facilities (SNFs) offer short-term, subacute care following injury or hospitalization, with Medicare fully covering stays ≤20 days. As the US population ages, SNF demand is increasing, yet this population remains poorly characterized. Prolonged stays increase pathogen exposure, with shared spaces and visitors potentially elevating viral transmission. We estimated the prevalence of self-reported respiratory symptoms, shared space use, and visitor frequency among SNF residents and their associations with length of stay (LOS). DESIGN: Prospective cohort study. SETTIN
