Percutaneous spinopelvic fixation technique using external fixation for focal kyphosis reduction in U-type sacral fractures: a case report.

BACKGROUND: Sacral fractures, particularly U-type fractures characterized by a transverse fracture line, result in significant instability and deformity, including focal kyphosis. These fractures challenge biomechanical integrity and neural structures, often leading to long-term disability if not corrected. Surgical approaches vary, but percutaneous spinopelvic fixation offers benefits like reduced soft tissue trauma and expedited recovery. This case report highlights managing a displaced U-type sacral fracture with focal kyphosis using a percutaneous spinopelvic reduction technique followed b

Autism and Homelessness: Risk and Protective Factors in a Bayesian Analysis of Autistic Characteristics.

Autistic individuals are overrepresented in homeless populations, yet mechanisms driving this association remain unclear, particularly in the context of diagnostic access and structural inequities. We surveyed 333 adults in Australia, assessing autistic traits (Autism-Spectrum Quotient [AQ]-50), homelessness risk factors, adaptive functioning and social support. Bayesian hierarchical logistic regression models were used to estimate associations. Associations between autistic traits and homelessness probability differed according to diagnostic status. Among participants without a formal diagnos

Pharmacist-led community network approach addressing social needs and adherence (Connect AF).

BACKGROUND: Social determinants of health (SDOH) are nonmedical factors that impact health outcomes and medication use. As national initiatives and value-based care models prioritize SDOH interventions, pharmacists are uniquely positioned to support these efforts. However, evidence describing pharmacist-led SDOH screening, particularly through collaborations between health-system and community pharmacists, remains limited. OBJECTIVE: To learn how community pharmacists address SDOH-related care gaps among patients with atrial fibrillation (AF) and how vertically integrated partnerships can str

Living in a block group with a higher eviction rate is associated with increased odds of preterm delivery.

BACKGROUND: Housing instability is associated with adverse pregnancy outcomes. Recent studies indicate that eviction, which may affect a larger segment of the population than other forms of housing instability, is also associated with adverse pregnancy outcomes. However, these studies evaluate eviction across large areas, such as counties, so it remains unclear whether these patterns extend to individual-level pregnancy outcomes. METHODS: We used data on a cohort of all singleton live births at a single Chicago hospital between March 2008 and March 2018 to investigate the associations between

Lessons Learned From Implementing a Caring Contacts Clinical Practice Guideline Recommendation.

The U.S. Department of Veterans Affairs (VA) and Department of Defense clinical practice guideline on the treatment of veterans at risk for suicide recommends considering caring contacts interventions after a psychiatric hospitalization for suicidal ideation or suicide attempt. This quality improvement project examined the implementation of the recommendation at a large VA health care system. The project enrolled 29% of hospitalized veterans (N=135 of 462). Enrollment barriers included lack of staff availability and veteran ineligibility due to homelessness or housing instability. Opportunitie

Needles in a Haystack: Screening and Healthcare System Evidence for Homelessness.

Effectiveness of screening for homelessness in a large healthcare system was evaluated in terms of successfully referring and connecting patients with appropriate prevention or intervention services. Screening and healthcare services data from nearly 6 million U.S. military veterans were analyzed. Veterans either screened positive for current or risk of housing instability, or negative for both. Current living situation was used to validate results of screening. Administrative evidence for homelessness-related services was significantly higher among positive-screen veterans who accepted a refe

Sexual Abuse is Associated with Negative Health Consequences among High-risk Men who have Sex with Men.

This study evaluated whether a history of sexual abuse could differentiate negative health consequences among men who have sex with men (MSM; N=148) enrolled in a risk counseling program. Over half (51.4%) reported an experience of sexual abuse. A history of sexual abuse was associated with increased psychological distress, increased rates of alcohol (AOR = 2.91; p < .01) and/or drug abuse (AOR = 2; p < .01) treatment, increased risk of housing instability (AOR = 2.13; p < .05), and increased risk for suicidality (AOR = 4.3; p < .001). Findings demonstrate that screening for sexual abuse may b

Characteristic hostility in schizophrenic outpatients.

In this study of 133 schizophrenic outpatients, we assessed characteristic hostility and correlates of hostility over a 6-month period. Results showed that 13 percent of the study group were characteristically violent, 18 percent were characteristically threatening, and another 21 percent were irritable and argumentative. About half (48%) were without hostility. A multiple regression model identified six variables--housing instability, hallucinations or delusions, schizoaffective diagnosis, lack of depression, alcohol use, and bizarre behavior--that together accounted for over 50 percent of th

Ecological and sociodemographic determinants of house infestation by Triatoma infestans in indigenous communities of the Argentine Chaco.

BACKGROUND: The Gran Chaco ecoregion, a hotspot for Chagas and other neglected tropical diseases, is home to >20 indigenous peoples. Our objective was to identify the main ecological and sociodemographic determinants of house infestation and abundance of Triatoma infestans in traditional Qom populations including a Creole minority in Pampa del Indio, northeastern Argentina. METHODS: A cross-sectional survey determined house infestation by timed-manual searches with a dislodging aerosol in 386 inhabited houses and administered questionnaires on selected variables before full-coverage insectici

HIV, homelessness, and public health: critical issues and a call for increased action.

Homelessness and housing instability are significant public health issues that increase the risks of HIV acquisition and transmission and adversely affect the health of people living with HIV. This article highlights the contributions of selected papers in this special issue of AIDS and Behavior and considers them within the broader context of prior research on the associations between housing status and HIV risk, use of HIV medical care, adherence to HIV treatment, and the physical health of HIV-seropositive persons. Special recognition is given to the roles of interrelated health problems, s

Perspectives on the Low Demand Transitional Model in Engaging and Housing hard-to-reach Veterans Experiencing Unsheltered Homelessness.

Nationwide around 226,080 homeless individuals are unsheltered each night. The U.S. Department of Veterans Affairs (VA) Low Demand program is a community-based model which provides supportive transitional housing to hard-to-reach, unsheltered veterans who often have complex needs and may be unable or unwilling to participate in supportive services which aim to prevent or rapidly re-house veterans when facing housing instability or homelessness. The goal of this study was to examine Low Demand community providers' perceptions of the facilitators and barriers to Low Demand model engagement and e

Leveraging a digital health intervention to improve recovery outcomes among people with substance misuse experiencing housing insecurity.

BACKGROUND: People with substance use disorders (PwSUD) who experience housing insecurity have disproportionate overdose deaths. Digital interventions may improve care among PwSUD, but evidence specifically for housing-insecure PwSUD, who face unique barriers, remains limited. METHODS: We examined the efficacy of a mHealth tool (uMAT-R) among people who have misused substances and reported current housing insecurity. Substance use, cravings, access to basic needs, social disconnection, and digital literacy were assessed at baseline and 1 month and compared by uMAT-R use. Generalized estimatin

Developing a community-based multidisciplinary service exit typology for young adults with serious mental health conditions.

Objective: Increasingly, community mental health providers are implementing multidisciplinary treatment models blending child and adult evidence-based practices to boost young adult engagement. Although promising, limited research examines how and why young adults disengage from these new models. This study examines provider documentation of treatment discharges to create a more developmentally-attuned young adult service exit typology. Method: Service records of 18-25 year-olds (n = 124) who discharged from a young adult-tailored multidisciplinary treatment team over a 5-year period were anal

Intimate Partner Violence Exposure and Adolescent Mental Health Outcomes: The Mediating Role of Housing Insecurity.

The present study investigated pathways from childhood exposure to mothers' intimate partner violence (IPV) to adolescent depression and anxiety. Further, the study examined whether housing insecurity mediated the link from IPV exposure to mental health outcomes. Data came from the Years 5, 9 and 15 interviews of the Fragile Families and Child Well-being Study; the analytic sample was limited to mothers with at least partial custody of children (N = 2,425). Structural equation modeling with latent variables investigated research questions. Confirmatory factor analysis (CFA) identified observed

Domestic Violence Housing First Model and Association With Survivors' Housing Stability, Safety, and Well-being Over 2 Years.

IMPORTANCE: Intimate partner violence (IPV) is a leading cause of homelessness and a serious threat to public health and well-being. OBJECTIVE: To determine whether the Domestic Violence Housing First (DVHF) model improves safety, housing stability, and mental health over 2 years. DESIGN, SETTING, AND PARTICIPANTS: This longitudinal comparative effectiveness study interviewed IPV survivors and reviewed their agency records. All unstably housed or homeless IPV survivors entering domestic violence (DV) services were eligible to participate in the study, ensuring capture of typical variability

The Value of Social Determinants of Health Needs Assessments in Primary Care.

Social determinants of health (SDOH) such as income, housing, food security, education, transportation, and healthcare access, exert a far greater influence on health outcomes compared with traditional clinical care alone. This policy commentary argues for the routine integration of SDOH needs assessments into primary care to enable more comprehensive, equitable, and effective patient care. By screening for social drivers, providers can identify non-medical obstacles to treatment adherence, connect patients with community resources, and develop realistic care plans tailored to individual circu

Using extreme risk protection orders to prevent violence among people experiencing homelessness in California and Colorado: a case series.

BACKGROUND: Extreme Risk Protection Orders (ERPOs) temporarily restrict firearm access from those at risk of harming themselves and/or others. This case series describes when and how ERPOs have been used for people experiencing homelessness. METHODS: We examined ERPO court case files previously abstracted in California (n = 37, 1/1/2016-6/30/2020) and Colorado (n = 12, 1/1/2020-12/31/2022) involving housing insecurity among respondents. We determined whether the respondent was experiencing homelessness (i.e., sleeping in a public place, shelter, vehicle, motel, or temporarily "couch surfing"

Homelessness at the San Diego Central Library: Assessing the Potential Role of Social Workers.

Nationwide, public libraries are experiencing an increase in "on-premise" opioid overdoses and other issues (e.g., suicide attempts) affecting unstably housed library users. The public library presents a unique opportunity to access an otherwise hidden population. In partnership with the San Diego Central Library, researchers led focus groups, in-depth interviews, and surveys with 63 library patrons experiencing homelessness or housing instability (n = 49) and library staff (n = 14) (January-June 2019). Using a consensus organizing framework and mixed methods approach, the researchers conducte

The Washtenaw ID Project: A Government-Issued ID Coalition Working Toward Social, Economic, and Racial Justice and Health Equity.

Introduction. The policing of identities through policies that restrict access to IDs issued by U.S. governmental entities disparately affects communities of color; communities who identify as low-income, immigrant, older, and/or transgender; and community members who experience chronic mental illness, housing instability, or incarceration. Yet government-issued IDs are increasingly needed to access health-promoting resources such as housing, banking, social services, and health care, and in interactions with law enforcement. Methods. Since 2012, the Washtenaw ID Project's coalition-building p
