Lumboperitoneal shunts for the treatment of normal pressure hydrocephalus.

Ventriculoperitoneal shunt placement is the standard of care for idiopathic normal pressure hydrocephalus (iNPH). Studies have reported shunt complication rates up to 38%, with subdural hemorrhage rates as high as 10%. Lumboperitoneal (LP) shunts with horizontal-vertical valves (HVV) are an alternative for cerebrospinal fluid (CSF) diversion that avoids direct cerebral injury and may reduce the risk of overdrainage. Here we reviewed our experience with LP-HVV shunt placement for iNPH. We retrospectively reviewed our 33 patients with LP-HVV shunts inserted for the treatment of iNPH from 1998 to

Salomón Hakim and the discovery of normal-pressure hydrocephalus.

INTRODUCTION: Normal-pressure hydrocephalus (NPH) is a chronic neurological disorder characterized by enlarged ventricles and a triad of clinical symptoms affecting gait, cognition, and urinary continence. Salomón Hakim first identified the syndrome in 1957 at the Hospital San Juan de Dios in Bogotá, Colombia. Even after decades of international focus and thousands of publications on his disorder, Hakim's story remains largely untold. METHODS: In this historical review, we explore the discovery of NPH through a series of personal interviews with Professor Hakim and his family, discussions wit

Characteristics and reversibility of dementia in Normal Pressure Hydrocephalus.

Studies of the cognitive outcome after shunt insertion for treatment of Normal Pressure Hydrocephalus have reported widely mixed results. We prospectively studied performance of 60 patients with Normal Pressure Hydrocephalus on a comprehensive battery of neuropsychological tests before and after shunt surgery to determine which cognitive functions improve with shunt insertion. We also administered a subset of cognitive tests before and after temporary controlled drainage of cerebrospinal fluid to determine if change on this brief subset of tests after drainage could predict which patients woul

[Predictors of outcome in patients with normal pressure hydrocephalus].

Despite emerging knowledge over 40 years, the postoperative results after shunt implantations in patients diagnosed for normal pressure hydrocephalus (NPH) did not improve significantly during the last decade. For that reason predictors have to be identified in order to predict preoperatively the course of disease. From 1982 until 2000 we examined in a prospective study 200 patients diagnosed for NPH. From the patients, who were surgically treated by a shunt implantation we could reexamine 155 (78%) in a meantime interval of seven month after the operation. The NPH was graduated according to t

The pathogenesis of normal pressure hydrocephalus: a theoretical analysis.

Hydrocephalus is an abnormal accumulation of cerebrospinal fluid (CSF) in the cerebral ventricles, usually caused by impaired absorption of the fluid into the bloodstream. Despite obstructed absorption and continued secretion of CSF into the ventricles at a near normal rate, the ventricular CSF pressure (VCSFP) is often normal. We attempt to understand how hydrocephalus can exist with normal VCSFP by exploring the role of the brain parenchyma in absorbing CSF in hydrocephalus. We test three theories: (1) the ventricular wall is impermeable to CSF; (2) ventricular CSF seeps into the parenchyma,

[A case of normal pressure hydrocephalus with orthostatic hypotension].

A 67-year-old male case of normal pressure hydrocephalus (NPH) after subarachnoid hemorrhage with orthostatic hypotension is reported. He was admitted with gait disturbance two months after a V-P shunt procedure for NPH. Because of shunt malfunction, a triad of clinical symptoms of NPH (dementia, gait disturbance and urinary incontinence) appeared and orthostatic hypotension developed. These conditions gradually worsened. After a reshunting procedure, the triad of NPH symptoms diminished and orthostatic hypotension disappeared. The same etiology that induced NPH symptoms was suspected in this

Conductance to outflow of CSF in normal pressure hydrocephalus.

Normal pressure hydrocephalus (NPH) is defined as a combination of dementia, gait disturbances and/or urinary incontinence, hydrocephalus, and a normal intracranial mean pressure. The clinical effect of CSF shunting in patients with this syndrome is sometimes striking, but generally only 50-60% of the shunted patients benefit from the treatment. It is assumed that the condition is caused by reduced conductance to outflow of CSF ( Cout ). A clinically usable method for the measurement of Cout has been developed. Cout has been measured in 80 patients with NPH. The results of clinical examination

Neurofibrillary tangles in the dementia of "normal pressure" hydrocephalus.

Routine neuropathological examination could not explain the dramatic improvement exhibited by one patient with "normal pressure" hydrocephalus after shunting. The improved patient contrasted remarkably with the unchanged condition of four others also shunted successfully. The five brains were analysed by quantitative morphometry to determine the degree of neurofibrillary tangle formation in mesial temporal neurons. The density of tangle-bearing nerve cells in the four unimproved cases was markedly greater than in age-matched control brains from nineteen normal subjects, and fell in the same ra

Lower Breakpoint of Intracranial Amplitude-Pressure Relationship in Normal Pressure Hydrocephalus.

The relationship between intracranial pulse amplitude (AMP) and mean intracranial pressure (ICP) has been previously described. Generally, AMP increases proportionally to rises in ICP. However, at low ICP a lower breakpoint (LB) of amplitude-pressure relationship can be observed, below which pulse amplitude stays constant when ICP varies. Theoretically, below this breakpoint, the pressure-volume relationship is linear (good compensatory reserve, brain compliance stays constant); above the breakpoint, it is exponential (brain compliance decreases with rising ICP).Infusion tests performed in 169

Cerebrospinal fluid pressure studies in normal pressure hydrocephalus and cerebral atrophy.

The surgical results obtained with a cerebrospinal fluid (CSF) shunt operation in 13 patients considered to be suffering from normal pressure hydrocephalus have been correlated with the findings of the constant manometric infusion test (IT) and of prolonged intracranial pressure recordings. A positive IT, high amplitude CSF pulse pressure and large transitory increases of CSF pressure during sleep seem to be useful criteria for the surgical prognosis. Ten more patients affected by primary cerebral atrophy have also been studied. The data obtained in both groups of patients have been utilized f

Differences in Brain Morphology between Hydrocephalus Ex Vacuo and Idiopathic Normal Pressure Hydrocephalus.

OBJECTIVE: The distinction between idiopathic normal pressure hydrocephalus (iNPH) and hydrocephalus ex vacuo caused by encephalic volume loss remains to be established. This study aims to investigate radiological parameters as clinically useful tools to discriminate iNPH from hydrocephalus ex vacuo caused by Alzheimer's disease (AD). METHODS: A total of 54 patients with ventriculomegaly (iNPH, 25; hydrocephalus ex vacuo, 29) were recruited in this study. Consequently, nine radiological parameters were compared between iNPH and hydrocephalus ex vacuo using magnetic resonance imaging (MRI). R

Characteristics of Postural Instability in Idiopathic Normal Pressure Hydrocephalus.

INTRODUCTION: In patients with idiopathic normal pressure hydrocephalus (iNPH), the characteristics of static steady-state balance disturbance are not as well understood as those related to gait. This study aimed to investigate differences in center of pressure (COP) parameters between patients with iNPH and age-matched healthy controls. METHODS: A total of 56 patients diagnosed with probable iNPH, who responded positively to cerebrospinal fluid tap test (CSFTT), along with 25 age-matched healthy controls, were enrolled. Prior to the CSFTT, COP measurements were taken using a force plate whil
