Comparative Effectiveness of Less vs. More Frequent Hydrocortisone Dosing in Septic Shock.

OBJECTIVES: Guidelines recommend hydrocortisone as an adjunctive treatment in septic shock, but the optimal dosing regimen is unknown. A national shortage of hydrocortisone in 2023 prompted a change in institutional practice for hydrocortisone administration from 50 mg every 6 hours to 100 mg every 12 hours in an effort to reduce waste and conserve vials, creating an opportunity to evaluate the comparative effectiveness of these two regimens. The primary efficacy outcome was time to shock resolution, and secondary outcomes evaluated in this study were mortality, renal replacement therapy (RRT)…

Hydrocortisone Mitigates Alzheimer's-Related Cognitive Decline through Modulating Oxidative Stress and Neuroinflammation.

Alzheimer's disease (AD), an age-related degenerative disorder, is characterized by β-amyloid deposition, abnormal phosphorylation of tau proteins, synaptic dysfunction, neuroinflammation, and oxidative stress. Despite extensive research, there are no medications or therapeutic interventions to completely treat and reverse AD. Herein, we explore the potential of hydrocortisone (HC), a natural and endogenous glucocorticoid known to have potent anti-inflammatory properties, in an Aβ1-42-induced AD mouse model. Our investigation highlights the beneficial effects of HC administration on cognitive …

Immediate versus modified release hydrocortisone in mitotane-treated patients with adrenocortical cancer.

OBJECTIVE: Mitotane induces hepatic CYP3A4 activity, resulting in accelerated cortisol inactivation, and also increases cortisol binding globulin (CBG). Therefore, higher hydrocortisone doses are required in patients with adrenocortical cancer (ACC) on mitotane treatment. Modified release hydrocortisone has not been used in mitotane-treated ACC patients yet. AIM: Case series to compare serum cortisol, calculated free serum cortisol and ACTH levels in ACC patients on mitotane treatment with immediate and modified release hydrocortisone. DESIGN: Pharmacokinetics of immediate and modified relea…

Hydrocortisone and dexamethasone dose-dependently stabilize mast cells derived from rat peritoneum.

BACKGROUND: Besides their anti-inflammatory properties, corticosteroid drugs exert anti-allergic effects. Exocytosis of mast cells is electrophysiologically detected as the increase in the whole-cell membrane capacitance (Cm). Therefore, the lack of such increase after exposure to the drugs suggests their mast cell-stabilizing effects. METHODS: We examined the effects of 1, 10, 100 and 200μM hydrocortisone or dexamethasone on the degranulation from rat peritoneal mast cells. Employing the whole-cell patch-clamp recording technique, we also tested their effects on the Cm during exocytosis. RE…

Hydrocortisone induced regional cerebral activity changes in schizophrenia: a PET scan study.

BACKGROUND: There is evidence that, even during remission, schizophrenia (SZ) patients are especially vulnerable to de-compensate under stress, and that they tend to have a high baseline serum cortisol levels. This study was undertaken to determine whether raising serum cortisol by the infusion of hydrocortisone, in the absence of additional psychological stress, would result in different cerebral activity changes in schizophrenic patients compared to normal controls (CON). We were especially interested in cerebral activity in regions such as the medial temporal lobe and hippocampus, since str…

Continuous subcutaneous hydrocortisone infusion in pediatric primary adrenal insufficiency: a cohort study.

OBJECTIVE: Primary adrenal insufficiency (PAI) in children, most commonly caused by congenital adrenal hyperplasia (CAH), is challenging to treat due to the short half-life of hydrocortisone and the difficulty in mimicking the physiological rhythm of cortisol. Continuous subcutaneous hydrocortisone infusion (CSHI) has shown benefits in CAH adults but remains poorly studied in children. The aim of our study was to evaluate the feasibility, safety, and clinical efficacy of CSHI in pediatric patients under oral treatment with poorly controlled PAI. METHODS: We conducted a retrospective monocentr…

Real-world outcomes with modified-release hydrocortisone in classic congenital adrenal hyperplasia.

OBJECTIVE: To evaluate outcomes after switching from conventional glucocorticoids (GCs) to modified-release hydrocortisone (MR-HC; Efmody) in adults with classic congenital adrenal hyperplasia (CAH) due to 21-hydroxylase deficiency under real-life conditions. DESIGN: Prospective, observational cohort study at a tertiary endocrine center. METHODS: Sixty-two adults with classic CAH (25 male; 42 salt-wasting) were switched to MR-HC and followed up for at least 12 months and up to 4 visits or until termination of treatment (median 23.5 months; visit 4 not yet completed in 6/62). Outcomes include…

Does perioperative hydrocortisone reduce morbidity after pancreatoduodenectomy? A propensity score matched analysis.

The use of steroids in pancreatic surgery may reduce complications through the anti-inflammatory potential. We aimed to evaluate the impact of hydrocortisone (HC) on postoperative morbidity after pancreatoduodenectomy (PD). The data were retrospectively extracted from a prospective clinical database. Complicatons after PD with 100 mg HC intravenously (i.v.) for 48 h and a corresponding propensity score-matched cohort without HC were evaluated by using logistic regression analyses. We analyzed 691 patients who underwent PD over a period of ten years. After propensity score matching, 220 patient…

Immunomodulatory Effects of Polysaccharides from Porphyra haitanensis in Hydrocortisone-Induced Immunocompromised Mice.

This study investigated the immunomodulatory effect of polysaccharides from Porphyra haitanensis (PHP) using a hydrocortisone-induced immunosuppressive model. Immunocompromised mice were treated with varying doses of PHP and the effects on macroscopic indicators, macrophage function, and both cellular and humoral immune functions were comprehensively assessed. The results showed that PHP significantly increased the body weight and indexes of the spleen and thymus, improved the disorder of blood cell populations, and enhanced macrophage activity. Furthermore, PHP improved T lymphocyte subtypes …

Dual-release hydrocortisone treatment: glycometabolic profile and health-related quality of life.

OBJECTIVE: Adrenal insufficiency (AI) is a chronic condition associated with increased mortality and morbidity. The treatment of AI in the last years has been object of important changes due to the development of a dual-release preparation of hydrocortisone. It differs from previous therapeutic strategy as it contemplates a once-daily tablet that allows more closely mimicking the physiological circadian cortisol rhythm. The aim of the study was to evaluate the effects of dual-release hydrocortisone treatment on the glycometabolic profile and health-related quality of life of patients with AI. …

Higher hydrocortisone dose increases bilirubin in hypopituitary patients- results from an RCT.

BACKGROUND: Bilirubin has anti-oxidative and anti-inflammatory properties, which may explain its proposed protective effects on the development of cardiometabolic disorders. Glucocorticoids affect heme oxygenase regulation in vitro, which plays a key role in bilirubin production. Effects of variations in glucocorticoid exposure on circulating bilirubin levels in humans are unknown. Here we tested whether a higher hydrocortisone replacement dose affects circulating bilirubin in hypopituitary patients. MATERIALS AND METHODS: A randomized double-blind cross-over study (ClinicalTrials.gov, number…

Hydrocortisone stimulates neurite outgrowth from mouse retinal explants by modulating macroglial activity.

PURPOSE: There is mounting evidence that retinal ganglion cells (RGCs) require a complex milieu of trophic factors to enhance cell survival … 2 stimulated significant neurite outgrowth from explants but not from purified RGCs. Elimination of hydrocortisone (HC) from EN2 reduced the mean neurites per explant by 37%. EN2-treated explants demonstrated increased expression of Gfap, Glul, Glt1, Cntf, Pedf, and VegfA compared with explants treated with EN2 without HC. Subsequent experiments showed that incr …

Efficacy of 5-aminosalicylic acid enemas versus hydrocortisone enemas in ulcerative colitis.

A controlled trial has been carried out in order to compare the efficacy of enemas containing a high dosage of 5-ASA (4 g) versus enemas containing hydrocortisone 100 mg. The trial was conducted on 86 patients, 44 of whom received 5-ASA and 42 received hydrocortisone. The results were favorable in terms of clinical, sigmoidoscopic, and histologic criteria for 5-ASA treatment. Other aspects have been investigated, such as retrograde spread of enemas which have been shown to reach the left colon. No nephrotoxicity was detected. The long term experience confirmed the preliminary positive results.

Time Adjustment of Hydrocortisone Doses During Shift Work in Patients with Adrenal Insufficiency.

Shift work causes a disruption between the circadian system and the external light-dark cycle, but also a misalignment between various levels of the circadian system. There is no information on patients with adrenal insufficiency (AI) who are working shifts. The objective of the study was to analyze the hormone replacement therapy with hydrocortisone (HC) and the adaptation scheme in patients with AI on shifts. Patients working on shifts (n=15) from two German endocrine centers received a questionnaire regarding their therapy scheme, dose adaptations, working shifts, dose adaptations during wo…
