Adjustment of the osmostat in primary aldosteronism.

OBJECTIVE: To analyze osmoregulation in primary aldosteronism. DESIGN: The physiologic and pathologic factors involved in function of the osmostat and in hypernatremia were reviewed. RESULTS: Patients with primary aldosteronism commonly have mild hypernatremia, with serum sodium concentrations usually less than 150 meq/L. Hypernatremia has been detected in patients with aldosterone-producing adrenal adenomas and adrenal hyperplasia. The patients seem to ingest normal amounts of water. Adjustment of the osmosta (in the hypothalamus) to a higher than normal level of plasma osmolality seems to …

Incidence and mortality prognosis of dysnatremias in neurologic critically ill patients.

BACKGROUND: Dysnatremia, which is associated with increased mortality in general intensive care units (ICU), has not been thoroughly studied in neurologic ICU (NICU). METHODS: Prevalence of dysnatremia was retrospectively assessed. The multivariable binary logistic regression model was used to determine the influence of dysnatremia on mortality. RESULTS: Of 519 patients, 106 (20.4%) were admitted with hyponatremia and 177 (34.10%) with hypernatremia. Hypernatremia was detected in 69 (13.29%) patients on admission to NICU and in 108 patients (20.81%) during the ICU stay. However, the incidenc…

Unusual Presentation of Central Diabetes Insipidus in a Patient With Neurosarcoidosis.

Hypernatremia is a frequent cause of intensive care unit admission. The patient presented in this article had hypernatremia refractory to D5W (dextrose 5% water) therapy, which led to a complex investigation. Workup revealed central diabetes insipidus most likely secondary to flare up of neurosarcoidosis. The challenge in terms of diagnosis was a presentation with low urine output in the setting of hypernatremia resistant to treatment with desmopressin. This case unfolded the role of hypothyroidism causing secondary renal dysfunction and hence needed continued treatment with thyroxine in addit…

Cerebral Sinovenous Thrombosis due to Hypernatremic Dehydration in a Neonate.

Cerebral sinus venous thrombosis is an uncommon complication of hypernatremic dehydration in neonates. Non-improvement in neurological status even after correction of hypernatremia should lead to suspicion of intracranial complications due to hypernatremia or its overtreatment. Slow correction of hypernatremia, calculated fluid administration, and anticoagulation improve outcome in neonates with CSVT.

Does hyperglycemia in hypernatremic preterm infants increase the risk of intraventricular hemorrhage?

OBJECTIVE: Hypernatremia and hyperglycemia are highly prevalent in preterm infants during the first week after birth, and both can lead to hyperosmolarity and osmotic shifts. The objective is to determine whether hyperglycemia increases the risk of intraventricular hemorrhage (IVH) in hypernatremic preterm infants. STUDY DESIGN: Single-center retrospective medical record review of 216 infants <1000 g birth weight and <29 weeks gestational age (admitted over a 9-year period) who had serum sodium levels and blood glucose levels monitored at least every 24 h and more frequently if indicated duri…

Outcome and frequency of sodium disturbances in neurocritically ill patients.

Sodium disturbances are frequent and serious complications in neurocritically ill patients. Hyponatremia is more common than hypernatremia, which is, however, prognostically worse. The aim of this study was to analyse outcome and frequency of sodium disturbances in relation to measured serum osmolality in neurologic-neurosurgical critically ill patients. A 5-year retrospective collection of patients (pts) and laboratory data were made from the Laboratory Information System database in the Clinical Biochemistry Department. The criteria for patients' inclusion was acute brain disease and serum s…

Hypernatremic chloride-depletion metabolic alkalosis successfully treated with high cation-gap amino acids: a case report.

Normal saline (NS) is recommended for the treatment of chloride-depletion alkalosis (CDA). However, its use in patients with drinking water restrictions or fluid volume deficiencies may lead to hypernatremia. We report the case of a 42-year-old Japanese man with ileus due to sigmoidal volvulus, who presented with CDA. After endoscopic decompression, NS was administered to treat the CDA. Despite the administration of NS, CDA persisted and hypernatremia developed. The infusion was then changed to high cation-gap amino acids (HCG-AA), which improved both metabolic alkalosis and hypernatremia. Thu…

Rising serum sodium levels are associated with a concurrent development of metabolic alkalosis in critically ill patients.

PURPOSE: Changes in electrolyte homeostasis are important causes of acid-base disorders. While the effects of chloride are well studied, only little is known of the potential contributions of sodium to metabolic acid-base state. Thus, we investigated the effects of intensive care unit (ICU)-acquired hypernatremia on acid-base state. METHODS: We included critically ill patients who developed hypernatremia, defined as a serum sodium concentration exceeding 149 mmol/L, after ICU admission in this retrospective study. Data on electrolyte and acid-base state in all included patients were gathered …

Approach and management of dysnatremias in cirrhosis.

Hypervolemic (dilutional) hyponatremia is the most common dysnatremia in cirrhosis, with a prevalence close to 50% in patients with ascites, while hypovolemic hyponatremia occurs in a minority of cases. Hyponatremia carries a poor prognosis, being associated with increased mortality and reduced survival after liver transplantation. Hypernatremia is rarer and is also associated with an adverse prognosis. Increased non-osmotic secretion of arginine vasopressin and altered renal tubular sodium handling due to impaired free water generation are the mechanisms leading to hypervolemic hyponatremia, …

[Disorders of serum sodium in emergency patients : salt in the soup of emergency medicine].

Electrolyte disorders are common and potentially fatal laboratory findings in emergency patients. Approximately 20 % of patients in the emergency department present with either hyponatremia or hypernatremia. Recently it was shown that disorders of serum sodium are not only an expression of the severity of the underlying disease but independent predictors for the outcome of patients. They directly influence patient daily life by causing not only gait and concentration disturbances but also an increased tendency to fall together with a reduced bone mass. Given these new data it is even more impo…

Central diabetes insipidus following carbon monoxide poisoning.

Diabetes insipidus is a rare complication after carbon monoxide poisoning. We report on a woman and her daughter who were noted to have polyuria and hypernatremia after exposure to carbon monoxide. Both patients were responsive to desmopressin therapy, and the diagnosis of central diabetes insipidus was made. The development and correction of hypernatremia further complicated the neurological damages caused by carbon monoxide poisoning. These cases illustrate the possibility of central diabetes insipidus and hypernatremia after carbon monoxide poisoning and the importance of immediate antidiur…
