Mania in a case of hyperparathyroidism.

Psychiatric manifestations of primary hyperparathyroidism are mediated by hypercalcemia. To date, most evidence indicates that hypercalcemia and increased cerebrospinal-fluid calcium levels produce depression symptoms. Presented here is a case report of a 52-year-old woman in a manic state. She had no psychiatric history but had substantially elevated parathyroid hormone levels and hypercalcemia. On the basis of emerging evidence that calcium channel-blockers effectively treat mania, the authors propose that elevated calcium levels may act through multiple mechanisms or on various regions of t…

Hyperparathyroidism in dogs with hyperadrenocorticism.

OBJECTIVES: To assess the effect of canine hyperadrenocorticism (HAC) on parathyroid hormone (PTH), phosphate and calcium concentrations. M … also the phosphate and post-ACTH cortisol concentrations. CLINICAL SIGNIFICANCE: Adrenal secondary hyperparathyroidism is a cause of increased PTH concentrations and may be associated with abnormalities in calcium and phosphate metabolism in dogs with HAC. The findings of this study could explain why canine HAC may cause clinical signs such as calcinosis cutis that …

A thymoma associated with hyperparathyroidism.

A 50-year-old woman sustaining a noninvasive thymoma accompanied by hyperparathyroidism is presented. Although this is the third case report to date in the literature of the combination of these two diseases, the patient did not evidence the myasthenia gravis cited in the two previous reports. In the two years following concomitant extended thymectomy and resection of the right superior parathyroid gland, including the adenoma, the patient has been following a favorable course without recurrence.

[Ultrasonography in the diagnosis of hyperparathyroidism].

Ultrasonography is a direct method of representation, giving various advantages over other methods in the examination of parathyroid glands: it is rapid, non-radiation and economically undemanding. Dynamic examination by means of the linear electronic transducer of 3.5 MHz and the use of water path was used in 37 patients. In 7 patients with primary hyperparathyroidism was the examination rapid and the pictures of the adenomas were persuasive. In 30 patients with secondary hyperparathyroidism were found characteristic pictures, if the glands were enlarged to 8 mm or more. For the diagnosis of …

The anatomy of primary hyperparathyroidism.

Although several classic anatomic studies describing the number, location, size, and weight of normal parathyroid glands have been reported, as detailed description of the parathyroid glands in a large series of patients with primary hyperparathyroidism has not been available. Drawings were made of the exact locations of each of the normal and enlarged parathyroid glands identified, immediately following neck explorations in all patients with primary hyperparathyroidism during a 4-year period (1977 to 1981). The enlarged glands were also measured and weighed after excision. The records of 273 …

The association of neurofibromatosis and hyperparathyroidism.

Two patients with coexisting neurofibromatosis and hyperparathyroidism are described, bringing the total number of such cases in the world literature to seven. Other more classic examples of the association of tumorous conditions of neuroectodermal and entodermal origin are discussed to support the suggestion that the association of these two diseases may be another variant of multiple endocrine neoplasia type 2 (MEN2b). It may be clinically profitable to investigate all patients with either disease in order to uncover their coexistence.

[Hypo- and hyperparathyroidism in pregnancy].

During pregnancy the maternal calcium metabolism is submitted to special loads. Overchallenging of the functional capacity of the parathyroid gland may evoke an insufficiency of the parathyroid gland which on its part induces a fetal secondary hyperparathyroidism. Therefore, the early diagnostic recognition of the insufficiency of the parathyroid gland is of importance, since (also in basic diseases known before pregnancy) now special therapeutic conditions are given. Maternal hyperparathyroidism leads to fetal hypoprarthyroidism with resulting developmental disturbances; it furthers the tende…

On the etiology of "primary" hyperparathyroidism.

A study of 92 patients operated on for primary hyperparathyroidism revealed that the condition was secondary to renal disease, thyrotoxicosis or malignancy in 35. Review of the literature revealed no reports of experimental studies on the relationship between hyperparathyroidism and thyrotoxicosis or malignancy. In renal disease, however, the author's own investigations show that the traditional concept of hypocalcemia as the most important factor in the development of parathyroid hyperfunction should be rejected. The parathyroid glands probably can be stimulated by various factors in "primary…

Exploration of the neck for hyperparathyroidism.

A step-by-step plan for exploration of the neck for hyperparathyroidism has evolved during a 15-year experience with 148 patients at the Toronto General Hospital. Selective catheterization of the veins of the neck with parathormone immunoassay has been a valuable aid in preoperative localization and is particularly helpful in those patients who have had a previous neck dissection. The success of the exploration is based on a practical knowledge of the embryology and anatomy of the parathyroid glands, meticulous dissection and the identification of all glands. The "angle of identification", as …

[The diagnosis of hyperparathyroidism and hypoparathyroidism].

Some recent pathophysiologic, diagnostic and therapeutic aspects of hyper- and hypoparathyroidism are reviewed. Three clinical forms of hyperparathyroidism can be distinguished: one with symptoms essentially affecting the skeleton, a second with renal symptoms and a third with scant and sometimes atypical symptoms. The most reliable laboratory criteria for diagnosis of hyperparathyroidism are serum levels of calcium and immunoreactive parathormone. Some authors believe that with a combination of these two parameters a diagnosis is possible in almost all cases. Other determinations such as seru…

[Sonographic localisation procedures in hyperparathyroidism -- persistent hyperparathyroidism after removal of four parathyroid glands].

This case report will give an example for the necessity of performing an ultrasound examination of the neck before resection of the parathyroid glands. In end-stage renal failure a 38-year-old man developed secondary hyperparathyroidism (HPT). Four glands were removed. Two years later the patient revealed persisting HPT. At this point an ultrasound examination was performed and demonstrated a fifth parathyroid gland located inside the thyroid gland. The case illustrates the benefit of cervical ultrasound, even in cases of secondary hyperparathyroidism, prior to any cervical exploration.

Recurrence of hyperparathyroidism; a long-term follow-up after surgery for primary hyperparathyroidism.

OBJECTIVE: In general it is thought that recurrence of primary hyperparathyroidism is a rare event. To our knowledge, however, only one large patient series has been reported with a mean of more than 7 years of follow-up. The aim of the present study was to determine the long-term recurrence rate in 785 out of 886 patients operated on for primary hyperparathyroidism and followed-up for a mean of 10.03 years after surgery. DESIGN: Medical records were scrutinised. The patients who were still alive answered a questionnaire and had laboratory tests. METHODS: Follow-up data concerning the state …

[Hyperparathyroidism-jaw tumor syndrome. A hereditary form of primary hyperparathyroidism with parathyroid carcinoma].

HISTORY: A 29-year-old man presented with a giant cell granuloma of the maxilla that had initially been diagnosed as a "brown tumor" (a bone replacing mass of fibrous tissue containing hemosiderin-pigmented macrophages and multinucleated giant cells). Because serum calcium and PTH were elevated, primary hyperparathyroidism was diagnosed. Three months later a parathyroid carcinoma and a brown tumour in the left femur were identified and removed surgically. Hyperparathyroidism-jaw tumor syndrome was suspected. INVESTIGATIONS AND DIAGNOSIS: Mutation analysis of the DNA revealed heterozygous nons…

Genetic analyses in patients with familial isolated hyperparathyroidism and hyperparathyroidism-jaw tumour syndrome.

BACKGROUND: A subset of familial isolated primary hyperparathyroidism (FIHP) is a variant of hyperparathyroidism-jaw tumour syndrome (HPT-JT). AIM/PATIENTS AND METHODS: We investigated the involvement of the HRPT2, MEN1 and CASR genes in 11 provisional FIHP families and two HPT-JT families. RESULTS: Germline mutations of HRPT2 were found in two of the 11 FIHP families and one of the two HPT-JT families. One FIHP family with parathyroid carcinoma and atypical adenomas and another FIHP family with cystic parathyroid adenoma had novel frameshift mutations of 518-521del and 62-66del, respectively…

Primary hyperparathyroidism in young patients in Russia: high frequency of hyperparathyroidism-jaw tumor syndrome.

BACKGROUND: Primary hyperparathyroidism (PHPT) is a relatively rare disorder among children, adolescents and young adults. Its development at an early age is suspicious for hereditary causes, though the need for routine genetic testing remains controversial. OBJECTIVE: To identify and describe hereditary forms of PHPT in patients with manifestation of the disease under 40 years of age. DESIGN: We enrolled 65 patients with PHPT diagnosed before 40 years of age. Ten of them had MEN1 mutation, and PHPT in them was the first manifestation of multiple endocrine neoplasia type 1 syndrome. METHODS…
