Quality of Life in Primary Hyperparathyroidism.

Quality of life is impaired in primary hyperparathyroidism (PHPT), regardless of the severity of the disease. Clinical studies have employed different instruments, including standardized and disease-specific questionnaires, and including patients with different phenotypes of PHPT. Neuropsychiatric symptoms and decline in cognitive status are common in PHPT. Patients may complain of these issues or they can be ascertained by questionnaires; they include depression, anxiety, impaired vitality, social and emotional functions, sleep disturbances, and altered mental function. Randomized controlled

FGF23 and primary hyperparathyroidism: is there a link?

INTRODUCTION: Data regarding the role of fibroblast growth factor 23 (FGF23) in primary hyperparathyroidism (PHPT) are scarce and discordant. Our study aimed to evaluate the prognostic impact of FGF23 upon the clinical and biochemical evolution of PHPT. MATERIAL AND METHODS: Forty-two patients with ages between 30 and 80 years, diagnosed with PHPT caused by a sporadic, solitary parathyroid adenoma, and referred to surgery (minimally invasive parathyroidectomy) were prospectively included in the study. Serum levels of FGF23, PTH, 25(OH)D3, calcium (Ca), phosphate (P), total procollagen type 1

Sclerostin and DKK1 in primary hyperparathyroidism.

Bone formation is influenced by the Wnt pathway through effects on osteoblast functionality, and these actions are opposed by two antagonists: sclerostin and Dickkopf-1 (DKK1). Decreased levels of serum sclerostin were found after treatment with the PTH analogue teriparatide and in patients with primary hyperparathyroidism (PHPT), while treatment with teriparatide of postmenopausal osteoporosis is associated with increases in serum DKK1. We studied mineral metabolism and Wnt pathway in 21 postmenopausal women affected by PHPT and in 42 age-matched healthy women. Mean serum calcium and PTH were

[Primary hyperparathyroidism. An alternative to the surgery].

Primary hyperparathyroidism is caused by an adenoma/hyperplasia in the parathyroid glands in which hypercalcemia is mainly due to an increased secretion of parathormone (PTH). The only definitive treatment is surgery. There are some patients at high surgical risk or who refuse surgery, and whose hypercalcemia cannot be controlled with conventional medical therapy such as hydration, diuretics and/or oral biphosphonates. We suggest the use of two drugs indicated for the treatment of hypercalcemia of other etiologies: zoledronic acid, a parenteral bisphosphonate, and cinacalcet, a calcimimetic ag

Characteristics of primary hyperparathyroidism in adolescents.

AIM: To analyze adolescent primary hyperparathyroidism (PHPT) in India. DESIGN: A retrospective analysis of consecutive medical records of patients with PHPT < or =20 years old who underwent parathyroid surgery at a tertiary care center in north India from 1993 to 2006. METHODS: Fourteen adolescents underwent clinical, biochemical, hormonal, radiological and scintigraphic evaluation for confirmation of PHPT followed by parathyroidectomy. RESULTS: Mean (+/- SD) age of these patients was 15 +/- 3.9 years (8 female). All were symptomatic at presentation except one who had multiglandular involv

Primary hyperparathyroidism in dogs and cats.

The most common cause of primary hyperparathyroidism in dogs and cats is a solitary adenoma involving an extracapsular parathyroid gland. The prognosis is excellent if the affected parathyroid gland is removed. Nonsurgical methods are discussed, although there are no current data to support any benefit over conventional surgery. The common postoperative complication to consider is hypocalcemia. Hypocalcemia can be successfully managed in these animals if it is anticipated and treated promptly.

Primary hyperparathyroidism and the cardiovascular system.

The role of primary hyperparathyroidism (PHPT) in heart disease is still somewhat uncertain in many respects. Patients with PHPT seem to have an increase in mortality and this seems mainly due to an overrepresentation of cardiovascular death. PHPT is reported to be associated with hypertension, disturbances in the renin-angiotensin-aldosterone system, cardiac arrhythmias as well as structural and functional alterations in the vascular wall. There is an increased prevalence of cardiac structural abnormalities such as LVH and functional properties of the heart may be affected by the hyperparathy

Primary hyperparathyroidism in the older person.

Primary hyperparathyroidism (PHPT) is a common condition which may have few symptoms. One of the principal concerns in the older person with minimally symptomatic PHPT is restoration of bone mineral density and prevention of fracture. Other important considerations are cardiovascular risk and quality of life. Surgery, the traditional treatment of choice, may not always correct these factors. We present a review of the literature and advice of medical therapies which should be of benefit, with emphasis on a multifaceted approach in protecting the patient from PHPT.

Epidemiology of primary hyperparathyroidism in Europe.

Primary hyperparathyroidism (PHPT) is one of the most common endocrine diseases. Its clinical presentation has dramatically changed in the last 40 years, and now the disease typically affects elderly women and is characterized by mild hypercalcemia and few traditional classic (bone and kidney) manifestations. The change in clinical presentation was largely caused by development of automated serum calcium measurement in the early 1970s that made possible the introduction of serum calcium determination in the routine biochemical screening and the identification of a large number of "asymptomatic
