When is AI "just another innovation"? A comparative conceptual analysis of artificial intelligence and evidence-based practice implementation.

BACKGROUND: Implementation science frameworks, such as the Consolidated Framework for Implementation Research (CFIR), are increasingly used to guide the implementation of artificial intelligence (AI) in healthcare. This assumes that AI systems can be understood using frameworks developed primarily for evidence-based practices (EBPs). However, AI systems vary substantially in their technical architecture, degree of autonomy, dependence on local data, regulatory status and capacity for change after deployment. AIM: To examine how different types of AI systems align with, extend or challenge cor…

Optimizing vaccine uptake in sub-Saharan Africa: a collaborative COVID-19 vaccination campaign in Madagascar using an adaptive approach.

BACKGROUND: The COVID-19 pandemic has highlighted the need for more effective immunization programs, including in limited resource settings. This paper presents outcomes and lessons learnt from a COVID-19 vaccination campaign (VC), which used a tailored adaptive strategy to optimise vaccine uptake in the Boeny region of Madagascar. METHODS: Guided by the Dynamic Sustainability Framework (DSF), the VC implementation was regularly reviewed through multi-sectoral stakeholder feedback, key informant interviews, problem-solving meetings, and weekly monitoring of outcome indicators to identify and

Pilot study assessing the possible benefits of a higher level of implementation of biosecurity measures on farm productivity and health status in Belgian cattle farms.

Over the last few years, the interest of decision-makers and control agencies in biosecurity (BS), aiming at preventing and controlling the introduction and spread of infectious diseases, has considerably increased. Nevertheless, previous studies highlighted a low implementation level of biosecurity measures (BSM), especially in cattle farms; different reasons were identified such as perceived costs, utility, importance, increased workload and lack of knowledge. In order to convince cattle farmers to adopt BSM, it is necessary to gather more information and evidence on their cost-effectiveness

Subtract Before You Add: Toward the Development of a De-Implementation Approach in School-Based Speech Sound Therapy.

PURPOSE: It is often difficult for school-based speech-language pathologists (SLPs) to prioritize implementing new practices for children with speech sound disorders (SSDs), given burgeoning caseloads and the myriad of other workload tasks. We propose that de-implementation science is equally as important as implementation science. De-implementation science is the recognition and identification of areas that are of "low-value and wasteful." Critically, the idea of de-implementation suggests that we first remove something from a clinician's workload before requesting that they learn and impleme…

Barriers and facilitators at pre-implementation of a type 2 diabetes screening program in dental care using the consolidated framework for implementation research and the theoretical domains framework.

BACKGROUND: Screening and referral for type 2 diabetes mellitus (T2DM) during dental care visits has the potential for expanding preventive care. Using the consolidated framework for implementation research (CFIR) and the theoretical domains framework (TDF), we examined the barriers and facilitators at pre-implementation of a community-driven T2DM screening program in an urban dental clinic serving Alaska Native and American Indian (AN/AI) adults. METHODS: This convergent mixed-methods parallel study was informed by the updated CFIR. Data were collected in 2023 through a 13-item survey of adu

Identifying the barriers to universal cervical length screening for preterm birth prevention at a tertiary hospital in Thailand (patient's perspectives): implementation research.

PURPOSE: To identify patient perspectives of barriers to cervical length screening to prevent preterm births. METHODS: In Phase I of this prospective descriptive implementation study, 40 pregnant women of up to 24 week gestation were interviewed. Phase II comprised questionnaire development and data validation. The questionnaire was subsequently administered to 400 participants in Phase III. RESULTS: Most participants (74.3%) realised preterm babies have complications and high care costs (53%). They recognised that premature-birth risk identification enables appropriate pregnancy care (93.8%

Shedding light on conditions for the successful passive dissemination of recommendations in primary care: a mixed methods study.

BACKGROUND: Passive dissemination of information in healthcare refers to the publication or mailing of newly established guidelines or recommendations. It is one of the least costly knowledge translation activities. This approach is generally considered to be ineffective or to result in only small changes in practice. Recent research, however, suggests that passive dissemination could, under certain conditions, result in modifications of practice, similar to more active dissemination approaches. The objective of our study was to uncover the conditions associated with the change in primary care

Implementation of antepartum preterm birth interventions: A scoping review.

While antenatal recommendations regarding preterm birth are essential to preventing neonatal morbidity and mortality, implementation of these recommendations underscore how health system capacity and systemic factors, such as access and quality, greatly influence their utilization. To date, there is limited synthesis focused on the implementation of antenatal preterm birth interventions. Our objectives were to focus on implementation science studies intended to 1) increase referral mechanisms of pregnant people to higher levels of care where the management of preterm labor or delivery is possi…

Call to action for health systems integration of point-of-care testing to mitigate the transmission and burden of sexually transmitted infections.

OBJECTIVES: In 2016, WHO estimated 376 million new cases of the four main curable STIs: gonorrhoea, chlamydia, trichomoniasis and syphilis. Further, an estimated 290 million women are infected with human papillomavirus. STIs may lead to severe reproductive health sequelae. Low-income and middle-income countries carry the highest global burden of STIs. A large proportion of urogenital and the vast majority of extragenital non-viral STI cases are asymptomatic. Screening key populations and early and accurate diagnosis are important to provide correct treatment and to control the spread of STIs.

Promoting early expression of breast milk in mothers of preterm infants in a neonatal unit: a best practice implementation project.

OBJECTIVES: The aim of this implementation project was to optimize lactation success in mothers of preterm infants by ensuring early (as soon as possible and within six hours) and frequent (eight to 12 times/day) milk expression. INTRODUCTION: Evidence suggests that breast milk improves enteral feeding tolerance and neurodevelopmental outcomes and decreases the risk of necrotising enterocolitis and late onset sepsis in preterm infants. It is widely accepted that breast milk expression should commence as soon as possible and within six hours of delivery. RESULTS: Following implementation of t

Growing implementation scientists: NHLBI's K12 programs in heart, lung, blood, and sleep research.

BACKGROUND: Implementation science seeks to accelerate the adoption, integration, and scale-up of evidence-based interventions (EBIs) to improve health and reduce disparities. As the field expanded, demand for trained implementation scientists rapidly outpaced workforce capacity. To address this need, the National Heart, Lung, and Blood Institute (NHLBI) launched the institutional Research Career Development Programs in T4 Implementation Research (K12) Next Generation of Implementation Science (iNexIS) in 2016. The program aimed to expand the national pool of rigorous implementation researcher…

Rapid and pragmatic implementation mapping for pre-implementation contextual analysis in multisite implementation trials.

BACKGROUND: Systematic and data-driven implementation strategy selection is typically a resource-intensive process limited by time and implementation science expertise. Simultaneously, implementation trials often include externally driven timelines. To facilitate the conduct of a multisite hybrid trial, we developed and deployed a tool to facilitate rigorous yet pragmatic implementation mapping for strategy selection. METHODS: As part of the HATRICC-US hybrid type II stepped wedge trial, we developed a strategy selection process based on implementation mapping and the Implementation Research …

Fast-tracking action on the Sustainable Development Goals by enhancing national institutional arrangements.

Six years remain to achieve the Sustainable Development Goals (SDGs). Despite some progress, institutional effectiveness for SDG achievement has not been delivered at a national level. Identification and establishment of an institutional framework to operationalise the 2030 Agenda within national plans, giving science-based coordination of SDG implementation a central role, is urgently required to accelerate progress. This paper tackles this challenge. Drawing on literature analysis, it asks: 1) What are the deficiencies in institutional national arrangements that hinder SDG implementation? 2)
