Hyponatremia in the syndrome of inappropriate secretion of antidiuretic hormone. Rapid correction with urea, sodium chloride, and water restriction therapy.

In the syndrome of inappropriate secretion of antidiuretic hormone (SIADH), rapid elevation of serum sodium concentration may be imperative to correct neurological symptoms. Seven patients with hyponatremia secondary to SIADH were treated by oral intake of two to three doses of 30 g of urea over 24 hours or infusion of 80 g of urea as a 30% solution of over six hours, water restriction (500 mL/24 hr), and sodium supplements (120 to 360 mmole/24 hr). Serum sodium concentration increased from 117 +/- 2 to 126 +/- 1.4 mmole/L (mean +/- SEM) after eight hours, to 130 +/- 1.3 mmole/L after 12 hours

Syndrome of inappropriate secretion of antidiuretic hormone in association with pulmonary carcinosarcoma.

A smoking patient presented with altered mental status, left sided weakness, and an opacity on the chest roentgenogram. During hospitalization he developed hyponatremia, which was corrected with hypertonic saline solution, fluid restriction, and demethylchlortetracycline. The clinical presentation and the laboratory findings supported the diagnosis of the syndrome of inappropriate secretion of antidiuretic hormone. This was believed to be associated with the pulmonary carcinosarcoma documented at necropsy.

[Sensitive radioimmunoassay for plasma arginine vasopressin (author's transl)].

Using an ion exchange resin, a sensitive radioimmunoassay for plasma arginine vasopressin (AVP) was developed. This assay was characterized by the absence of blank values, an excellent recovery rate, great sensitivity (0.1 pg of AVP was significantly detected) and reproducibility. In 8 normal men, plasma AVP after overnight dehydration was 1.57 +/- 0.17 pg/ml and dropped to 0.58 +/- 0.11 pg/ml after 20 ml/kg oral water loading. Significant correlations between plasma AVP levels and plasma or urinary osmolality confirm the validity of this assay. In complete pituitary diabetes insipidus (n = 4)

The syndrome of inappropriate secretion of antidiuretic hormone. A case report.

A 72-year-old woman with the syndrome of inappropriate secretion of antidiuretic hormone of unknown cause during more than one year of observation is reported. Plasma vasopressin levels were excessively elevated, even during a water load test. Her serum electrolyte abnormalities and general state were ameliorated after fluid restriction. During treatment with demeclocycline the patient was able to increase fluid without deterioration.

[Hypo-osmolar hyponatremic syndromes: physiopathology and clinical aspects].

Hyponatremia in virtually all patients results from water retention due to an inability to excrete ingested water. In most cases, this defect represents the persistent secretion of ADH (such as in effective circulating volume depletion, and in the syndrome of inappropriate ADH secretion), although free water excretion can also be limited in disorders in which ADH levels may be appropriately suppressed (such as in advanced renal failure, and in primary polydipsia). The symptoms of hyponatremia primarily reflect neurologic dysfunction induced by cerebral edema and are related both to the severit

Acute neurological Behçet's disease complicated by a syndrome of inappropriate secretion of antidiuretic hormone.

We report a rare case of acute neurological Behçet's disease (neuro-BD) diagnosed concurrently with systemic Behçet's disease (BD), classified as definite BD, and complicated by syndrome of inappropriate antidiuretic hormone secretion (SIADH). A man in his 60s presented with fever, headache and mucocutaneous lesions including oral aphthae, folliculitis-like rashes, genital ulcers and hyponatraemia. He was diagnosed with BD based on mucocutaneous findings. Brain MRI showed no abnormalities, and cerebrospinal fluid analysis revealed aseptic meningitis, supporting the diagnosis of acute neuro-BD.

A Case Report of Late Onset Mania Caused by Hyponatremia in a Patient With Empty Sella Syndrome.

Recurrent manic-like episodes can be induced by hyponatremia possibly due to empty sella syndrome. In the present case, the patient was proven to have syndrome of inappropriate antidiuretic hormone (SIADH) secretion with manic symptoms that resolved after the normalization of the plasma sodium level. To our knowledge, this is the first case of hyponatremia-induced manic symptoms in a patient with empty sella syndrome. More attention should be paid to late-onset mania, because it may be the sign of a more serious medical problem.

Fever and syndrome of inappropriate antidiuretic hormone secretion in a patient with multiple system atrophy after treatment with duloxetine.

BACKGROUND: Multiple system atrophy (MSA) is a neurodegenerative disease that affects the extrapyramidal, cerebellar, and autonomic nervous systems. In some patients, hypothalamic involvement can lead to impaired thermoregulation and hyponatremia. Duloxetine, a serotonin-norepinephrine reuptake inhibitor used for chronic pain, has rarely been linked to fever and syndrome of inappropriate antidiuretic hormone secretion (SIADH). CASE PRESENTATION: A 66-year-old man with clinically probable MSA and baseline hyponatremia (serum sodium 121.6 mmol/L) started duloxetine 30 mg/day for migrating pain.

SIADH as an Underrecognized Manifestation of Porphyria-like Crises in Hereditary Tyrosinemia Type 1: Clinical and Pathophysiological Insights.

Hereditary tyrosinemia type 1 (HT1) is a rare metabolic disorder caused by fumarylacetoacetate hydrolase deficiency, leading to the accumulation of toxic metabolites such as fumarylacetoacetate (FAA) and succinylacetone (SA). We report an 11-year-old boy with poorly controlled HT1 who presented with a severe neurovisceral crisis after suboptimal adherence to nitisinone (NTBC) therapy, characterized by abdominal pain, hypertension, paralytic ileus, seizures, and profound hyponatremia. Biochemical evaluation revealed markedly elevated urinary δ-aminolevulinic acid (ALA), consistent with a porphy

[SIADH in a patient receiving quetiapine therapy].

Hyponatremia is a common electrolyte disturbance associated with the use of medication and can have serious consequences. The syndrome of inappropriate antidiuretic hormone secretion (SIADH) is the cause in the majority of cases. This article describes a 53-year-old patient with the following complaints: restlessness, urinary complaints, increased urgency, thirst and confusion. Hyponatremia was diagnosed. A restrictive fluid policy in combination with reduction of the causative medication was initiated, as a result of which the patient made a full recovery. This case illustrates the potential

Recurrent hyponatremia due to syndrome of inappropriate antidiuresis after traumatic brain injury: two case reports.

BACKGROUND: Transient hyponatremia due to syndrome of inappropriate antidiuresis (SIAD) is a frequent (20-50%) complication of traumatic brain injury (TBI), but it rarely persists or recurs. There are only few published reports of patients suffering from non-transient hyponatremia due to chronic SIAD after TBI. We report two more cases with this condition. CASE 1: A 36-year-old woman suffering from major depression and treated with olanzapine reported severe TBI after a severe fall. Following head injury, she developed severe hyponatremia, which was managed with fluid restriction and salt sup

Aquaporins in Edema.

One of the most prevalent indications of water-electrolyte imbalance is edema. Aquaporins (AQPs) are a protein family that can function as water channels. Osmoregulation and body water homeostasis are dependent on the regulation of AQPs. Human kidneys contain nine AQPs, five of which have been demonstrated to have a role in body water balance: AQP1, AQP2, AQP3, AQP4, and AQP7. Water imbalance is connected with AQP dysfunction. Hyponatremia with elevated AQP levels can accompany edema, which can be caused by disorders with low effective circulating blood volume and systemic vasodilation, such a

Chloride and Potassium Assessment Is a Helpful Tool for Differential Diagnosis of Thiazide-Associated Hyponatremia.

CONTEXT: Differential diagnosis of thiazide-associated hyponatremia (TAH) is challenging. Patients can either have volume depletion or a syndrome of inappropriate antidiuresis (SIAD)-like presentation. OBJECTIVE: To evaluate the impact of the simplified apparent strong ion difference in serum (aSID; sodium + potassium - chloride) as well as the urine chloride and potassium score (ChU; chloride - potassium in urine) in the differential diagnosis of TAH, in addition to assessment of fractional uric acid excretion (FUA). METHODS: Post hoc analysis of prospectively collected data from June 2011

Syndrome of Inappropriate Antidiuretic Hormone Release Secondary to Ectopic Olfactory Neuroblastoma: Case Report and Review of Literature.

Ectopic olfactory neuroblastoma (ONB) is a rare neuroendocrine sinonasal malignancy which arises from sinonasal regions where olfactory neuroepithelium does not exist. Presentation of syndrome of inappropriate antidiuretic hormone release (SIADH) in patients with ectopic ONB is extremely rare. We report a case of a 22-year-old patient with ectopic ONB and paraneoplastic SIADH that was managed at our center. The ONB was arising from the left lamina papyracea and extending into the maxillary and ethmoid sinuses and filling the nasal cavity. Correction of sodium levels occurred within 24 hours of

Anterior Mediastinal Neuroblastoma Associated with Syndrome of Inappropriate Antidiuretic Hormone Secretion: A Morphologic, Immunohistochemical, and Genetic Case Report and Review of the Literature.

We report a mediastinal neuroblastoma in an octogenarian with paraneoplastic syndrome of inappropriate antidiuretic hormone secretion (SIADH). Neuroblastomas are very rare tumors in adults, with thoracic or mediastinal locations being especially uncommon. These neoplasms have been occasionally associated with the SIADH. Given the rarity of incidence and paucity of diagnostic and outcomes data, the significance of standard neuroblastoma prognostic characteristics is unclear, and no treatment paradigms exist for these patients. Further studies are needed to inform future clinical guidelines.

[Hyponatraemia in elderly patient - significance, clarification and therapy].

In the general population, the prevalence of mild, usually asymptomatic hyponatremia ranges from 1,7-7,7 %. With increasing age, it rises to 11,6 % in > 75-year-olds. The prevalence is much higher in nursing home residents and hospital patients. Hyponatremia thus represents the most common electrolyte disturbance in the elderly. Despite the existing evidence on the acute as well as chronic effects of hyponatremia, too little attention is paid to it in clinical practice. An algorithm has proven to be useful for the diagnosis of hyponatremia, which enables the cause to be identified in a few sim

A Nationwide Analysis of Aneurysmal Subarachnoid Hemorrhage Mortality, Complications, and Health Economics in the USA.

Aneurysmal subarachnoid hemorrhage (aSAH) is a devastating neurological condition. Endovascular coiling or surgical clipping have equivocal success rates, but relatively little is known regarding the health economics and complications of these procedures at the population level. We aimed to analyze the complication profiles and healthcare resource utilization (HRCU) associated with the treatment of aSAH in the USA. We performed a retrospective analysis utilizing the IBM MarketScan database between 2008 and 2015. Primary outcomes included economic analysis stratified by post-operative complicat
