The pattern of failure and predictors of locoregional control in lateralized buccogingival cancer after postoperative radiation therapy.

BACKGROUND: To evaluate the failure pattern and identify predictors of locoregional control in lateralized buccogingival cancer after postop … ad contralateral neck failure. Four of 5 patients with marginal failure had recurrent tumors in the infratemporal fossa. In multivariate analysis, extracapsular spread and positive or close surgical margin were associated with poor locoregional control. CONCLUSION: Local in-field recurrence is the most common failure pattern in lateralized buccogingival cancer after …

The approaches and outcomes of skull base surgery for pediatric sarcoma after initial therapy.

OBJECTIVE: Surgical removal of the residual pediatric sarcoma after initial therapy is common; however, that in the skull base area still presents a formidable challenge. We reviewed the approaches and outcomes of skull base surgery for pediatric sarcoma. METHODS: Thirteen patients with pediatric sarcoma who received skull base surgery were reviewed retrospectively. Tumor sites, surgical approach, complications, regional recurrence after surgery, and survival rate were analyzed. RESULTS: The residual tumor sites were the infratemporal fossa (8), nasal cavity and paranasal sinus (4) and orbit…

Juvenile Nasopharyngeal Angiofibroma: Maxillary Swing Approach.

INTRODUCTION: The maxillary swing approach is a very efficient surgical procedure for treating advanced-stage juvenile nasopharyngeal angiofibroma (JNA). It allows wide access to the pterygopalatine fossa, infratemporal fossa, and skull base, allowing for the entire removal of malignancies with considerable local extension. MATERIALS AND METHODS: The purpose of this study was to assess the efficacy and outcomes of the maxillary swing technique for the treatment of extensive nasopharyngeal angiofibroma. An observational study on the presenting features, surgical technique, outcomes, and compli…

Ameloblastoma of the zygomatic region with infratemporal and orbital extension: a case report.

Ameloblastoma is a slow-growing but locally invasive odontogenic tumor with a recognized tendency for recurrence despite its histologically … e, measuring approximately 6.5 × 4.5 × 6.0 cm, with extension into the zygomatic-orbital region and infratemporal fossa, while the maxilla appeared relatively preserved. Incisional biopsy favored ameloblastoma over adenomatoid odontogenic tumor, and definitive characterization was established after resection. The patient underwent en bloc tumor excision through a tra …

Vidian nerve schwannoma with middle cranial fossa extension resected via a maxillary swing approach.

BACKGROUND: Vidian nerve schwannoma is an extremely rare type of facial nerve schwannoma. To the best of our knowledge, only 1 case has been reported. METHODS: We report an additional case of vidian nerve schwannoma with middle cranial fossa extension in a 49-year-old Japanese woman. The surgical approaches for infratemporal fossa schwannomas are reviewed, and the maxillary swing approach we used is described. RESULTS: We adopted a maxillary swing approach combined with endonasal endoscopic techniques for the resection of the lesion. Gross total resection was achieved without sacrificing the…

Lingual nerve revisited-A comprehensive review Part II: Surgery and radiology.

The lingual nerve (LN) is a branch of the mandibular division of the fifth cranial nerve, the trigeminal nerve, arising in the infratemporal fossa. It provides sensory fibers to the mucous membranes of the floor of the mouth, the lingual gingiva, and the anterior two-thirds of the tongue. Although the LN should rarely be encountered during routine and basic oral surgical procedures in daily dental practice, its anatomical location occasionally poses the risk of iatrogenic injury. The purpose of this section is to consider this potential LN injury risk and to educate readers about the anatomy o…

Eosinophilic granuloma of the temporal bone in an adult: controversies in the management.

Eosinophilic granuloma is a rare, benign, lesion characterized by uncontrolled proliferation of Langerhan's cells. It is commonly found in a bone and has been described in almost every bone in the body. It usually affects children and is seldom found in adults. A case of 60-year-old adult male with an eosinophilic granuloma of the right temporal bone with infiltration of the temporal and infratemporal fossa and osteolysis of the squama of the temporal bone is presented. Diagnostic procedure and course of treatment are described. Controversies in the management and different approaches in thera…

Unusual presentations of lymphangioma.

L ymphangioma is a rare but potentially life-threatening disease, particularly in children. Three unusual cases of lymphangioma located in the infratemporal fossa, submaxillary gland, and cervicomediastinal regions are presented. These unusual sites illustrate that lymphangiomas may be found at sites distant from the locations of the embryological lymph sacs. The importance of radiological examination is emphasized. Although there are many modes of therapy for this disease, complete surgical excision when possible is advocated. The literature regarding the embryology, presentation, clinical wo…

Microsurgical anatomy of the jugular foramen region.

The popularization of skull base approaches for lesions at the jugular foramen requires a thorough understanding of the normal anatomic features of this region. We present a series of color photographs with a review of the vital structures at the skull base in the region of the jugular foramen exposed via the infratemporal fossa approach. The jugular bulb and its relationship to the ninth, tenth, eleventh, and twelfth cranial nerves and to the internal carotid artery and otic capsule are demonstrated. All relationships are described in the lateral surgical position. The clinical usefulness of …

Unusual cause of maxillary sinus mass with proptosis.

We report a case of a 21-year-old Indian man with an 8-month history of left-sided headache, maxillary sinus mass, proptosis and swelling of the left temple, whose contrast-enhanced CT scans of the paranasal sinuses showed an enhancing, destructive soft tissue mass involving the left maxillary sinus, orbit, infratemporal fossa and anterior cranial fossa, suggestive of a malignancy or chronic granulomatous disease. Histopathological examination of the sinus mass, which was debulked and partially excised via an endoscopic approach, suggested a diagnosis of immunoglobulin G4-related sclerosing di…

Endoscopic transorbital approach to anterolateral skull base through inferior orbital fissure: a cadaveric study.

BACKGROUND: Endoscopic transorbital approach (eTOA) has been announced as an alternative minimally invasive surgery to skull base. Owing to the inferior orbital fissure (IOF) connecting the orbit with surrounding pterygopalatine fossa (PPF), infratemporal fossa (ITF), and temporal fossa, the idea of eTOA to anterolateral skull base through IOF is postulated. The aim of this study is to access its practical feasibility. METHODS: Anatomical dissections were performed in five human cadaveric heads (10 sides) using 0-degree and 30-degree endoscopes. A stepwise description of eTOA to anterolateral…

New approach for operative management of vascular lesions of the infratemporal internal carotid artery.

Direct operative management of aneurysms of the internal carotid artery is associated with significant morbidity. Preauricular surgical appr … ligated between points of anastomosis, thus isolating the lesion and eliminating dissection in the infratemporal fossa. The posterior location of the vein graft also avoids potentially infected areas adjacent to the oropharynx, nasopharynx, and soft tissues of the face. Use of the external carotid artery decreases carotid cross-clamp time and minimizes graft length, …

Maxillary Nerve as Landmark for Exposure of the Internal Maxillary Artery in Extracranial-Intracranial Bypass Surgery.

OBJECTIVE: To establish a new method for fast exposure of the internal maxillary artery (IMA) during extracranial-intracranial bypass surgery. METHODS: To explore the positional relationship between the IMA and the maxillary nerve and pterygomaxillary fissure, 11 formalin-fixed cadaveric specimens were dissected. Three bone windows of the middle fossa were created for further analysis. Then the IMA length that could be pulled up above the middle fossa was measured after different degrees of removal of bony structure. The IMA branches under each bone window were also explored in detail. RESUL

Inferolateral Displacement of a Maxillary Third Molar Along the Buccal Fat Pad: A Case Report and Anatomical Note.

Iatrogenic displacement of maxillary third molars is an uncommon complication, usually reported in the maxillary sinus or deep posterior spaces. Buccal displacement along the buccal fat pad is rare and may be overlooked. We report the case of a 13-year-old female patient referred after attempted extraction of a maxillary right third molar germ under local anesthesia, resulting in inferolateral displacement. Cone-beam computed tomography localized the tooth in a superficial buccal position, outside the maxillary sinus, infratemporal fossa, pterygopalatine fossa, and parapharyngeal space. Clinic…

Endoscopic drainage of an odontogenic pterygoid muscle abscess.

The infratemporal fossa (ITF) is a potential space bounded by bony structures that can be occupied by both benign and malignant tumors. It is also a potential area of abscess development, most commonly of dental origin. As with any abscess, the treatment of an ITF abscess is surgical drainage. We present a case of an ITF abscess involving the pterygoid muscles following dental extraction in a poorly controlled diabetic patient. The ITF was accessed with an endoscopic transseptal approach through the maxillary sinus to drain the abscess. This case of successful management supports the feasibili…

Arachnoid cyst with extraordinary extracranial extension in the skull base as a result of an iatrogenic defect of the middle cranial fossa floor: case report and literature review.

Arachnoid cysts are benign intracranial lesions that are typically diagnosed incidentally. They are divided into two types: congenital and acquired. Acquired arachnoid cysts are rare and usually arise after trauma, infection, or haemorrhage. In this report, a rare case of an iatrogenic multiloculated arachnoid cyst as an unusual complication of a skull base defect is presented. It extended extracranially into the sphenoid sinus, the ethmoid, the infratemporal fossa, the nasopharynx and the nasal cavity, as well as into the pterygomaxillary and retromaxillary space, appearing with a distinct cl…

Orofacial haemangiopericytoma--a rare case.

Haemangiopericytoma is an uncommon vascular tumour that originates from small pericapillary spindle-shaped cells. Clinical diagnosis of such a lesion is an extremely challenging task because there are very few detailed reports of such lesions and one would hardly think of a diagnosis such as haemangiopericytoma while examining a lesion. We report here a case of haemangiopericytoma arising from the infratemporal fossa involving the orbit, the lateral wall of the nasal cavity, the maxillary sinus, the maxillary alveolar process and the ethmoidal sinus. The clinical features, radiological finding…
