Glargine basal-bolus insulin regimen versus insulin pump therapy: comparison of glycemic control.

BACKGROUND: Study results have varied when blood sugar control for subjects using a glargine basal-bolus insulin regimens (GBBIR) have been compared with those using continuous subcutaneous insulin infusion (CSII). Of 12 studies comparing GBBIR with CSII, six found improved glycemic control with CSII and six found no significant difference. These studies were often limited by small numbers of subjects and other study design parameters. METHODS: We reviewed hemoglobin (Hb) A1c data for youths with type 1 diabetes (n=43) changing from conventional split mix insulin regimens (CSMIR) to GBBIR and…

Importance of hepatitis C virus-associated insulin resistance: therapeutic strategies for insulin sensitization.

Insulin resistance is one of the pathological features in patients with hepatitis C virus (HCV) infection. Generally, persistence of insulin resistance leads to an increase in the risk of life-threatening complications such as cardiovascular diseases. However, these complications are not major causes of death in patients with HCV-associated insulin resistance. Indeed, insulin resistance plays a crucial role in the development of various complications and events associated with HCV infection. Mounting evidence indicates that HCV-associated insulin resistance may cause (1) hepatic steatosis; (2)…

Uterine leiomyomata in relation to insulin-like growth factor-I, insulin, and diabetes.

BACKGROUND: Insulin-like growth factor-I (IGF-I) and insulin stimulate cell proliferation in uterine leiomyoma (fibroid) tissue. We hypothesized that circulating levels of these proteins would be associated with increased prevalence and size of uterine fibroids. METHODS: Participants were 35-49-year-old, randomly selected members of an urban health plan who were enrolled in the study in 1996-1999. Premenopausal participants were screened for fibroids with ultrasound. Fasting blood samples were collected. Associations between fibroids and diabetes, plasma IGF-I, IGF binding protein 3 (BP3), an…

Insulin sensitivity assessment with euglycemic insulin clamp in adult beta-thalassaemia major patients.

OBJECTIVE: To assess insulin sensitivity in young adult normoglycemic beta-thalassaemia major patients. METHODS: We measured insulin sensitivity with the euglycemic insulin clamp in 10 young adult (mean age 24.85 +/- 2.45 yrs) normoglycemic beta-thalassaemia major patients and 10 sex- & age-matched controls. Liver iron accumulation was assessed by magnetic resonance imaging (MRI). RESULTS: Glucose infusion rate (M) required to maintain euglycemia was significantly reduced in thalassaemic patients compared to controls (261.5 +/- 63.5 mg/m2 x min vs. 355.6 +/- 35.3 mg/m2 x min, P = 0.008). Con…

Impact of sleep deprivation on insulin secretion, insulin sensitivity, and other hormonal regulations.

Sleep deprivation, or sleep debt, is a very common condition in the lifestyles of many individuals. This condition has a profound effect on the endocrine system, leading to cardiovascular complications among other disturbances. Sleep deprivation decreases insulin sensitivity and is associated with metabolic control in diabetic patients. Other hormones such as cortisol, growth hormone, insulin-like growth factor-1, prolactin, leptin, thyroid hormone, and dehydroepiandrosterone sulfate, as well as the regulation pathways of these substances, are affected by the quantity and quality of sleep. The…

Calcium phosphate-PEG-insulin-casein (CAPIC) particles as oral delivery systems for insulin.

An oral delivery system for insulin was developed and functional activity was tested in a non-obese diabetic (NOD) mice model. Calcium phosphate particles containing insulin was synthesized in the presence of PEG-3350 and modified by aggregating the particles with caseins to obtain the calcium phosphate-PEG-insulin-casein (CAPIC) oral insulin delivery system. Single doses of CAPIC formulation were tested in NOD mice under fasting or fed conditions to evaluate the glycemic activity. The blood glucose levels were monitored every 1-2h for 12h following the treatments using an ACCU CHECK blood glu…

The relationship between insulin sensitivity and insulin-like growth factor-binding protein-1.

Insulin has been shown to regulate insulin-like growth factor-binding protein-1 (IGFBP-1) in vivo and in vitro. Insulin resistance is a feature of the polycystic ovary syndrome (PCOS). We have studied the relationship between insulin sensitivity (S1) and the circulating concentration of IGFBP-1 in a group of young women and in some who had PCOS. A case-control study has been carried out comparing reproductively normal women with women with PCOS (defined as women with oligo- or amenorrhea associated with androgen excess). Fifteen women with clinical PCOS and ten age- and weight-matched controls…

Glycaemic control of insulin-dependent diabetes mellitus in Sudan: influence of insulin shortage.

Insulin availability and routine diabetes care were cross-sectionally investigated in 122 (M/F; 59/63) insulin-dependent diabetic patients aged 6-60 years with > or = 1 year duration using a structured questionnaire interview followed by a free conversation. Haemoglobin A1c, blood glucose, and serum lipids were measured in the fasting state to assess the metabolic control. Only 12% of the patients had acceptable glycaemic control (HbA1c < 7.5%). Increased age, shorter diabetes duration, and higher body mass index were associated with better metabolic control. Omission or reduction of the insul…

Puberty and polycystic ovarian syndrome: the insulin/insulin-like growth factor I hypothesis.

OBJECTIVES: To provide an up-to-date review of studies that have examined the physiological effects of insulin and insulin-like growth factor I (IGF-I) on ovarian growth, maturation, and steroid synthesis, their physiological role in puberty, and their pathophysiological role in polycystic ovarian syndrome (PCOS). To deduce from these data a hypothesis, explaining the pathogenetic connections between puberty and PCOS. DATA IDENTIFICATION: The most relevant studies related to this topic have been identified through a computerized bibliographic search (MEDLINE) and through manual scanning of wh…

Amylin and insulin co-replacement therapy for insulin-dependent (type I) diabetes mellitus.

Amylin is a proteinaceous hormone secreted form insulin-producing pancreatic beta-cells following stimulation by food molecules such as glucose and arginine. Amylin decreases insulin-stimulated glucose uptake in skeletal muscle and counteracts the ability of insulin to suppress output of glucose from the liver. Substantial evidence supports the view that maylin is a second glucoregulatory hormone produced from islet beta-cells, which can modulate a number of metabolic processes also regulated by insulin. The islet beta-cell may therefore transmit a dual message to peripheral tissues through th…

Insulin autoimmune syndrome associated with benign monoclonal gammopathy. Evidence for monoclonal insulin autoantibodies.

A 64-yr-old man with benign monoclonal gammopathy developed recurrent episodes of severe hypoglycemia but lacked evidence of insulinoma or exogenous insulin administration. The patient's plasma was found to contain anti-insulin antibodies and large amounts of extractable insulin (1110 microU/ml), which was identified as human insulin by high-performance liquid chromatography (HPLC). The anti-insulin antibodies consisted solely of IgG and lambda-light chains. Scatchard analysis of these antibodies revealed an almost straight-line relationship, with markedly low affinity and high capacity. An im…

Comparative clinical pharmacology of human insulin (Novo) and porcine insulin in normal subjects.

The pharmacokinetics and pharmacologic effects of human insulin (Novo) derived from porcine insulin were studied by a double-blind crossover comparison with porcine monocomponent insulin in healthy volunteers. Both insulins were given by subcutaneous (s.c.) and intravenous (i.v.) injection to healthy male volunteers. The doses of injected insulin were 0.05 U/kg and 0.1 U/kg for the s.c. study, and 0.025 U/kg and 0.05 U/kg for the i.v. study. The plasma immunoreactive insulin (IRI) increased rapidly, plasma C-peptide and glucose decreased gradually, and plasma glucagon increased transiently aft…

Decreased insulin binding in cultured lymphocytes from two patients with extreme insulin resistance.

[125I]Insulin binding has been studied in two patients with extreme insulin resistance using cultured B-lymphocytes transformed with Epstein-Barr virus. A cell line from a female infant with leprechaunism had insulin binding which was decreased 90% below the lower limit of normal. Lymphocytes from a young woman with type A extreme insulin resistance (associated with acanthosis nigricans and virilization) had insulin binding which was 80% depressed. In both cases, the defect in binding resulted from a decrease in the number of receptors per cell. The remaining receptors had normal properties, i…

Successful immunosuppressive therapy in insulin resistant diabetes caused by anti-insulin receptor autoantibodies.

A 45-year-old, non-obese female patient with no previous history of insulin administration was found to have extreme insulin resistance and abnormally high plasma immunoreactive insulin in the absence of anti-insulin antibodies in the serum. Clinically, there was no ketonuria. The patient also had evidence of Sjogren's syndrome with several immunologic features including hypergammaglobulinemia, positive antinuclear antibodies, accelerated erythrocyte sedimentation rate and leukopenia. Plasma pancreatic glucagon and C-peptide were elevated, but other endocrinologic abnormalties were not present…

Insulin signaling in the heart.

Insulin receptors are highly expressed in the heart and vasculature. Insulin signaling regulates cardiac growth, survival, substrate uptake, utilization, and mitochondrial metabolism. Insulin signaling modulates the cardiac responses to physiological and pathological stressors. Altered insulin signaling in the heart may contribute to the pathophysiology of ventricular remodeling and heart failure progression. Myocardial insulin signaling adapts rapidly to changes in the systemic metabolic milieu. What may initially represent an adaptation to protect the heart from carbotoxicity may contribute …

The future of insulin therapy.

Although insulin therapy was already introduced one-hundred years ago, insulin formulations are still being refined to reduce the risk of hypoglycaemia and of other insulin side effects such as weight gain. This review summarises the available clinical data for some ongoing developments of new insulins and evaluates their potential for future insulin therapy. Once-weekly insulins will most likely be the next addition to the insulin armamentarium. First clinical studies indicate low peak-to-trough fluctuations with these insulins indicating the potential to achieve better glycaemic control or r…
