[Hyperosmolar diabetic coma without ketoacidosis in purulent meningitis (author's transl)].

Hyperosmolar diabetic coma without ketoacidosis was observed as a complication of purulent meningitis in 3 patients. Diagnostic difficulties arise due to slow onset of hyperosmolality and misinterpretation of organic cerebral fits and the increasing disturbance of consciousness. Therapy is complicated by the necessity of treatment with anticonvulsants and sodium containing antibiotics which affect sugar and sodium balance. Abrupt lowering of serum osmolality leads to further increase of cerebral oedema. Slow normalisation of blood sugar and sodium values over several days may lead to satisfact

Diabetes and hypoglycemia in chronic pancreatitis.

Fifty-nine patients with chronic pancreatitis were studied in retrospect. The incidence of overt diabetes was high, 36/59. Half of the diabetics were insulin-dependent, and among these labile diabetes with hyperglycemia and high amounts of glucose in the urine was not uncommon. Hypoglycemic episodes were noted in 14 of the 18 insulin-treated patients, and in 3 patients severe hypoglycemia was believed to be the cause of death. Mechanisms leading to such disastrous hypoglycemia are discussed, and a hypothesis regarding lack of glucagon as the cause of severe hypoglycemic attacks was experimenta

Ambulatory long-term treatment of childhood diabetes mellitus.

Increasing evidence is accumulating to show the beneficial effect of a good balance of diabetes in protecting against diabetic microangiopathy. The pediatrician's responsibility is to recommend and supervise the best use of the three basic cornerstones of the longterm-treatment, insulin, diet and exercise. The review deals with insulin therapy focusing on insulin treatment "tailored" to the stage of the disease. Twice-daily insulin is in most cases preferable to a once-daily regimen. The risk of over-insulinization is stressed. An essential part of long-term treatment is to instruct the child

Changing profile of diabetic ketosis.

Forty-three cases of diabetic ketosis were analysed to determine the mode of presentation, treatment modalities and outcome. Among these cases 62.8% were non-insulin dependent diabetes mellitus (NIDDM) patients and 37.2% belonged to the insulin dependent diabetes mellitus (IDDM) group. Six patients had blood glucose levels of more than 250 mg/dl but less than 300 mg/dl who were grouped separately for analysis under the term "euglycaemic diabetic ketoacidosis (EGDK)". Infection was the commonest precipitating factor in diabetic ketosis in all groups. Abdominal pain and vomiting occurred with NI

Patient-specific deep offline artificial pancreas for blood glucose regulation in type 1 diabetes.

Due to insufficient insulin secretion, patients with type 1 diabetes mellitus (T1DM) are prone to blood glucose fluctuations ranging from hypoglycemia to hyperglycemia. While dangerous hypoglycemia may lead to coma immediately, chronic hyperglycemia increases patients' risks for cardiorenal and vascular diseases in the long run. In principle, an artificial pancreas - a closed-loop insulin delivery system requiring patients to manually input insulin dosage according to the upcoming meals - could supply exogenous insulin to control the glucose levels and hence reduce the risks from hyperglycemia

[Hypoglycemia in childhood diabetes].

Severe hypoglycemic episodes in diabetic children are a serious complication of present medical therapy. With the recent trend towards intensified insulin therapy, the incidence of severe hypoglycemia will probably increase. The pathophysiological mechanisms in the development of severe hypoglycemia are lack of modulation of plasma insulin levels, diminished or abolished glucagon release, delayed epinephrine release, and diminished glucose threshold for awareness of hypoglycemic symptoms, especially in well stabilized diabetics. The consequences of severe hypoglycemia are EEG changes, focal or

Hyperosmolar hyperglycemic coma in an infant with neonatal diabetes mellitus.

Neonatal diabetes mellitus (NDM) is a rare disease distinct from type 1 diabetes mellitus (T1DM). While T1DM results from autoimmune destruction of pancreatic beta cells, NDM arises from genetic mutations to insulin production and secretion pathways. We present a case of an infant presenting to the emergency department with shock and poor responsiveness, found to have neonatal DM with hyperosmotic hyperglycemic syndrome (HHS). This report distinguishes NDM from T1DM, illustrating the subtle but clinically significant differences in pathophysiology and implications for initial management of NDM

Reversal of diabetes by syngeneic transplantation of a radiation-induced rat insulinoma.

The growth and metabolic effects of a radiation-induced rat insulinoma were examined after subcutaneous subscapular transplantation into normal and streptozotocin diabetic NEDH rats. Streptozotocin diabetic rats exhibited hyperglycaemia, hypoinsulinaemia, impaired glucose tolerance without an insulin response, polyuria, polydipsia, hyperphagia and weight loss. Transplantation of tumour fragments gradually improved the physical and metabolic state over the following 3 weeks. Coincident with a progressive rise in plasma insulin between 10 and 17 days, the diabetic rats gained weight and reduced

Wernicke's encephalopathy induced by hyperemesis gravidarum, associated with bilateral caudate lesions on computed tomography and magnetic resonance imaging.

The case of an 18-year-old woman with Wernicke's encephalopathy induced by hyperemesis gravidarum is reported. She had severe vomiting and received antiemetic therapy and intravenous administration of glucose and low-dose insulin solution without thiamine. She developed coma, nystagmus, ataxia and polyneuropathy. CT and MR imaging showed bilateral caudate lesions as well as symmetrical periventricular lesions of the thalamus and hypothalamus and periaqueductal gray matter. Caudate lesions are quite rare in Wernicke's encephalopathy.

[Effect of clinical hyperthyroidism and hypothyroidism on patent diabetes. 59 cases].

Fifty-nine patients with both clinical evidence of thyroid dysfunction and patent diabetes mellitus were investigated in our diabetology department. Patients with euthyroid goitre and iatrogenic or pituitary hypothyroidism were excluded from the study. Among the 45 diabetics with hyperthyroidism, 32 had Graves' disease and 13 had toxic adenoma; 71% were insulin-treated. Hyperthyroidism had passed unnoticed in 7 of these 32 patients because fatigue and loss of weight, which initially were the predominant or sole symptoms, are extremely frequent in uncontrolled diabetes. These symptoms, as well

Treatment of diabetic ketoacidosis and non-ketotic hyperosmolar diabetic coma.

Although mortality of diabetic ketoacidosis (KA) has decreased during the past 20 yr to 1-2%, hyperosmolar non-ketotic coma (HNC) is still lethal in 20-30% of cases due to severe underlying conditions or to complications. The most frequent causes of death are infections and thromboembolic disorders. The strategies of initial treatment of KA and HNC are similar; in KA, insulin, fluid and electrolyte replacement have first priority. In HNC, rehydration and electrolyte administration are of primary importance. It is now generally recognized that insulin therapy is best performed using low doses (

["Little hedgehogs of doubts"--success and discourse of insulin shock therapy in the 1930s to 1950s].

Insulin shock therapy was one of the major somatic treatments in psychiatry in the 1930s. Even though this treatment was risky, expensive and completely empirical, it spread all over the industrialised world within few years. The development of several centres for this new treatment contributed to this success story, mainly the Viennese Psychiatric-neurological clinic and the Swiss asylum Münsingen. Visiting psychiatrists in these clinics and Jewish émigrés became important advocates of the new therapy in other countries. The success of insulin coma therapy was also based on the hope of many p…

Characteristics of severe hypoglycemia in the patient with insulin-dependent diabetes.

To avoid, delay, or ameliorate the microvascular complications of diabetes, intensive insulin therapy with the goal of normoglycemia is required. In most patients with insulin-dependent diabetes mellitus (IDDM), this cannot be achieved because of severe hypoglycemia, which is one of the major causes of morbidity in diabetic patients. To assess the frequency and characteristics of severe hypoglycemia in a single diabetologist's practice, we surveyed 211 consecutive patients with IDDM for a history of severe hypoglycemia (SH), defined as events requiring the assistance of another person. Of thes

Regional differences in the effects of insulin-induced hypoglycemia on phospholipid metabolism of rat brain.

Insulin-induced hypoglycemia in rats may lead to stimulated brain activity and if severe enough, they may develop a stupor-coma condition. In this study, the effects of insulin-induced hypoglycemia on brain phospholipid metabolism were examined in rats which were prior injected with (32)Pi. Three hours after insulin injection (1 or 5 units/100 g body wt, i.p.), there was an increase (25%) in radioactivity of the lipid phase of cerebral cortex, but radioactivity in the cerebellum tended to decrease instead. Radioactivity in the aqueous phase of cortex was not altered after insulin injection, bu

A few facts from the history of selected biological treatment methods in psychiatry.

Biological treatment of mental disorders has obviously strong links to psychopharmacology. It is no wonder that the greatest expectations fo … some efficacy. In the absence of other, more efficacious treatment methods, these therapies-such as insulin coma or atropine coma-brought at least some relief to those who suffered from serious mental disorders. Obviously, at present we are aware that these therapies should be considered far less efficient than modern psychotropic drugs; however, at the time w …
