We report a case of thyrotoxicosis in a male patient under alpha-interferon (INF-alpha) therapy. Data from the literature show that treatment with IFN-a alone or combined with interleukin-2 (IL-2) can induce dysthyroidism (hypothyroidism, hyperthyroidism, thyroiditis). The frequency of dysthyroidism under INF-alpha therapy ranges from 10 to 40% depending on the authors. During combined INF-alpha and IL-2 therapy these figures reach 20 to 91%. We investigated cytokine mRNA expression in human palatine tonsils from 20 patients. Expression of TNF-alpha mRNA was detected in 11 of 20 patients. IL- 1 alpha mRNA was detected in 2 patients and IL-2 mRNA in one patient. No expression of IL-4, IL-6 and interferon-gamma was detected in any of the 20 patients. Fourteen patients with aggressive juvenile papillomatosis were treated with systemic administration of alpha-type interferon (IFN). This initial dosage of 2 million units alpha-interferon/m2 was modified if a favorable response in the papilloma growth occurred, or if persistent drug-related side effects developed. A total of 66 patients with advanced renal cell cancer received a combination of recombinant interferon alpha-2a (18 times 10(6) units subcutaneously 3 times weekly) and vinblastine (0.1 mg. per kg. intravenously every 3 weeks).
