The current status of intraoperative iPTH assay in surgery for primary hyperparathyroidism.

Intraoperative intact parathyroid hormone (iPTH) monitoring has been accepted by many centers specializing in parathyroid surgery as a useful adjunct during surgery for primary hyperparathyroidism. This method can be utilized in three discreet modes of application: (I) to guide surgical decisions during parathyroidectomy in one of the following clinical contexts: (i) to confirm complete removal of all hyperfunctioning parathyroid tissue, which allows for termination of surgery with confidence that the hyperparathyroid state has been successfully corrected; (ii) to identify patients with additi

Spinal subependymoma mimicking an intradural extramedullary tumor: illustrative case.

BACKGROUND: Spinal subependymomas are rare, benign tumors that typically arise within the spinal cord and usually appear as intramedullary lesions on MRI. The authors report a rare case of cervical spinal subependymoma with exophytic growth that mimicked an intradural extramedullary tumor on MRI. OBSERVATIONS: A 53-year-old man presented with right-sided paresthesia. MRI revealed a spinal tumor causing marked spinal cord compression. The lesion was isointense on T1-weighted images, slightly hyperintense on T2-weighted images, and showed no gadolinium enhancement. A focal CSF cleft between the

Vaginal Cuff Dehiscence as an Unusual Cause of Pneumoperitoneum: A Case Series.

Pneumoperitoneum is most commonly associated with gastrointestinal perforation and often prompts emergent surgical exploration; however, gynecologic sources such as vaginal cuff dehiscence (VCD) can rarely be the cause. We present two unusual cases of pneumoperitoneum secondary to VCD following remote hysterectomy, each with distinct presentations and operative findings. The first case involves a 38-year-old woman with prior Roux-en-Y gastric bypass and hysterectomy who presented with acute-on-chronic abdominal pain and imaging evidence of free intraperitoneal air. Diagnostic laparoscopy and i

Should We Scan Hidradenitis Suppurativa Patients? A Systematic Review of Radiologic Findings.

OBJECTIVE: To bring awareness and close gaps between dermatologists and radiologists about the contribution of imaging techniques for diagnosis, treatment, and follow-up of hidradenitis suppurativa (HS). DATA SOURCES: Investigators searched the PubMed database for articles on HS and radiology techniques. STUDY SELECTION: Databases were searched up to December 2018. The query retrieved 257 publications, of which 103 were unique; of these, 7 were inaccessible. From the remaining 96, 33 were irrelevant (did not discuss HS lesion features). After applying the inclusion criteria, 63 studies were

Morbidity, mortality, and quality of life following surgery for intracranial meningiomas. A retrospective study in 257 cases.

The authors report 257 patients who underwent 338 craniotomies for the removal of meningiomas. The average duration of observed survival was 9.0 years, while that with acceptable quality of life was 8.3 years. Multiple factors including the size and location of tumors, the degree of tumor excision, the histological features, and the preoperative condition of the patients are important in both duration of survival and quality of life following surgery for intracranial meningiomas. Computerized tomography permits early diagnosis and is invaluable in follow-up assessment. It allows for recognitio

Intracranial Myeloid Sarcoma Metastasis Mimicking Acute Subdural Hematoma.

Myeloid sarcoma, a rare consequence of myeloproliferative disorders, is rarely seen in the central nervous system, most commonly in the pediatric population. Although there are a handful of case reports detailing initial presentation of CNS myeloid sarcoma in the adult population, we have been unable to find any reports of CNS myeloid sarcoma presenting as a large mass lesion in a herniating patient. Here, we present the case of a patient transferred to our facility for a very large subdural hematoma. Based on imaging characteristics, it was felt to be a spontaneous hematoma secondary to coagu

Subtrochanteric Osteotomy in Direct Anterior Approach Total Hip Arthroplasty for Crowe IV Dysplasia-Surgical Technique and Literature Review.

For Crowe IV dysplasia, the clinical efficacy and surgical technique of subtrochanteric osteotomy (SO) within the direct anterior approach total hip arthroplasty (DAA-THA) was a subject of debate. This study aimed to describe the surgical technique and clinical outcomes in 11 cases of SO in DAA-THA and to summarize the relevant literature on this topic. Between June 2016 and June 2023, we retrospectively evaluated patients diagnosed with Crowe IV hip dysplasia at our institution. Criteria identified 11 patients who underwent SO during DAA-THA. Comprehensive data encompassing demographic inform

Intraoperative consultation in gynecologic pathology.

Intraoperative consultation represents an integral part of diagnostic protocols in gynecologic oncology. It may be indicated 1) to evaluate the biologic nature of a pathologic process (distinction between nonneoplastic lesions and tumors), 2) to classify the histologic type of tumor and assess its biologic behavior (typing), 3) to confirm or rule out the metastatic origin of a tumor, 4) to determine the degree of differentiation and extent of local spread of a malignant tumor (grading and staging), 5) to detect tumor deposits in lymph nodes, 6) to examine surgical resection margins, 7) to dete

A comparison of the ability of the EEGo and BIS monitors to assess emergence following neurosurgery.

PURPOSE: Deep anesthesia during microvascular decompression (MVD) for trigeminal neuralgia and cerebral aneurysm clipping may delay emergenc … ifference in wake-up times between the EEGo and BIS groups. Additionally, there were no episodes of intraoperative awareness and no differences in patient satisfaction. Compared to EEGo waveform output, a decrease in BIS output was delayed in four patients receiving propofol for burst suppression during cerebral aneurysm clipping, indicating enhanced hypnosis. One case of …

Takotsubo cardiomyopathy and anaesthesia: case report and review of the literature.

Takotsubo cardiomyopathy is an acute syndrome characterized by cardiac failure from disturbances in the contractility of the left ventricle. It is presumably caused by sympathetic over stimulation. We describe a case of postoperatively developed Takotsubo cardiomyopathy in a 69-year-old female. The syndrome developed in connection with awareness during complete residual paralysis. The literature on this syndrome is reviewed and implications for anaesthesia described.

Pseudoaneurysm of the posterior circumflex humeral artery in an avid volleyball player.

Posterior circumflex humeral artery (PCHA) aneurysms are rare, with only 52 documented cases. They are often diagnosed in athletic individuals who participate in sports requiring repetitive overhead motion. We report a case of a 55-year-old man who developed axillary pain and a perceptible mass after playing volleyball. Intraoperatively, the patient was found to have a PCHA pseudoaneurysm and underwent open surgical excision with primary repair of the axillary artery. Despite increased recognition, PCHA aneurysms remain largely underdiagnosed. Heightened awareness may reduce diagnostic delays

Anomalous Vascular Anatomy on Preoperative CT and Surgical Implications for Clavicle Fractures: A Case Report.

CASE: A 64-year-old woman sustained a right diaphyseal clavicle fracture along with ipsilateral rib fractures after being hit by a motor vehicle while cycling. She underwent open reduction and internal fixation of the right clavicle and was found intraoperatively to have an anomalous external jugular vein coursing anterior to the clavicle. CONCLUSION: Anomalous vessels around the clavicle are rare but can be a source of significant hemorrhage when injured. Cross-sectional imaging obtained as part of the trauma workup provides surgeons an opportunity to identify these vessels preoperatively; a

Amyand's Hernia: Ultrasonography Findings.

BACKGROUND: Amyand's hernia is described as the presence of an appendix vermiformis in the inguinal hernia sac. The incidence of Amyand's hernia is approximately 1% of all inguinal hernias. Amyand's hernia is diagnosed intraoperatively or preoperatively with radiologic examinations. CASE REPORT: Two cases of Amyand's hernia, with and without acute appendicitis, are reported here. WHY SHOULD AN EMERGENCY PHYSICIAN BE AWARE OF THIS?: Amyand's hernia is a rare entity, and physical signs, laboratory results, and symptoms are not always helpful in diagnosis. Inverted left atrial appendage bonded to atrial septum.

Inverted left atrial appendage (LAA) is a rare surgical complication. Our patient, a boy aged 2 years, was diagnosed with a partial atrioventricular defect with mild regurgitation of the left atrioventricular valve and a large primum atrial septal defect. Direct postoperative transesophageal echocardiography revealed a new left atrial mass attached to the atrial septum, without left ventricle inflow obstruction. Out of concern about the nature of this mass, we chose surgical direct examination. Intraoperatively, we diagnosed it as an inverted LAA accidentally attached to the atrial septum sutu

Kuttner tumor (chronic sclerosing sialadenitis).

Kuttner tumor (KT), known descriptively as chronic sclerosing sialadenitis, is a chronic inflammatory disease of the salivary gland. Clinically, it produces a firm swelling of the glands and may be difficult to distinguish from neoplasia. The diagnosis can only be made histologically and should not be difficult if the pathologist is aware of the condition's existence. Errors in diagnosis arise because KT is underrecognized and few cases have been reported in the English literature. We present a case of KT initially diagnosed as a primary salivary gland neoplasm that intraoperative frozen secti

An Asymmetric Medio-Lateral Flexion Laxity Target Yields Excellent Results in Robotic-Assisted Total Knee Arthroplasty With Functional Knee Positioning.

BACKGROUND: Functional knee positioning (FKP) in robotic-assisted (RA) total knee arthroplasty (TKA) aims to optimize soft-tissue balance through patient-specific implant fine-tuning. However, it remains unclear (1) how intraoperative ligament laxity affects joint awareness and (2) whether the discrepancy between intraoperative planned and achieved laxity is affected by preoperative coronal limb alignment or fixed flexion deformity (FFD). METHODS: This retrospective study analyzed 130 patients undergoing image-based RA TKA with FKP using a cementless cruciate-retaining implant. Intraoperative

Silent rupture: unmasking a posterior capsular rupture in a seemingly uncomplicated cataract.

Cataract surgery is typically considered routine, especially when preoperative things are normal. We present a rare intraoperative complication where the cataractous lens nucleus dropped into the vitreous cavity immediately following viscoelastic injection-prior to capsulorhexis-in a patient with Fuchs' heterochromic iridocyclitis and no overt signs of zonular instability or capsular pathology. The case underscores the need for heightened surgical awareness in seemingly uncomplicated cataract cases, particularly in the presence of subtle clinical history. Surgical management included anterior
