Sickness certification difficulties in Ireland--a GP focus group study.

BACKGROUND: Sickness certification causes problems for general practitioners (GPs). Difficulty with the assessment of capacity to work, conflict with patients and other non-medical factors have been shown to influence GPs' decision-making. Inadequate leadership and management of certification issues add to GPs' difficulties. AIMS: To explore problems associated with sickness certification, as part of a larger mixed method research project exploring GPs' experiences and perceptions of sickness certification in Ireland. METHODS: A qualitative study in an urban region of Ireland. A focus group …

Molecular epidemiology of circulating measles virus in Ireland 2002-2007.

The molecular characterization of measles virus (MeV) is a valuable epidemiological tool to monitor virus transmission and to discriminate between imported and endemic infection. There has been significant immigration into Ireland in recent years and many individuals originate from regions of high measles incidence. Ireland has had a number of outbreaks of MeV which appear attributable to sub-optimal vaccine uptake and possibly imported strains as new genotypes have been identified in recent years. To ascertain any significant changes in circulating measles genotypes we investigated 65 confirm…

Characterization of Staphylococcus aureus isolated from dairy animals in Ireland.

Enterotoxigenic Staphylococcus aureus frequently contaminate milk and milk products causing food poisoning. Staphylococcus aureus were isolated from bovine, ovine and caprine milk and milk filters from 78 dairy production holdings supplying the farmhouse cheese sector in Ireland, using standard culture methods. Molecular methods were applied to study the distribution of genes encoding staphylococcal enterotoxins and toxic shock syndrome toxin in the collection. Multilocus variable number tandem repeat analysis was used to subtype the collection. One hundred and two Staphylococcus aureus (54 mi…

Human seroprevalence to Coxiella burnetii (Q fever) in Northern Ireland.

Despite the widespread prevalence of infection with Coxiella burnetii, there have been few large population-based studies examining the epidemiology of this infection. The aim of this study was to examine the distribution and determinants of C. burnetii past infection in Northern Ireland (NI). Coxiella burnetii phase II specific IgG antibodies were measured by enzyme-linked immunosorbent assay in stored serum from 2,394 randomly selected subjects, aged 12-64, who had participated in population-based surveys of cardiovascular risk factors performed in 1986 and 1987. The overall prevalence of C.…

Lifestyle-related cancer death rates in Ireland: decreasing or increasing?

We examined death patterns of tobacco-related cancers (lung, oesophagus, larynx, and pharynx) and of non-tobacco related cancers (breast and prostate) from 1972 to 2001 in Ireland, utilizing the WHO Cancer Mortality Database. Estimated-annual-percent-changes (EAPC) in age-adjusted cancer death rates (standardised to world standard population) were calculated by age (10-year groups), period (10-year intervals) and sex. Overall, EAPCs of all the cancer sites studied showed a declining trend in the most recent period (1992-2001), with male oesophageal cancer rates showing a deceleration between 1…

MRSA bacteraemia: North/South Study of MRSA in Ireland 1999.

Retrospective aggregate data on all Staphylococcus aureus isolates recovered from blood cultures during 1998 were collected in both jurisdictions on the island of Ireland, Northern Ireland (North) and the Republic of Ireland (South), as part of the North/South Study of MRSA in Ireland 1999. A postal questionnaire was used to gather the data, and all diagnostic microbiology laboratories in the North and 98% of laboratories in the South participated. S. aureus bacteraemia occurred at rates of 20.4 per 100,000 population in the North and 24.5 per 100,000 in the South (missing data from one labora…

Acinic cell carcinoma in Northern Ireland: a 10-year review.

12 cases of acinic cell carcinoma diagnosed in Northern Ireland from 1942 to 1982 on which there was at least 10 years' follow-up were reviewed, by pooling cases from all regional pathology laboratories. Clinical findings at presentation were established from case notes, together with details of eventual clinical outcome. Sections were examined to determine tumour size, morphological pattern and cytological constitution, the presence or absence of cytological atypia, mitotic activity, lymphocytic and desmoplastic responses, and to assess for infiltrative margins and adequacy of excision. The t…

Brain abscesses in Northern Ireland: a 30 year community review.

One hundred and seventy-two cases of intracranial abscesses, obtained from Neurosurgical and Centralised Autopsy Records for Northern Ireland for the 30 years, 1947--1976, have been reviewed. The incidence of the disease has fallen from five to three per million of population per year over the past three decades. Intracranial abscesses were three times as common in males as in females. Twenty-nine per cent of the abscesses were in the temporal lobe, 25% frontal, 10% parietal, 6% cerebellar, 3% occipital, and 7% were either subdural or in deep sites such as the thalamus; the remainder (20%) wer…

Disparities in the Epidemiology of COVID-19 in Irish Travellers in the Republic of Ireland.

INTRODUCTION: Irish Travellers, a distinct minority ethnic group, make up 0.64% of the population in the Republic of Ireland (n = 32,949) according to the national census. Major health disparities exist between Travellers and the general population including shorter life expectancy and higher mortality rates from respiratory and cardiovascular disease. METHODS: Here, the epidemiology of COVID-19 in Irish Travellers was investigated. Public health surveillance data on 238,907 notified COVID-19 cases in Ireland from March 2020 to April 2021 were analysed. RESULTS: The crude incidence rate (CIR…

Developing and Calibrating a Colorectal Cancer Microsimulation Model for Northern Ireland.

BACKGROUND: Individual-level microsimulation models are essential for evaluating colorectal cancer (CRC) screening programmes to capture the heterogeneity in disease progression. To ensure regional relevance, such models require detailed natural history structures and robust calibration to population-specific data. This study presents the development of the first CRC natural history microsimulation model tailored to Northern Ireland (NI) for evaluating the NI Bowel Cancer Screening Programme (NI BCSP). METHOD: The model simulates individual trajectories from adenoma onset to CRC diagnosis. Ei…

Non-Wilms Renal Tumours in Children: The Republic of Ireland Experience.

Background: Non-Wilms renal tumours (NWRTs) are rare paediatric malignancies and account for a small but clinically significant proportion of childhood renal cancers. Due to their low incidence and biological heterogeneity, outcome data are limited, and management is largely extrapolated from international collaborative protocols. No national data describing the incidence and outcomes of NWRTs in children in the Republic of Ireland (ROI) have previously been published. Objective: To determine the incidence, treatment strategies, and survival outcomes of NWRTs in children in the ROI. Methods: A…

Action potential: Impact of climate change on neurological disease in Ireland.

Anthropogenic climate change has led to a significant increase in global average temperatures and the frequency of extreme heat events. Rising temperatures have a broad range of health implications and have the potential to significantly impact neurological disorders and brain health. A growing body of evidence suggests that the incidence and severity of many common neurological conditions (stroke, epilepsy, headache, neuroinflammation, and neurodegenerative disorders) are affected by climate change. Ireland's average temperature is rising, and we are likely to experience extreme heat events m…
