The present article discusses specific, directly gene-dependent ultrastructural markers of dominantly inherited epidermal disorders that serve as clues to their underlying molecular genetic abnormalities. These are epidermolysis bullosa simplex Koebner and Weber-Cockayne with rupture or non-assembly of basal cell keratins and point mutations in keratins 5 and 14. Clumping of basal cell keratins is pathognomonic of EB Dowling-Meara and caused by mutations in hot spots of the rod domain of K5 and K 14. Clumps and aggregates of basal keratins occur side by side in the same cell and thus do not in

Schöpf-Schultz-Passarge syndrome is a rare ectodermal dysplasia described by Schöpf in 1971. The disease is characterized by hypodontia, hypotrichosis, palmoplantar keratoderma, hypoplastic nails and eyelid hydrocystomas. We report the case of a 71-year-old man, the first French description of this syndrome, who remarkably presented all the signs. The patient also had skin tumors, multiple eccrime hidrocystomas and aspectific macular degeneration. A review of the literature is discussed.

Olmsted syndrome is a very rare and severe cicatrizing keratoderma associated with periorificial lesion. Most cases are sporadic but familial occurrence has been also seen. Till now around 73 cases have been reported and none of the reported cases have 4 siblings affected from this disease. We are reporting cases of 4 siblings of age 30 year female, 26 year female, 20 year male and 10 year male who were born to a third degree consangueinous marriage and presented with palmoplantar keratoderma, periorificial hyperkeratosis, flexion deformity, pseudoainhum and contracture of digits. There was no

PLACK syndrome (OMIM 616295) is a rare genodermatosis associated with peeling skin, leukonychia, acral punctate keratosis, cheilitis, and knuckle pads and is caused by loss-of-function mutations in the CAST gene, which encodes calpastatin, a calcium-dependent protease. This case report highlights a case of PLACK syndrome presenting with the unique findings of striate hyperkeratosis on the palms as well as life-threatening cardiomyopathy. We review why CAST mutations might impact cardiac function and raise awareness of the potential association between PLACK syndrome and cardiac manifestations.

We report a case of a 15-year-old boy with hyperkeratotic lesions that were linear or striated on the palms and nummular on the soles. He was the only family member known to be affected, suggesting that the condition could be attributed to a de novo mutation or the recessive form of keratoderma palmoplantaris striata, described by Degos as chronic idiopathic acrokeratosis. The lesions did not improve with topical treatments (keratolytic agents, emollients, or corticosteroids) or oral retinoids. We observed that scratching of the affected areas was the main reason for deterioration of the lesio

Papillon-Lefevre syndrome and Haim Munk syndrome are palmoplantar keratodermas associated with premature periodontal destruction. The additional findings of Haim Munk Syndrome include onychogryphosis, arachnodactyly, acral osteolysis and pes planus. Both are associated with mutations in the lysosomal protease cathepsin C. We describe a patient with phenotype for Haim Munk Syndrome and genetic analysis revealed a homozygous point mutation in exon 1 of the gene encoding cathepsin C.

The Schöpf-Schulz-Passarge syndrome is a rare genodermatosis with autosomal recessive transmission. It is characterized by palmoplantar keratoderma, eyelid apocrine hydrocystomas, hypodontia, hypotrichosis and hypoplastic nails. Several epithelial tumors have been described in this syndrome. This report describes a case with actinic keratoses, two tumors of the follicular infundibulum and one poroma with follicular differentiation. This is the first report of an association between the Schöpf-Schulz-Passarge syndrome and a poroma with follicular differentiation.

Paraneoplastic syndromes with cutaneous findings localized to the epidermis are discussed in this article. A paraneoplastic syndrome is a condition that arises in association with a malignancy elsewhere in the body but, in itself, is not cancerous. Generally, the onset and course of the disease will closely correlate with the malignancy, as described in Curth's original criteria for paraneoplastic syndromes, although this is not always the case. Subjects discussed include malignant acanthosis nigricans, the sign of Leser-Trélat, tripe palms, palmoplantar keratodermas, Bazex syndrome, and acqui

Pityriasis Rubra Pilaris (PRP) is an uncommon skin disorder characterized by follicular keratosis, palmoplantar keratoderma, and erythroderma. The traditional preferred treatment is oral retinoids, but over the last decade, biologic therapy with anti-TNF agents has been used with success. We report the case of a 51 year- old man with a clinical and histopathological diagnosis of PRP. He underwent therapy with adalimumab and showed clearance of skin lesions within the fourth week of treatment.

Paraneoplastic acrokeratosis of Bazex (PAB) or Bazex's syndrome is a rare, paraneoplastic syndrome that mostly affects men over 40 years old, and generally associates with squamous cell carcinoma of the upper aerodigestive tract. We describe a 57-year-old woman with the characteristic features of PAB, including violaceous erythematous, scaling eruption, palmoplantar keratoderma and nail dystrophy. Further analysis revealed the underlying neoplasm to be cholangiocarcinoma. The clinical aspects of PAB and associated neoplasms are reviewed.

We present a case of keratosis punctata involving the instep of both feet in addition to palmar and plantar creases, a finding not previously reported to our knowledge. We also discuss a closely related entity, keratosis punctata palmaris et plantaris (KPPP), and why we believe our case does not simply represent a variant of keratosis punctata of the palmar creases (KPPC).
