Primary total knee arthroplasty using constrained condylar knee design for severe deformity and stiffness of knee secondary to post-traumatic arthritis.

BACKGROUND: Key to a successful outcome of total knee arthroplasty (TKA) is to attain optimum alignment, adequate balance, and deformity correction. In primary TKA, this can be achieved efficiently by posterior stabilized (PS) design with or without the sub-periosteal release. However, certain circumstances such as post-traumatic arthritis are often associated with severe deformities with a significant bone defect, stiffness, and instability. Such deformities are extremely difficult to balance with soft tissue release only and require additionally constrained prostheses even in primary TKA. In…

Factors influencing ankle alignment changes following medial unicompartmental knee arthroplasty: Preoperative knee and ankle deformities and extent of knee alignment correction.

INTRODUCTION: The impact of medial unicompartmental knee arthroplasty (MUKA) on ankle alignment is not well-studied. This study aims to investigate the changes in ankle alignment following MUKA and identify the influencing factors. MATERIALS AND METHODS: A retrospective analysis included 175 patients undergoing MUKA between 2018 and 2020. Patients were categorized into varus (n =  113) or valgus (n =  62) ankle groups based on preoperative ankle deformities. Preoperative and postoperative full-length standing radiographs were used for radiographic measurements. RESULTS: Following MUKA, signi…

The patient acceptable symptom state for the knee society score, oxford knee score and short form-36 following unicompartmental knee arthroplasty.

PURPOSE: The patient acceptable symptom state (PASS) is a target value on a patient-reported outcome measures (PROM) scale beyond which patients deem themselves to have attained an acceptable outcome. This study aimed to define the PASS thresholds for generic and knee-specific PROMs at 2 years after unicompartmental knee arthroplasty (UKA). METHODS: Prospectively collected data of 955 patients who underwent UKA for medial osteoarthritis at a single institution was reviewed. Patients were assessed preoperatively and 2 years postoperatively using the Knee Society Knee Score (KSKS), Function Sco…

Should Medicare Remove Total Knee Arthroplasty From Its Inpatient Only List? A Total Knee Arthroplasty Is Not a Partial Knee Arthroplasty.

BACKGROUND: The Centers for Medicare and Medicaid Services have solicited comments to consider removing total knee arthroplasty (TKA) from the Inpatient Only list, as it has done for unicompartmental knee arthroplasty (UKA). The purpose of this study is to determine whether Medicare-aged patients undergoing TKA had comparable outcomes to those undergoing UKA. METHODS: We queried the American College of Surgeons-National Surgical Quality Improvement Program database for all patients aged 65 years or older who underwent elective TKA or UKA from 2014 and 2015. Demographic variables, comorbiditie…

Improved knee biomechanics among patients reporting a good outcome in knee-related quality of life one year after total knee arthroplasty.

BACKGROUND: It is not well understood why one in five patients report poor outcomes following knee arthroplasty. This study evaluated changes in knee biomechanics, and perceived pain among patients reporting either a good or a poor outcome in knee-related quality of life after total knee arthroplasty. METHODS: Twenty-eight patients (mean age 66 (SD 7) years) were included in this prospective study. Within one month of knee arthroplasty and one year after surgery, patients underwent three-dimensional (3D) gait analysis, completed the Knee Injury and Osteoarthritis Outcome Score (KOOS), and rat…

The influence of knee position during static calibration trials on evaluation of knee loading during gait in individual with medial knee osteoarthritis.

Quantitative three-dimensional gait analysis has been used to evaluate the loading at the knee (i.e. external knee adduction moment, EKAM) during level ground walking in individuals with knee osteoarthritis (OA). The magnitude of EKAM can be influenced by some factors, such as knee marker position and foot placement angles in static calibration trials, which may lead to inaccurate functional assessments and intervention planning. This study aimed to clarify the effects of knee position during static calibration trials on the evaluation of knee loading during gait in individuals with medial kne…

Septic knee: multimodality approach.

Septic arthritis of the knee is an acute infection that can rapidly destroy articular cartilage if not promptly recognized and treated. Early diagnosis is often difficult because symptoms overlap with crystal or inflammatory arthropathies and post-traumatic effusions, while fever and laboratory markers may be absent or nonspecific. Synovial fluid analysis remains the diagnostic cornerstone, but imaging is essential to confirm effusion, define the extent of synovitis and periarticular involvement, and detect complications such as osteomyelitis or abscess. Radiography provides a rapid baseline, …

Juvenile Knee Osteochondritis Dissecans.

Osteochondritis dissecans (OCD) is defined as "a focal, idiopathic alteration of subchondral bone with risk for instability and disruption of adjacent articular cartilage that may result in premature osteoarthritis." Hypotheses regarding the etiology of OCD include ischemia, repetitive trauma, accessory centers of ossification, and genetic factors. OCD may present in any joint and throughout the articular surface but most commonly presents at the lateral aspect of the medial femoral condyle. Knee pain is the most commonly presenting symptom, and nearly one-third of patients present with bilate…

Knee Injuries in Baseball.

While knee injuries account for just 7% of injuries in professional and collegiate baseball, they are the fourth most common cause of missed professional games. The 3 most common knee-related issues in baseball are patellar tendinopathy, meniscus tears, and medial collateral ligament sprains. While injuries such as anterior cruciate ligament tears and patellar dislocations are less common, they are notable due to their potentially significant effect on time missed. Treatment considerations of all these pathologies are multifactorial and should include skeletal maturity, level of play, timing o…

Knee Arthrocentesis in Adults.

Arthrocentesis of the knee is a procedure in which a needle is inserted into the knee joint, and synovial fluid is aspirated. An arthrocentesis can be diagnostic or therapeutic. Synovial fluid may be removed for testing to determine the nature of the knee effusion. If septic arthritis is suspected, urgent arthrocentesis before initiation of antibiotic treatment is indicated. Moreover, arthrocentesis can also aid in diagnosing crystal-induced arthritis such as gout or pseudogout, or non-inflammatory arthritis such as osteoarthritis. Identifying the cause of the knee effusion can guide treatment…

Multiple Ligament Knee Reconstructions.

Patients with multiligament knee injuries require a thorough examination (Lachman, posterior-drawer, varus, valgus, and rotational testing). Diagnoses are confirmed with magnetic resonance imaging as well as stress radiographs (posterior, varus, and valgus) when indicated. Multiple systematic reviews have reported that early (<3 weeks after injury) single-stage surgery and early knee motion improves patient-reported outcomes. Anatomic-based reconstructions of the torn primary static stabilizers and repair of the capsular structures and any tendinous avulsions are performed in a single-stage. O…

Spontaneous atraumatic knee haemarthrosis.

Spontaneous knee haemarthrosis is a rare condition, most prevalent in the elderly with osteoarthritis. Recent reports have proposed that the source of bleeding is the peripheral arteries supplying the posterior horn of the lateral meniscus. In this case, a 62-year-old active man presented with acute postero-lateral left knee pain and swelling with limited weight bearing or movement of the knee. No recent history of trauma but history of lateral compartment dominant osteoarthritis and arthroscopic partial lateral meniscectomy of left knee. Aspiration showed a large haemarthrosis and following a…

The Irreducible Knee Dislocation.

Irreducible knee dislocations are a small subset of acute knee dislocations and are extremely rare. The most common type of irreducible knee dislocation is posterolateral, which can be challenging to diagnose both clinically and on plain radiographs. Vascular injury is uncommon and closed reduction is seldom possible due to medial femoral condyle buttonholing through soft tissues requiring open or arthroscopic reduction. Ligament injuries mostly include medial collateral ligament, anterior cruciate ligament, and posterior cruciate ligament. Literature has numerous case reports with a couple of…
