BACKGROUND: Joint injury is a major risk factor for osteoarthritis and provides an opportunity to prospectively examine early processes associated with osteoarthritis. We investigated whether predefined baseline demographic and clinical factors, and protein analytes in knee synovial fluid and in plasma or serum, were associated with clinically relevant outcomes at 2 years after knee injury.
METHODS: This longitudinal cohort study recruited individuals aged 16-50 years between Nov 1, 2010, and Nov 28, 2014, across six hospitals and clinics in London, UK. Participants were recruited within 8 we

Injury to the knee during athletics is common and may limit future sports participation, but its long-term effects on patients are less well characterized. Examining the development of end-stage osteoarthritis (OA) in these patients may help better clarify this relationship. We hypothesize that sports-related knee injuries are associated with subsequent unilateral knee OA and need for total knee arthroplasty (TKA) relative to bilateral knee replacement. We present a single-hospital case-control study of 124 consecutive patients undergoing primary TKA over a 6-month period for end-stage OA. Pat…

Vascular injury after total knee arthroplasty (TKA) is exceedingly rare, with the potential to have catastrophic consequences. This injury has the potential to become more common with the rising incidence of this procedure and an aging population. We present a case of a 71-year-old woman who suffered from chronic instability and multiple dislocations of TKA following two falls from standing, both of which occurred and were observed in the hospital. She sustained a popliteal artery injury, with a subsequent bypass performed the same day. The course was complicated by recurrent dislocation requi

OBJECTIVE: The knee ligaments, as a passive knee joint stability device, provide protection for the knee joint and ensure its functional integrity. This role has long been known and recognized by people. The original purpose of knee ligament reconstruction after knee ligament injury is to restore its anatomical structure and mechanical stability mechanism.
METHODS: Taking athletes as the research object, randomized controlled trials (RCTs) on improving ankle joint function of athletes related to proprioception training at home and abroad were included. The search time was from the establishme

BACKGROUND: Changes in the joint microenvironment after an injury to the articular surface of the knee have been implicated in the pathogenesis of osteoarthritis. While prior studies focused on changes in this microenvironment after anterior cruciate ligament ruptures, few have explored the biomarker changes that occur in the setting of meniscal injuries.
PURPOSE: To determine whether meniscal injury results in significant alterations to synovial fluid biomarker concentrations as compared with noninjured contralateral knees. Additionally, to explore the relationship between synovial fluid bio

OBJECTIVE: This study aims to explore a grading diagnostic method for the binary classification of meniscal tears based on magnetic resonance imaging radiomics. We hypothesize that a radiomics model can accurately grade meniscal injuries in the knee joint. By extracting T2-weighted imaging features, a radiomics model was developed to distinguish meniscal tears from nontear abnormalities.
MATERIALS AND METHODS: This retrospective study included imaging data from 100 patients at our institution between May 2022 and May 2024. The study subjects were patients with knee pain or functional impairme

BACKGROUND: Injuries to the medial structures of the knee are common in multi-ligamentous knee injuries (MLKIs), which account for 0.02% of orthopedic injuries each year. The most common medial structure involved is the superficial medial collateral ligament (sMCL) with possible additional injury to the posterior oblique ligament (POL) and deep medial collateral ligament (dMCL). There has been little evidence for the superior management of these structures regarding the use of repair or reconstruction to reproduce overall knee function. Although reconstruction may provide more valgus stability…

Cartilage injuries are common and may predispose to early accelerated osteoarthritis. Magnetic resonance (MR) imaging is critical in the detection and characterization of acute and chronic cartilage injuries. Several surgical approaches including microfracture, osteochondral allografting, autologous osteochondral transfer system, and autologous chondrocyte implantation have been developed to treat cartilage lesions. The goals of these treatments include relief of symptoms, restoration of joint function, and delay of progression of osteoarthritis. Imaging evaluation by MR imaging plays a crucia

We present an unusual and complex case of a 16-year-old adolescent male who injured his right knee and sustained combined avulsion injuries of posterior cruciate ligament (PCL) at the tibial insertion site, iliotibial band at lateral tibial condyle, and lateral collateral ligament (LCL) at femoral insertion site akin to osteoligamentous posterolateral corner injury. Anatomical reduction of the femoral LCL avulsion fragment was performed and fixed with a two 4-mm partially threaded cancellous screw. Iliotibial band avulsion was buttressed using Ellis t-plate and fixed with two 4-mm partially th

OBJECTIVE: Floating knee injuries are complex injuries and are usually caused by high-velocity trauma. These injuries are often associated with life treating injuries, which should take precedent over extremity injuries. The authors reviewed the outcomes of floating knee injuries managed in this institute from 2003 to 2015.
METHOD: A retrospective study was conducted of all patients with floating knee injuries from2003 to 2015. Twelve patients were included in the study. Data related to fracture type, associated injuries, treatment modalities, and complications were noted. Functional assessme…

BACKGROUND: It is suggested the anterior cruciate ligament (ACL) plays a significant role in knee proprioception, however, the effect of ACL injury on knee proprioception is unclear. Studies utilising the two most common measurement techniques, joint position sense and threshold to detect passive motion, have provided evidence both for and against a proprioceptive deficient following ACL injury.
OBJECTIVE: The objective of the study was to undertake a meta-analysis investigating the effects of ACL injury, treated conservatively or by reconstruction, on proprioception of the knee, measured usi

BACKGROUND: Prolotherapy is an alternative therapy for chronic musculoskeletal injury including joint laxity. The commonly used injectant, D-glucose (dextrose), is hypothesized to improve ligament mechanics and decrease pain through an inflammatory mechanism. No study has investigated the mechanical effects of prolotherapy on stretch-injured ligaments.
HYPOTHESES: Dextrose injections will enlarge cross-sectional area, decrease laxity, strengthen, and stiffen stretch-injured medial collateral ligaments (MCLs) compared with controls. Dextrose prolotherapy will increase collagen fibril diameter

Meniscal and articular cartilage injuries in skeletally immature patients appear to be occurring with increased frequency, particularly in athletically active children. The orthopaedic surgeon should understand the principles of diagnosis and management, as well as be aware of current surgical treatment options available.

There is controversy about the incidence of and predisposing factors to the development of peripheral nerve palsy after total knee arthroplasty (TKA). In this study, 19 patients with a documented neurologic complication were identified after a retrospective review of 1,476 primary TKAs performed between January 1970 and December 1998 at the New York Presbyterian Hospital at Columbia University, for an overall incidence of 1.3%. Contrary to previously published data, valgus deformity, flexion contracture, the use of postoperative epidural anesthesia, the prolonged use of pneumatic tourniquets,

We created an experimental model to evaluate the effects of strain rate on the mechanism of combined cruciate ligament injuries in knee hyperextension. Using straight knee hyperextension to rupture the anterior and posterior cruciates, two strain rates (approximately 100% per second and 5400% per second) were applied to reproduce two clinical injury patterns of the knee: low energy (sporting) and high energy (pedestrian-motor vehicle accident). Ten pairs of fresh-frozen cadaveric knees were injured to 45 degrees of hyperextension. Strain rate sensitivity of the posterior cruciate ligament was

Total knee arthroplasty (TKA) is a commonly performed orthopedic procedure. The incidence of vascular complications after TKA is low. However, these complications may be debilitating, limb-threatening, and potentially avoidable. Our first patient, who had no preexisting vascular occlusive disease, developed an ischemic extremity after TKA. The second patient underwent TKA and was diagnosed with a severely ischemic limb 48 hours postoperatively. Both patients underwent above-knee to below-knee popliteal artery saphenous vein bypass grafting. Although limb salvage was obtained in both cases, one

Twelve patients with swelling and functional embarrassment of the knee joint caused by a traffic accident were radiographically found to have tibial plateau fractures (7 patients), posterior dislocation (1), fracture of anterior tibial spine (1), single fracture of the posterior tibial spine (1), anterior dislocation (1), and negative findings (1). Arthroscopy was performed 2 to 10 days after trauma, revealed associated injuries not shown on X-ray films in every knee including rupture of cruciate ligaments (ACL 8, PCL 1), tear of menisci (lateral one 8; medial one 2), and disruption of collate

The damage of the knee extensors was examined with integrated EMG and with histochemical and electron microscopical methods in 23 patients with meniscal injury. The integrated DMG has proved in 2/3 of the patients tonic, in 1/3 both tonic and phasic damage of the muscle function. With histochemical examination the decrease of the ratio of the fibers of type 1 was found and also the atrophy of the same fibre type cells in 18-36 per cent. Ultrastructural examinations have shown out of the damage of the contractive elements an increase of the interstitial connective tissue together with alteratio

During 1959 to 1986, 62 Cases with cruciate ligament injury of the knee joint were treated in our hospital. Thirty of these cases were anterior cruciate ligament injuries and 32 cases were posterior The diagnosis of the cruciate ligament injuries was made by stability tests of the cruciate ligament. When the knee was greatly swollen with multiple ligament injuries, a series of X-ray films should be taken to test the knee stability under local or nerve block anesthesia. Fresh tear of the cruciate ligament (33 cases) was treated with immediate surgical repair except in one. Old ligament injuries
