Neuro-otologic manifestations of sarcoidosis.

Sarcoidosis is an idiopathic disease which presents in anatomic areas of concern to otorhinolaryngologists. It can cause dysfunction of both auditory and vestibular systems. In patients known previously to have sarcoidosis, this disease should be seriously considered. In patients presenting with otologic disorders and associated facial nerve paralysis or other neuropathies, uveitis, granulomatous meningitis or diabetes insipidus, sarcoidosis should be suspected. An examination of the eyes as well as a chest X-ray is imperative. Sudden and fluctuating neurosensory hearing loss has been reported

Otologic disorders causing dizziness, including surgery for vestibular disorders.

This chapter will focus on vertigo/dizziness due to inner-ear malformations, labyrinthine fistula, otosclerosis, infectious processes, and autoimmune inner-ear disorders. Inner-ear malformation due to dehiscence of the superior semicircular canal is the most recently described inner-ear malformation. Vertigo/dizziness is typically induced by sound and pressure stimuli and can be associated with auditory symptoms (conductive or mixed hearing loss). Labyrinthine fistula, except after surgery for otosclerosis, in the context of trauma or chronic otitis media with cholesteatoma, still remains a ch

Delayed endolymphatic hydrops as a clinical entity.

Delayed endolymphatic hydrops (DEH) is a clinical entity that can be differentiated from Ménière's disease and is typically observed in patients who have been suffering from longstanding unilateral profound inner-ear hearing loss. DEH probably is caused by delayed atrophy or fibrous obliteration of the endolymphatic resorptive system of the membranous labyrinth. The time that elapses between the occurrence of hearing loss and the onset of DEH can range from 1 to 74 years. The most common cause of hearing loss preceding DEH is juvenile-onset unilateral profound deafness (early childhood unilate

Vestibular histopathology of the human temporal bone. What can we learn?

Histologic sections of the human temporal bone display snapshots of the entire lifetime integrated into the moment the bone enters fixative. The bulk of the literature on vestibular histopathology is anecdotal and descriptive in nature, rather than quantitative. This is because the means of describing and measuring patients' vestibular symptoms are poorly developed, and the complex geometry of the vestibular labyrinth complicates efforts to study it in serial histologic sections. Histopathologic findings in the common peripheral vestibulopathies, including Meniere's syndrome, benign paroxysmal

Internal auditory artery infarction: clinicopathologic correlation.

OBJECTIVE: To study the pathophysiology of labyrinthine infarction. BACKGROUND: The syndrome of sudden onset vertigo or hearing loss is commonly attributed to inner ear vascular disease, yet histologic studies of isolated labyrinthine infarction in humans have been rare and have not included a complete examination of the vertebrobasilar vascular system. METHODS: Temporal bones, brainstem, cerebellum, and the supplying blood vessels were subjected to gross and microscopic postmortem examinations in a 92-year-old woman who had a sudden onset of vertigo and hearing loss in the right ear 7 years

Viral labyrinthitis: early pathology in the human.

The histologic findings in the temporal bones of three patients who died from viral encephalopathy are presented. Pathology was restricted to the scala media, vestibular labyrinth, and internal auditory canal and was considered to be expressions of viral labyrinthitis. The changes were different degrees of degeneration of the organ of Corti, early encapsulation of the tectorial membrane, degeneration of the stria vascularis, and round cell infiltration of the modiolus and contents of the internal auditory canal. A new finding in the organ of Corti and early stages of cystic degeneration of the

[Cochleovestibular syndrome disclosing dolicho-ectasis of the basilar trunk and vertebral artery].

The authors present a case of a cochleovestibular syndrome with a scanner showing a calcified dolico-ektasis of the basilar truncus and the left vertebral artery. The patient had typical Ménière's disease symptomatology. A retrolabyrinthic diagnosis with substantial recruitment was indicated by the results of the audio-vestibular explorations (stapedius reflex, electroculography, early brainstem evoked response, electrocochleography). The electrocochleography with glycerol test indicated labyrinthic hydrops. Finally, some reflections on the role of neurovascular problems in the pathogeny of so

[The indications for middle ear instillation of gentamicin in Menière's disease (author's transl)].

Menière's disease treated by use of Gentamicin sulphate instillation into the middle ear is reported. This drug is injected via a small plastic tube which is inserted into the middle ear. 4 mgm (0.1 ml) of Gentamicine sulphate are instilled every 5 hours until the labyrinth begins to show a reaction. After this treatment 95 per cent of patients no longer suffer from vertigo and tinnitus and the pressure feeling are favourably influenced. The hearing mainly remains unchanged, but sometimes may even improve.

Altered gene expression and spongiotrophoblast differentiation in placenta from a mouse model of diabetes in pregnancy.

AIMS/HYPOTHESIS: Pregnancies complicated by diabetes have a higher risk of adverse outcomes for mothers and children, including predisposition to disease later in life, e.g. metabolic syndrome and hypertension. We hypothesised that adverse outcomes from diabetic pregnancies may be linked to compromised placental function, and sought to identify cellular and molecular abnormalities in diabetic placenta. METHODS: Using a mouse model of diabetic pregnancy, placental gene expression was assayed at mid-gestation and cellular composition analysed at various stages. Genome-wide expression profiling

[New prospects in the treatment of vertigo].

Vertigo results from sensory conflicts that are usually intralabyrinthine or between two labyrinths. Symptomatic treatments rests on the reduction of these conflicts by vestibuloplegic drugs. However, this treatment should not last for more than 48 or 72 hours since in established vestibular lesions central compensation demands that the centres be informed. The use of sedative drugs in vertigo, therefore, should be reduced, and vestibular rehabilitation is recommended in most cases. Only Meniere's disease escapes this rule, being a self-limited process.

Review: morphological changes associated with endolymphatic hydrops.

Endolymphatic hydrops of the inner ear is identified as a swelling of the endolymphatic spaces. This morphopathology in man can only be confirmed at post-mortem examination although it is believed to underlie the auditory dysfunction and vestibular disturbances associated with Menière's disease. This is an illusive inner ear disorder characterized typically by the fluctuant hearing loss, tinnitus and episodes of vertigo. Menière's disease remains a major problem in otorhinolaryngology since the cause of the disease is not known and various treatments are recommended, often with unsatisfactory

Benign paroxysmal vertigo of childhood.

Benign paroxysmal vertigo of childhood is a vestibular disorder characterized by multiple sudden brief episodes of true vertigo with nystagmus usually beginning after the age of 4. The diagnosis is based on the characteristic history, because the otologic and neurologic examinations, electronystagmogram, audiogram, and electroencephalogram are usually normal. The etiology of the disorder remains unknown, although a vascular disturbance of the posterior circulation with ischemia of the labyrinth or vestibular nuclei is most commonly postulated. A review of the literature and five new cases are

Perspectives on the pharmacotherapy of vertigo.

Recent evidence from experiments in animals suggests a rationale for more effective pharmacologic and physical therapy in patients with vertigo. In particular, one must consider the possible effects of medications on the adaptive processes that naturally mediate recovery from vestibular lesions. The challenge now is for clinicians to devise medication and exercise regimens for patients with vestibular disturbances and critically (and quantitatively) evaluate their effects.

Immunologic and electrophysiological response to cytomegaloviral inner ear infection in the guinea pig.

We investigated the development of labyrinthitis in animals inoculated with guinea pig cytomegalovirus (GPCMV). These animals all became deaf eight days after inoculation into the perilymphatic compartment and showed severe inflammatory changes within the inner ear. Animals that received inactivated virus maintained their hearing throughout the observation period and exhibited normal cochlear morphology. Neither group had serum or perilymph antibody to GPCMV eight days after viral inoculation. We then investigated the effect of systemic immunity to GPCMV on inner ear viral challenge in animals
