BACKGROUND: Natural language processing is a collection of techniques designed to empower computer systems to comprehend and/or produce human language. The purpose of this investigation was to train several large language models (LLMs) to explore the tradeoff between model complexity and performance while classifying narrative feedback on trainees into the Accreditation Council for Graduate Medical Education subcompetencies. We hypothesized that classification accuracy would increase with model complexity.
METHODS: The authors fine-tuned several transformer-based LLMs (Bidirectional Encoder R…

BACKGROUND: Research on child protective services (CPS) is impeded by a lack of high-quality structured data. Crucial information on cases is often documented in case files, but only in narrative form. Researchers have applied automated language processing to extract structured data from these narratives, but this has been limited to classification tasks of fairly low complexity. Large language models (LLMs) may work for more challenging tasks.
OBJECTIVE: We aimed to extract structured data from narrative casework reports by applying LLMs to distinguish between different subtypes of violence:…

INTRODUCTION: Cataracts are a significant cause of blindness. While individuals frequently turn to the Internet for medical advice, distinguishing reliable information can be challenging. Large language models (LLMs) have attracted attention for generating accurate, human-like responses that may be used for medical consultation. However, a comprehensive assessment of LLMs' accuracy within specific medical domains is still lacking.
METHODS: We compiled 46 commonly inquired questions related to cataract care, categorized into six domains. Each question was presented to the LLMs, and three consu…

OBJECTIVES: Large language models (LLMs) are increasingly used in health care communication but can inadvertently perpetuate stigmatizing language toward individuals with alcohol and substance use disorders. Despite growing interest in LLM performance, a focused evaluation of their propensity for SL and strategies to mitigate it remains lacking.
METHODS: We generated 60 clinically relevant questions ["prompts"; 20 each for alcohol use disorder (AUD), alcohol-associated liver disease (ALD), and substance use disorder (SUD)] and tested 14 LLMs. Two physicians independently assessed all response…

BACKGROUND: Natural Language Processing (NLP) and Large Language Models (LLMs) hold largely untapped potential in infectious disease management. This review explores their current use and uncovers areas needing more attention.
METHODS: This analysis followed systematic review procedures, registered with the Prospective Register of Systematic Reviews. We conducted a search across major databases including PubMed, Embase, Web of Science, and Scopus, up to December 2023, using keywords related to NLP, LLM, and infectious diseases. We also employed the Quality Assessment of Diagnostic Accuracy St…

Clinical documentation burden increased exponentially since the Health Information Technology for Economic and Clinical Health Act in 2009. Large language models (LLMs) offer substantial potential for alleviating this burden by analyzing clinical notes. This study validates GPT-based LLMs for extracting clinical data from spine surgery operative reports, expanding prior work to a new note type and clinical domain. Operative reports from 88 patients undergoing lumbar decompression and/or fusion surgery in 2022 for lumbar spondylolisthesis were retrospectively reviewed. Three independent reviewe…

Large language models (LLMs) are promising for healthcare question answering, yet adapting them with sensitive clinical data can induce unintended memorization and privacy leakage. We present MedDP, an efficient privacy-budgeted adaptation framework that delivers an auditable utility-privacy trade-off under ($\epsilon,\delta$)-differential privacy. Our method, MedDP, applies DP-SGD only to low-rank adapter parameters while freezing the base model, using per-example gradient clipping and calibrated Gaussian noise to enable explicit privacy accounting with minimal compute and memory overhead. To…

CRISPR-Cas systems revolutionize life science. Metagenomes contain millions of unknown Cas proteins. Traditional mining relies on protein sequence alignments. In this work, we employ an evolutionary scale language model (ESM) to learn the information beyond sequences. Trained with CRISPR-Cas data, ESM accurately identifies Cas proteins without alignment. Limited experimental data restricts feature prediction, but integrating with machine learning enables trans-cleavage activity prediction of uncharacterized Cas12a. We discover 7 undocumented Cas12a subtypes with unique CRISPR loci. Structural

BACKGROUND: Clinical practice guidelines provide important evidence-based recommendations to optimize patient care, but their development is labor-intensive and time-consuming. Large language models have shown promise in supporting academic writing and the development of systematic reviews, but their ability to assist with guideline development has not been explored. In this study, we tested the capacity of LLMs to support each stage of guideline development, using the latest SAGES guideline on the surgical management of appendicitis as a comparison.

INTRODUCTION: Crash reports support the development of traffic safety countermeasures, but these reports often suffer from underreporting of crucial crash factors due to miscoded entries during data collection. To rectify these issues, the current practice relies on manual information rectification, which is time consuming and error prone, especially with large data volumes. To address these hurdles, we develop a framework to analyze traffic crash narratives and uncover underreported crash factors by capitalizing on the capabilities of Large Language Models (LLM).
METHOD: The framework integr…

BACKGROUND: Residency leaders increasingly rely on personal statements to assess applicant communication skills and individuality. Personal statements written by large language models (LLMs) have been rated more readable but more generic than applicant-written ones, yet the prevalence of LLM use and its effect on applicant writing remain unknown.
OBJECTIVE: To measure the prevalence of LLM-associated language in personal statements from the 2017 through 2024 application cycles and assess associated changes in applicant writing.

Large language models (LLMs) have emerged in recent years as innovative artificial intelligence systems with early potential in clinical decision-making. This is the first systematic review to evaluate LLMs' oncological decision-making and compare their treatment recommendations to "gold standard" oncological multi-disciplinary team (MDT) decision-making. PubMed, EMBASE and Medline databases were last searched on 20th January in line with PRISMA guidelines. All relevant peer-reviewed publications comparing LLM and MDT treatment recommendations in patients with cancer were included. Studies usi…

Retrieval-augmented generation (RAG) is an emerging technique that enhances large language models (LLMs) by retrieving relevant information from reliable external knowledge (REK). Despite its success in various language processing tasks, the clinical application of RAG in radiology is still novel. To evaluate the utility of RAG in radiological staging tasks, we compared the performance of NotebookLM, a RAG-equipped LLM (RAG-LLM), with its internal model, Gemini 2.0 Flash. Using Japan’s current pancreatic cancer staging guideline as REK, we compared three LLM settings—(1) NotebookLM with REK (R…

Using large language models (LLMs) with persona-based prompt engineering, this study simulates realistic insufficient effort responding (IER) data under controlled conditions, overcoming the limitations of traditional methods in ecological validity and controllability. The core objective is to generate controlled, distinct IER and non-IER datasets, thereby improving further research on detection methods. Our strategy involved systematically manipulating persona attributes, such as behavioral descriptions and psychological attributes, to produce synthetic IER data under controlled conditions. W…
