Nasal leech infestation is a rare etiology of epistaxis. Due to its insidious presentation and inconspicuous site of infestation, it is possible for the primary care setting to miss the diagnosis. We report a case of nasal leech infestation in an eight-year-old male child who was repeatedly treated for upper respiratory infection before finally being referred to the otorhinolaryngology clinic. We emphasize the importance of having a high index of suspicion and thorough history taking, especially of jungle trekking and hill water exposure in unexplained recurrent epistaxis.

Successful microvascular ear replantation is well described, and often involves the use of arterial inflow anastomoses with leeches and systemic anticoagulation to augment outflow. Failed replantation attempts have also been published, with most of the failures attributed to venous drainage problems. A case of successful microvascular replantation of a traumatically amputated ear is presented. The postoperative course was complicated by venous thrombosis requiring the use of medicinal leeches and systemic heparinization for salvage. Background: Infections with Aeromonas spp. are a recognized complication of leech therapy for circulatory complications in replanted digits. Fourteen cases of nasal hirudiniasis, seen during a period of less than a year in Kumaon Hills of Northern India are reported. Unilateral epistaxis was the main symptom. This study aims to explain microscopic replantation in a rare case of a wholly amputated penis after prolonged ischemia.
