Evaluating Weight Status and Sex as Moderators of the Association of Serum Leptin with Bone Mineral Density in Children and Adolescents
.

BACKGROUND/AIMS: Animal studies suggest that leptin may adversely affect bone mineral density (BMD). Clinical studies have yielded conflicting results. We therefore investigated associations between leptin and bone parameters in children. METHODS: 830 healthy children (age = 11.4 ± 3.1 years; 75% female; BMI standard deviation score [BMIz] = 1.5 ± 1.1) had fasting serum leptin measured with ELISA and body composition by dual-energy X-ray absorptiometry. The main effects for leptin and BMIz plus leptin's interactions with sex and BMIz were examined using hierarchical linear regressions for app

Effects of demineralized bone matrix on tendon-bone healing: an in vivo, experimental study on rabbits.

OBJECTIVE: The aim of this study was to investigate the effects of demineralized bone matrix (DBM) on tendon-bone healing. METHODS: The extensor digitorum longus tendon was fixed with pegged suture technique in a tunnel at the proximal tibia in both legs of 12 New Zealand rabbits. Rabbit DBM was applied in the tunnel on the right limbs before fixation (study group), while the fixation was performed without DBM in the left legs (control group). Randomly, four rabbits were sacrificed at the 3rd, four rabbits at the 6th and the remaining four rabbits at the 9th week with an intravenous high dose

Clinical factors as predictors of the risk of falls and subsequent bone fractures due to osteoporosis in postmenopausal women.

In Japan, the "bedridden state" is one of the most serious problems the aged face, and it is becoming a social problem. The main causes of the bedridden state are cerebrovascular disorders and bone fractures following falls. The purpose of this study was to predict risk factors for falls and resultant bone fracture due to osteoporosis. We explored mobility parameters for possible fall prevention. In order to examine the correlation between the risk of falling and resultant bone fracture due to osteoporosis, logistic regression analysis was performed between bone mass (independent variable) and

Tourniquet-induced ischemia and reperfusion in subcutaneous tissue, skeletal muscle, and calcaneal cancellous bone.

This study aimed to evaluated ischemic metabolites in subcutaneous tissue, skeletal muscle, and calcaneal cancellous bone before, during, and after tourniquet application in a simultaneous paired comparison of tourniquet-exposed and non-tourniquet-exposed legs. Ten patients scheduled for hallux valgus or hallux rigidus surgery were included. Microdialysis catheters were placed to simultaneously and continuously sample the metabolites glucose, lactate, pyruvate, and glycerol bilaterally for 12 h in subcutaneous tissue, skeletal muscle, and calcaneal cancellous bone. A tourniquet was applied on

Comparative Outcomes of Cast and Removable Support in Fracture Fifth Metatarsal Bone: Systematic Review and Meta-Analysis.

Fractures of the metatarsals account for 35% of all foot fractures. Conservative management of fractures proximal to the metaphyseal-diaphyseal junction of the fifth metatarsal bone (pseudo-Jones) is protected weightbearing. The methods of protected weightbearing include a short-leg cast and splint (boot cast, Jones bandage, and elastic bandage). However, no consensus has yet been reached regarding which method is most suitable. We conducted a systematic review and meta-analysis to compare the outcomes of a short-leg cast and splint for pseudo-Jones metatarsal fractures. We searched the PubMed

Diffuse osteosclerosis in a patient with prostate cancer.

A 61-year-old man was referred to our outpatient clinic because of severe bilateral upper leg pain for 1 year. On admission, the patient had anemia and a high serum alkaline phosphatase level. Lumbar and femoral neck T-scores were +10.5 and +9.6, respectively. His radius 33 % T-score was -2.8. Plain radiographs of the patient's pelvis, spine, and long bones revealed osteosclerosis. The patient had previously undergone a prostate biopsy, which showed prostate adenocarcinoma (Gleason score 3 + 4). The patient's total and free prostate-specific antigen were very high. According to previous record

The rate of bone mineralization in birds is directly related to alkaline phosphatase activity.

Recent studies have suggested that a biochemical marker, plasma alkaline phosphatase (ALP), can be used as a general indicator of skeletal development in vertebrate animals. In birds, age-related variation in ALP activity, presumably due to bone formation processes, has been demonstrated, but to date, a direct connection between bone mineralization and enzyme activity has been elusive. In this study, we show that the activity of a bone isoform of ALP (bone ALP) is closely related to the overall rate of skeletal mineralization in nestlings of a small passerine bird, the great tit (Parus major L

Sex assessment on the basis of long bone circumference.

Discriminant functions have long been used to classify individuals into groups according to the dimensions of their bones. Although lengths, widths, and diameters have been extensively used, the circumferences have not been adequately validated. In this work, the importance that the circumferences of long bones can have in assigning the sex of ancient human remains is demonstrated. The functions produced by using just one circumference achieved accuracies higher than 80%, and circumference at the radial tuberosity of the radius is able to classify 92.8% of skeletons from the Late Roman site of

Limb salvage in recurrent synovial sarcoma of the right ankle and lower leg.

Synovial sarcoma of the extremities represents 7% of all soft-tissue sarcomas. This article presents the case of a patient who was treated for a synovial sarcoma of the lateral aspect of the distal lower leg extending to the ankle and involving the fibular bone. The patient underwent a wide excision of the tumor, including the fibular bone, followed by radiation and chemotherapy, rather than undergo an amputation of the right leg. Consideration is also given to the traumatic etiology of the tumor.

Radiographic Predictors of Leg Shortening After Curved Intertrochanteric Varus Osteotomy in Osteonecrosis of the Femoral Head.

PURPOSE: Curved intertrochanteric varus osteotomy (CVO) is a joint-preserving option for young patients with osteonecrosis of the femoral head (ONFH), but postoperative leg length discrepancy (LLD) remains a concern. This study investigated factors associated with leg shortening in both the early postoperative phase (P1) and the healing phase until bone union (P2). METHODS: This retrospective study included 48 patients (51 hips) with non-traumatic ONFH who underwent CVO. Radiographic evaluations were performed preoperatively, immediately postoperatively, and at bone union. Pearson's correlati

Anomalous intraosseous venous drainage: Bone perforators?

Objectives Anomalous intraosseous venous drainage is a rare and almost unknown entity; only 14 cases have been reported in the literature and 4 mentioned in textbooks. We report the characteristics of 35 further cases observed in 32 patients. Method After the presentation of two cases at the congress of the French Society of Phlebology in Paris (2013), 12 colleagues joined to present a large series of so-called bone perforators observed in their practice, all identified with at least a duplex investigation. Results Thirty-two patients suffering from varicose veins and/or skin changes (C2-C6) a

Proximal Tibial Giant Cell Tumor in a Skeletally Immature Child: A Diagnostic Dilemma and Review of the Literature.

INTRODUCTION: Giant cell tumor (GCT) of bone is a benign, locally aggressive neoplasm that primarily involves the epiphysis of long bones in skeletally mature individuals. It is extremely rare in skeletally immature patients. We report a rare case of proximal tibial GCT in a 7-year-old girl with a pathological fracture and metadiaphyseal extension, highlighting diagnostic considerations and long-term outcomes. CASE REPORT: A 7-year-old girl presented with pain, swelling, and deformity of the left upper leg following trivial trauma. Radiographs and non-contrast computed tomography scan demonst

Roles of Local Soluble Factors in Maintaining the Growth Plate: An Update.

The growth plate is a cartilaginous tissue found at the ends of growing long bones, which contributes to the lengthening of bones during development. This unique structure contains at least three distinctive layers, including resting, proliferative, and hypertrophic chondrocyte zones, maintained by a complex regulatory network. Due to its soft tissue nature, the growth plate is the most susceptible tissue of the growing skeleton to injury in childhood. Although most growth plate damage in fractures can heal, some damage can result in growth arrest or disorder, impairing leg length and resultin

Soft-tissue infection secondary to cellulitis killed St. John of the Cross (1542-1591).

PURPOSE: St. John of the Cross (1542-1591) died aged 49 years after 3 months of excruciating pain following a trivial lesion in his right foot. Erysipelas, a superficial bacterial infection of the skin, and subsequent sepsis were previously suggested as the cause of his death. Here, an alternative diagnosis is proposed. METHODS: An accurate perusal of his biography allowed the symptomatology, the clinical evolution, the depth of the infection and the associated systemic manifestations displayed by Fray John to be reconstructed. RESULTS: St. John of the Cross developed cellulitis in the foot,

Erdheim-Chester disease: the effect of bisphosphonate treatment--a case report.

Erdheim-Chester disease is a distinctive pathologic and radiographic entity characterized by bilateral symmetric sclerosis of the diametaphyseal regions of long bones and infiltration of foamy lipid-laden histiocytes. It is a rare histiocytic disease of unknown etiology that is characterized pathologically by xanthogranulomatous infiltrates of multiple organs. We present a patient in her early sixties with bilateral mild knee and leg pain. The patient showed a typical bilateral symmetric medullary sclerosis at the diametaphyseal portions of long bones of the lower extremity. The diagnosis was

Short beak: a new autosomal recessive semilethal mutation in Japanese quail.

Short break (SBK) is a new semilethal mutation of Japanese quail (Coturnix japonica). The SBK individuals are characterized externally by short breaks, shanks, and digits. The shank is also thicker than the wild type. The shape of the mutant beak does not show a parrotlike appearance, contrasting with that of other poultry chondrodystrophic mutants reported in the literature. Bones in the fore and hind limbs of the SBK mutant are also shorter than the wild types. The humerus and ulna in the wing are significantly thicker than the wild types. Other bones in the wing and leg show approximately t
