Factors leading to lens implant decentration and exchange.

PURPOSE: To examine the intra- and post-operative factors leading to posterior chamber intraocular lens (IOL) decentration in patients requiring IOL exchange, and to identify avoidable causes of IOL decentration. METHODS: Case records of 17 patients who had undergone posterior chamber IOL exchange were examined for: (i) any complication or alteration to the original intended surgical procedure, (ii) IOL type and position at the completion of initial surgery, (iii) IOL position at the time of re-operation. RESULTS: The decentred lens implants were injected silicone plate-haptic IOLs in 10 pat

[Surgical treatment of contusive dislocation of the lens].

54 of 60 consecutive cases of contusive lens dislocation (incomplete 45 cases and complete 15 cases) were treated surgically by (1) loop delivery of the lens in 39 cases, (2) double needle fixation (Barraquer's method) in 4 cases, (3) intracapsular cryo- or silicon ball extraction in 5 cases, (4) extracapsular cortical aspiration in 3 cases, and (5) VISC lensectomy and vitrectomy in 3 cases. Surgical treatment is indicated in cases of (1) lens dislocation into the anterior chamber or incarcerated in the pupil; (2) secondary glaucoma induced by pupil block; (3) secondary uveitis; (4) visual imp

Peripheral Peters' anomaly: a histopathologic case report.

The clinical and pathologic findings of a case of congenital bilateral, peripheral corneal leukoma associated with iris coloboma and anterior subluxation of the lens are reported. The changes in the region of keratolenticular apposition include partial absence of Bowman's membrane, stromal vascularization and thinning with large-diameter collagen fibrils, and absence of Descemet's membrane and endothelium. The authors postulate that dysembryogenesis of the tertiary vitreous, with iris coloboma, resulted in apposition of the lens to cornea during the second and third month of gestation and that

Complications of endocapsular tension ring implantation in a child with Marfan's syndrome.

A 4-year-old boy with Marfan's syndrome had severe visual impairment after subluxation of the crystalline lens with bisection of the pupil. In the first eye, a capsular tension ring and an intraocular lens (IOL) were uneventfully placed in the capsular bag after phacoemulsification. During implantation of the endocapsular tension ring in the second eye, an inadvertent tear of the anterior capsule occurred, and the posterior chamber IOL was placed in the sulcus. In both eyes, severe lens epithelial proliferation with secondary IOL decentration developed postoperatively. Several surgical revisio

Vitrectorhexis and lens aspiration with posterior chamber intraocular lens implantation in spherophakia.

UNLABELLED: We describe a technique that uses the vitrector to perform successful lens aspiration and posterior chamber intraocular lens (IOL) implantation in children with spherophakia and anterior lens subluxation. After an anterior chamber maintainer is placed, the ocutome is introduced through a limbal incision to perform a circular vitrectorhexis to avoid excessive manipulation of the unstable lens followed by gentle cortex aspiration. A foldable IOL is injected into the sulcus (3-piece IOL) or bag (1-piece IOL) if the capsule is sufficiently stable. Through a pars plana incision, the ocu…

Corneal biomechanics and their association with severity of lens dislocation in Marfan syndrome.

PURPOSE: To investigate corneal biomechanical properties and its associations with the severity of lens dislocation in patients with Marfan syndrome. METHODS: A total of 30 patients with Marfan syndrome and 30 age-, sex- and axial length (AL)-matched controls were recruited. Corneal biomechanical parameters of both groups were measured by CorVis ST and were compared between groups. Potential associations between corneal biomechanical parameters and severity of lens dislocation were also investigated. RESULTS: Lower applanation 1 velocity (A1V) (0.13 ± 0.004 vs. 0.15 ± 0.003, P = 0.016), shor

Two Surgical Approaches for Intraocular Lens Dislocation: Pars Plana Vitrectomy Versus Core Vitrectomy with Lens Exchange.

PURPOSE: To compare the efficacy and safety of core vitrectomy and pars plana vitrectomy for lens exchange in patients with intraocular lens dislocation. METHODS: This is a retrospective study conducted at one eye center in Zurich, Switzerland. We reviewed 124 eyes with dislocated intraocular lens undergoing lens exchange carried out by two surgeons between 03/2016 and 12/2019 (45 months). Intraocular pressure (IOP) and best-corrected visual acuity (BCVA) were analyzed preoperatively and at 5 time points up to 12 months after lens exchange. Data on postoperative complications were collected.

Research on calculation of the IOL tilt and decentration based on surface fitting.

The tilt and decentration of intraocular lens (IOL) result in defocussing, astigmatism, and wavefront aberration after operation. The objective is to give a method to estimate the tilt and decentration of IOL more accurately. Based on AS-OCT images of twelve eyes from eight cases with subluxation lens after operation, we fitted spherical equation to the data obtained from the images of the anterior and posterior surfaces of the IOL. By the established relationship between IOL tilt (decentration) and the scanned angle, at which a piece of AS-OCT image was taken by the instrument, the IOL tilt a

Corneal perforation with extrusion of lens in a burn patient.

The incidence of ophthalmologic injuries has been reported to be between 20 and 25% in all the burn patients. As most of the eye injuries are associated with severe burns, attention is often directed towards the life-threatening events and eye injuries may be overlooked and undertreated. This case report presents a patient who had 40% flame burn and had recently been transferred from another hospital. He was examined by an ophthalmologist at the referring hospital and it was documented that both of his eyes were normal. He was transferred into the Burns Unit at the Prince of Wales Hospital 8 h

Traumatic anterior lens dislocation: a case report.

A 45-year-old man presented to the emergency department complaining of decreased vision and pain in the left eye after blunt trauma to the eye. On evaluation, the vision was limited to detecting hand motions, and the intraocular pressure was 37 mmHg. Secondary acute angle-closure glaucoma, with pupillary block due to anterior dislocation of the lens, was diagnosed. The intraocular pressure remained elevated after medical therapy, and the patient underwent intracapsular cataract extraction and anterior vitrectomy. The possibility of elevated intraocular pressure due to lens dislocation or other

[Spontaneous and bilateral anterior luxation of the lens in a 5-year-old child].

The authors report a case of bilateral spontaneous dislocation of the lens in a five-year-old boy. This rare complication can be encountered in certain congenital abnormalities of the lens and zonule. The dislocation has been revealed by an acute glaucoma. The mains causes like Marfan's syndrome and homocystinuria were not found but due to their most severe prognosis. The patient must be followed carefully.

The aetiology of canine glaucoma.

Glaucoma is a disease complex in which one of several exciting factors is responsible for an elevation of the intraocular pressure beyond its normal physiological limitations. At present two types of primary glaucoma may be described for the dog, while the recognition of secondary glaucoma as a complication to lens luxation, uveitis, trauma, infection and neoplasia has long been established. Identification of the causal factor within this complex plays a necessary part in the institution of successful treatment.
