Stress, development and mental health study, the follow-up study of Finnish TAM cohort from adolescence to midlife: cohort profile.

PURPOSE: This cohort profile describes the Stress, development and mental health study (TAM), which is a cohort study investigating risk and protective factors as well as longitudinal associations regarding mental health and well-being from adolescence to midlife. This interdisciplinary cohort study operates, for example, in the fields of public health, social medicine, psychiatry and the life course perspective. PARTICIPANTS: In 1981 (n=2242, 98.0% of the target population), 1982 (n=2191, 95.6%) and 1983 (n=2194, 96.7%) during school classes, surveys were conducted to all Finnish-speaking pu…

Unfolding States of Mind: A Dissociative-Psychedelic Model of Ketamine-Assisted Psychotherapy in Palliative Care.

Background/Objectives: Patients in palliative care often experience multifaceted forms of suffering that extend beyond physical symptoms, including existential distress, loss of meaning, and emotional pain. Ketamine-assisted psychotherapy (KAP) has emerged as a promising intervention for alleviating such complex forms of suffering, yet models specifically tailored to palliative populations remain scarce. This narrative review synthesizes current evidence on ketamine's neurobiological, psychological, and experiential effects relevant to end-of-life care, and presents a novel, time-limited KAP m

An intensive model of care for hepatitis C virus screening and treatment with direct-acting antivirals in people who inject drugs in Nairobi, Kenya: a model-based cost-effectiveness analysis.

BACKGROUND AND AIMS: Hepatitis C virus (HCV) treatment is essential for eliminating HCV in people who inject drugs (PWID), but has limited coverage in resource-limited settings. We measured the cost-effectiveness of a pilot HCV screening and treatment intervention using directly observed therapy among PWID attending harm reduction services in Nairobi, Kenya. DESIGN: We utilized an existing model of HIV and HCV transmission among current and former PWID in Nairobi to estimate the cost-effectiveness of screening and treatment for HCV, including prevention benefits versus no screening and treatm

Stumblers and Tumblers: Two Pathways to "Unintentional" Fall-Related Traumatic Brain Injury.

Traumatic brain injury (TBI), including concussion, is the commonest neurological condition in high-income countries and is the second commonest condition next to migraines. Although most of these injuries are unintentional, substance abuse and age-related physiological factors have been implicated as causal factors of fall-related TBIs. Our study used qualitative methods and a life course perspective to examine whether life events and psychosocial antecedents, such as early adverse childhood experiences, play a role in the occurrence of non-intentional fall-related TBI. In-depth interviews we…

[Impact of Treatments to Improve Cognitive Function and Quality of Life on Cancer Patients with Carcinoma of the Testes].

BACKGROUND: Testicular cancer is one of the most common and most treatable cancers in men aged 15-49 years. The high survival rates mean that it is essential to maintain quality of life and minimize adverse effects associated with treatment. Both malignant tumors and the modalities used to treatment them can have adverse effects from both a psychosocial and a neurocognitive function perspective. Recently, attention has focused on the negative impact of the disease and its treatment on the brain and on cognitive function, which can result in a form of neurocognitive dysfunction known as "chemo

Psychometric Properties of the Instrument for Scoring Clinical Outcomes of Research for Epidermolysis Bullosa patient score (iscorEB-p): a patient-reported outcome measure.

BACKGROUND: In contrast to clinical diagnosis via external examination, patient-related outcome measures (PROMs) allow access to patients' internal perceptions. In the case of epidermolysis bullosa (EB) - a rare disease characterized by a wide variety of symptoms and individual disease courses - it is important to integrate the patient's perspective into diagnostic processes. The Instrument for Scoring Clinical Outcomes of Research for EB (iscorEB) is an EB-specific measurement tool, combining a clinician score (iscorEB-c) and a patient questionnaire (iscorEB-p). OBJECTIVES: The aim of this s

Redesigning cardiovascular medicine around sex differences.

Cardiovascular disease remains the leading cause of death in women and a major contributor to disability, yet the evidence guiding diagnosis and treatment is still largely derived from studies in which men and male-pattern disease phenotypes have predominated. Women exhibit distinct cardiovascular physiology that contributes to sex differences in cardiovascular health and disease across the life course. However, these biological differences rarely inform clinical decision-making and remain incompletely understood, owing to a research ecosystem that inadequately addresses sex-specific biology.

When hypereosinophilia occurs during dupilumab therapy: evidence-based strategies for clinical management in respiratory indications.

INTRODUCTION: Dupilumab is a monoclonal antibody which selectively targets T2 inflammation by binding the IL-4 and 13 receptor and blocking its contribution to the immune activation. Clinical trials and real-life studies on severe asthma and nasal polyps patients have reported blood eosinophils fluctuation over the treatment course. AREAS COVERED: In the present narrative review the authors aim to provide a practical perspective on the recent evidence related to hypereosinophilia occurring during dupilumab therapy in respiratory indications, its underlying mechanisms, frequency, and clinical

Pregnancy and the liver.

Some of the physiological changes that occur in pregnancy manifest in the liver. These alterations might exacerbate or improve some pre-existent liver diseases, while many conditions remain unaffected. Some hepatic manifestations during pregnancy are secondary to disorders unique to pregnancy. Due to improved management of chronic conditions and assisted conception methods, pregnancies in people with cirrhosis or after liver transplantation are increasingly common. With pregnancy also becoming more common in older people and with the rising prevalence of comorbidities, such as obesity, diabete

Alcohol Use and Moffitt's Maturity Gap Thesis for Adolescent Offending: An Evolutionary Perspective and Analysis.

While a wealth of research has focused on testing several arguments from Moffitt's developmental taxonomy of antisocial behavior-mainly the presence of life-course-persistent vs. adolescence-limited offending and predictors of each trajectory-much less attention has been devoted to examining how evolutionarily adaptive lifestyle factors common during adolescence may condition the relationship between the maturity gap and delinquent offending. One factor that may play a role during this period of development is alcohol use, as many adolescents begin to experiment with consuming alcohol in varyi

Practical recommendations on treatment of multiple sclerosis with Cladribine: an Israeli Experts Group‏ Viewpoint.

Cladribine tablets (Mavenclad®) were approved by the European Union in 2017 as high-efficacy therapy for highly active relapsing-remitting multiple sclerosis. In Israel, Mavenclad® was approved in 2018. Real-life experience has confirmed the efficacy of cladribine tablets over at least 4 years from the initial course. During the last years, several questions were raised concerning the management of people with MS who show disease activity during years 3 and 4 post-cladribine initiation and what treatment decisions are needed beyond year 4. A few expert boards have tried to provide insight base

Incarceration and adversity histories: Modeling life course pathways affecting behavioral health.

A consequence of a growing incarceration rate is that an increasing number of children face having an incarcerated household member, a known contributor to diverse lifelong behavioral health risks such as substance use and mental health impairment. Few studies have explored how household incarceration uniquely contributes to these subsequent behavioral health concerns, nor mediational contributors to these associations, within a theoretical framework. Using state Behavioral Risk Factor Surveillance System survey data (n = 14,001), this study tests pathways of household incarceration and Advers

Remission, recovery, relapse, and recurrence in eating disorders: conceptualization and illustration of a validation strategy.

In a consensus-building process a group of experts from 19 European countries (COST Action B6) adapted the terms partial and full remission, relapse, recovery, and recurrence according to principles described by Frank et al. for depression. The empirical validity of the operationalizations was illustrated by longitudinal data on the post treatment course of 233 anorectic and 422 bulimic patients (diagnosed according to DSM-IIIR) from the German Project TR-EAT. These data were collected 2.5 years after admission using the Longitudinal Interval Follow-up Evaluation (LIFE) and statistically explo

Hemicorporectomy: a case study from a physical therapy perspective.

Hemicorporectomy, literally translated, means half (hemi), body (corpus), removal (ectomy). The procedure is indicated as a last resort measure for patients with life-threatening conditions such as advanced pelvic tumors, pelvic osteomyelitis, crushing pelvic trauma, or intractable decubiti in the pelvic region. We report a case in which this mutilating procedure was performed on a 49-year-old man with extensive perineal and sacral sores. Squamous cell carcinoma was detected and isolated to the pelvic region. His medical history was significant for paraplegia secondary to a gunshot wound in 19

[Depression and aging].

Old age is a qualitative state during which depression frequently occurs. This illness presents particular features which should be considered in the course of both normal ageing and senility. Viewed as an existential crisis, ageing may be considered as a depression promoting factor. Rich in depression promoting factors, this period of life is a sequence of crises which result in reorganizations heavily dependent on the adaptive capabilities of the aged person. During ageing, depression exhibits particular features that one should be able to recognize and distinguish from the consequences of d

Identifying patient-reported outcomes in rheumatoid arthritis: the impact of foot symptoms on self-perceived quality of life.

BACKGROUND: The importance of patient-reported outcome measures in healthcare is increasingly recognized but these need to be patient generated. Given that foot symptoms are very common in rheumatoid arthritis (RA), we chose a patient-centred model with which to investigate the patients' perspective on how their foot symptoms affected them as individuals and impacted on their self-perceived quality of life, rather than using the traditional approaches of clinical examination (e.g. prevalence of deformities) or radiological assessments. METHODS: A 33-item self-administered postal questionnaire
