Early phase trials in soft-tissue sarcomas: clinical benefit of inclusion in early lines of treatment, molecular screening, and histology-driven trials.

BACKGROUND: The prognosis of patients with advanced soft-tissue sarcomas (STS) remains dismal, and systemic therapeutic options are limited. Early phase trials are becoming increasingly safe and effective. This study aimed to identify the prognostic factors for progression-free survival (PFS). PATIENTS AND METHODS: This retrospective analysis included all STS patients participating in early phase trials at Gustave Roussy and Léon Bérard between 1 January 2012 and 31 December 2020. RESULTS: Overall, 199 patients accounted for 214 inclusions in advanced STS. The most frequent histotypes were w

Myxoid tumours of soft tissue.

Myxoid tumours of soft tissue encompass a heterogeneous group of lesions characterized by a marked abundance of extracellular mucoid (myxoid) matrix. This group of tumours demonstrate significant variability in their biological behaviour thus including tumours which are entirely harmless, tumours with a tendency to recur locally but not metastasize, and malignant tumours. There appears to be a considerable degree of overlap clinically and morphologically between the various tumour types in this group, generating potential diagnostic problems for the clinician and pathologist alike. While dilig

Radiological and multidisciplinary management of extraskeletal myxoid chondrosarcoma: A case report.

Extraskeletal myxoid chondrosarcoma (EMC), a very rare subtype of soft tissue sarcoma (STS), distinguished by its histopathological profile, has unique clinical prognosis of potential for late local recurrences and distant metastases due to its molecular alterations. The infrequent occurrence of EMC, coupled with its radiological characteristics that often overlap with those of other myxoid-rich neoplasms, may impose considerable diagnostic difficulties for clinicians. A 43-year-old male who presented with a progressively enlarging mass in the distal right thigh and knee region since six-month

Combined morphologic and karyotypic study of 59 atypical lipomatous tumors. Evaluation of their relationship and differential diagnosis with other adipose tissue tumors (a report of the CHAMP Study Group).

Fifty-nine cases of atypical lipomatous tumors (ALT) of soft tissue (atypical lipomas, well-differentiated liposarcomas) were studied morphologically and cytogenetically as part of an international collaborative study. Forty-nine cases were deeply seated (including retroperitoneum), and 10 were superficial. Clonal chromosomal abnormalities were found in 55 cases (93%). Supernumerary ring or giant marker chromosomes (RGCs), the sole consistent alteration, were found in 37 ALTs (63%). They were more common in tumors that were large (p < 0.001), deeply seated (p < 0.005), that contained lipoblast

Robotic resection of a mediastinal liposarcoma.

A 76-year-old man presented with new onset atrial fibrillation. During his workup, he underwent a computed tomography angiogram of his coronary arteries that revealed multivessel disease and an incidental heterogeneous fat and complex fluid density in the posterior aortopulmonary window space. The mass was further characterized with the use of magnetic resonance imaging and a dedicated computed tomography scan of the chest showing a complex fat-containing tumour most consistent with either an atypical lipomatous tumour or a well-differentiated liposarcoma. Given this differential, the patient

Perinephric Myxoid Pseudotumor of Fat: A Series of 29 Cases Demonstrating Inconsistent Associations With Underlying Kidney Disease.

Perinephric myxoid pseudotumor of fat (PMPTF) is a recently characterized lesion typically associated with non-neoplastic renal disease. Its pathogenesis is thought to result from chronic renal "irritation," either due to mass effect from renal carcinoma or inflammation related to benign renal conditions. Prompted by several cases arising in the absence of underlying renal pathology, we conducted a multi-institutional study of 29 mass-forming cases with detailed clinical, histologic, and molecular characterization. We also reviewed 79 published cases to place our findings in a broader context.

Small Round Cell Tumors of Soft Tissue and Skeleton in Children and Youth: A Narrative and TruSight-Based Illustrative Review of Diagnostic Cases.

Small round cell tumors (SRCT) affecting soft tissue and bone are a distinct category of malignancies. These lesions frequently exhibit similar clinical and radiologic features, may harbor overlapping histologic and immunophenotypic features, and generally have distinct prognostic outcomes. Therefore, in some instances, the diagnosis and accurate subclassification require molecular confirmation. We aimed to provide a comprehensive overview of the morphologic, immunohistochemical, and molecular features of SRCTs of soft tissue and bone in pediatric and young adult patients, with an emphasis on

Fat-Containing Soft Tissue Tumors in Children, Adolescents, and Young Adults: Which Require Biopsy?

PURPOSE: To confirm the overall benignity of fat-containing soft tissue tumors (STT) on a pediatric cohort and to define the clinical and imaging features that warrant a biopsy. METHODS: A retrospective monocentric study was conducted on patients aged less than 25 years consecutively referred for fat-containing STT to our Comprehensive Cancer Center between 1998 and 2022. Tumor imaging characteristics at diagnosis (US, CT, or MRI) were correlated with pathology. RESULTS: The database extraction identified 63 fat-containing tumors with clinical, histologic, and imaging data available for revi

[Retroperitoneal giant angiomyolipoma diagnosed post-partum with lymph node involvement].

HISTORY AND PRESENTING COMPLAINT: A 30-year-old primipara after a normal pregnancy had delivered a 3340 g child. After an uneventful post-partum period she had noticed her abdomen failing to reduce in size. INVESTIGATIONS, DIAGNOSIS AND TREATMENT: The abdominal sonography discovered a large retroperitoneal tumor. CT and MRI showed a giant tumor which originated from the right kidney. Suspecting the diagnosis of renal liposarcoma the kidney and tumor were excised with removal of enlarged precaval and preaortal lymph nodes. Gross inspection revealed a ca. 3,2 kg myxoid tumor, measuring 27 x 19

Lipoblastoma-Like Tumor: A Case Report From Costa Rica.

Lipoblastoma-like tumors (LLTs) are rare adipocytic neoplasms, most commonly found in the vulvar region. Their histopathological and molecular features often overlap with other adipocytic tumors, making diagnosis challenging. We report the case of a 41-year-old woman who presented with a painless mass in the right labia majora. An initial excisional biopsy confirmed a lipoblastoma-like tumor, characterized by spindle cells in a myxoid stroma, focal positivity for CD34 and S100, and absence of DDIT3 rearrangement. Imaging revealed residual nodular lesions, prompting a wide local excision, which

Perinephric myxoid pseudotumor of fat: a distinctive pseudoneoplasm most often associated with non-neoplastic renal disease.

In 2009, Tanas et al reported unusual changes in the perinephric fat, mimicking well-differentiated liposarcoma. We report 11 perinephric masses showing similar changes but chiefly arising in patients with non-neoplastic renal disease. Tissue from 11 perinephric masses was retrieved, and immunohistochemistry for IgG/IgG4 and fluorescence in situ hybridization (FISH) for MDM2 amplification was performed. Clinical information was obtained. Cases occurred in 10 males and 1 female (43-84 years of age; median, 63.5 years). Ten patients presented with perinephric masses (size range, 2-28 cm), and on

Airway obstruction caused by functional failure of a tracheal stent.

Central airway obstruction can be treated with stenting of the airway. A 30-year-old man with known metastatic myxoid liposarcoma presented with central airway obstruction due to tracheal compression from mediastinal metastases. A self-expanding metallic stent was placed under general anaesthesia. During emergence, he developed complete airway obstruction with ensuing hypoxaemia. Emergency intubation was performed and the patient was manually ventilated with high airway pressures to maintain oxygenation. The rigid bronchoscope was reintroduced, which revealed complete collapse of the stent and

Dedifferentiated liposarcoma composed predominantly of rhabdoid/epithelioid cells: a frequently misdiagnosed highly aggressive variant.

Dedifferentiated liposarcoma is one of the most common sarcoma types in adults with a predilection for the retroperitoneum. We have recently encountered 6 cases of DDL composed predominantly of rounded, rhabdoid or epithelioid cells mimicking rhabdoid melanoma, epithelioid rhabdomyosarcoma or undifferentiated carcinoma. Patients were 5 males and one female aged 64 to 81 years (median, 68). Tumors originated in the retroperitoneum (n=5; 3 in the psoas muscle) and deep soft tissue of the thigh (n=1). All 3 patients with follow-up died of metastatic disease within 4 to 8 months. Preoperative biop

Myxoid Inflammatory Myofibroblastic Sarcoma: Clinicopathologic Analysis of 25 Cases of a Distinctive Sarcoma With Deceptively Bland Morphology and Aggressive Clinical Behavior.

The number of recognized sarcoma types harboring targetable molecular alterations continues to increase. Here we present 25 examples of a distinctive myofibroblastic tumor, provisionally termed "myxoid inflammatory myofibroblastic sarcoma," which might be related to inflammatory myofibroblastic tumor, and which occurred in 13 males (52%) and 12 females at a median age of 37 years (range: 7 to 79 years). Primary tumor sites were peritoneum (18 patients; 72%), paratesticular (2; 8%), chest wall (1), upper extremity (1), esophagus (1), retroperitoneum (1), and uterus (1). Nine peritoneal tumors (

Distal extremities soft tissue sarcomas: Are they so different from other limb localizations?

BACKGROUND AND OBJECTIVES: Soft tissue sarcoma localization in distal extremities (DESTS) of the limbs (hand/fingers, and foot/toes) is unusual. The literature is scarce about their behavior and this study was designed to assess their epidemiological characteristics, outcomes, and prognosis compared to other limb localizations (OLSTS). METHODS: From 1980 to 2010, adult DESTS and OLSTS in 22 centers were included. Demographics, tumor type, treatment modalities, and latest follow-up status were collected. Primary endpoints were overall survival and local/metastatic recurrence incidences. RESUL

PLAG1-Driven Lipoblastomatosis: Diagnostic Insights from a Case Report and Literature Review.

Lipoblastomatosis is a rare benign adipocytic tumour of infancy and early childhood, characterized by rapid growth. Diagnosis is challenging, owing to tumour heterogeneity and overlap with other pediatric soft-tissue neoplasms. PLAG1 rearrangement is the defining molecular hallmark, with an expanding list of fusion partners. We report a case of a 1-year-old child with a firm, nonmobile mass in the right thigh, which on CECT was deep-seated and heterogeneously enhanced. Gross evaluation of the excision specimen demonstrated an ill-circumscribed soft tissue mass. Histopathologically, the tumour

Detection of specific genetic abnormalities by fluorescence in situ hybridization in soft tissue tumors.

For the detection of chromosome translocations/chimeric genes and specific genetic abnormalities in soft tissue tumors, we conducted fluorescence in situ hybridization (FISH) analysis on 280 cases of soft tissue and other tumors using formalin-fixed paraffin-embedded tissue sections. The detection rate of the FISH split-signal was 84% (129/154 cases) for the translocation-associated soft tissue tumors, such as Ewing's sarcoma/primitive neuroectodermal tumor, synovial sarcoma, alveolar rhabdomyosarcoma, myxoid liposarcoma, clear cell sarcoma and so forth. Positive split-signals from EWSR1, SS18

Characteristic magnetic resonance imaging features of atypical spindle cell/pleomorphic lipomatous tumor.

Atypical spindle cell/pleomorphic lipomatous tumor (ASPLT) is a newly classified benign lipomatous neoplasm characterized by diverse histological features, leading to broad differential diagnoses. This study evaluated the magnetic resonance imaging (MRI) features of ASPLT to identify markers for distinguishing them from other soft tissue tumors. Nineteen patients with histologically confirmed ASPLT across multiple institutions were retrospectively evaluated. MRI features, including tumor margins, capsules, septa, fat content, hyperintense areas on T2-weighted imaging (T2WI), edema, and contras
