Sialolithiasis is defined as calcified stone(s) in the salivary duct or glands. Submandibular gland sialolithiasis is the most common (80 to 90%), followed by parotid gland sialolithiasis (5 to 15%). The typical clinical presentation is salivary gland swelling after eating. As the swelling persists, symptoms owing to local inflammation, such as pain and trismus, emerge. In severe cases, cellulitis and even abscess formation occur and subsequently lead to salivary gland atrophy or fistula formation if the sialolithiasis remains untreated. The most common treatment is complete excision of the af

Bouveret syndrome is a rare cause of gastric outlet obstruction due to gallstone migration through a bilioenteric fistula. We present a 76-year-old woman with multiple comorbidities who developed Bouveret syndrome and was treated with endoscopic holmium laser lithotripsy. Although the obstructing stone was successfully fragmented and removed, the patient later developed a jejunal perforation. She was ultimately transitioned to hospice care. To our knowledge, jejunal perforation after endoscopic laser lithotripsy for Bouveret syndrome has not been previously reported. This case highlights a rar

Introduction and Objective: The novel thulium fiber laser (TFL) has been shown to break stones more rapidly than the holmium:YAG laser (HL). However, some evidence suggests that the TFL generates more heat. The purpose of this study is to compare ureteral temperatures generated by these lasers during ureteroscopic laser lithotripsy in a benchtop model. Methods: A 1-cm BegoStone was manually impacted in the proximal ureter of a three-dimensional printed kidney-ureter model and submerged in 35.5°C saline. Lithotripsy was performed using a 7.6F flexible ureteroscope and a 200 μm laser fiber witho

Bouveret syndrome, a rare cause of intestinal obstruction, occurs by passage of a gallstone through a cholecystoduodenal fistula into the intestinal lumen. Presenting symptoms are nausea, vomiting, and abdominal pain. In some cases, chronic symptoms result in weight loss. Typically, the syndrome is diagnosed via x-ray, ultrasound, or computed tomography. Treatment options are endoscopic or surgical. Endoscopic approaches include mechanical lithotripsy, electrohydraulic lithotripsy, stone extraction, laser lithotripsy, extracorporeal shockwave lithotripsy, and/or duodenal stenting. When stone f

PURPOSE OF REVIEW: Percutaneous nephrolithotomy (PCNL) remains the gold standard for the treatment of large and complex renal calculi. Traditionally, stone fragmentation during PCNL has relied on mechanical lithotripsy combined with manual or passive fragment extraction. This review examines whether recent advances in stone fragmentation and evacuation technologies represent a true paradigm shift in percutaneous stone surgery or rather a progressive refinement of established principles.
RECENT FINDINGS: Recent literature highlights a progressive transition from conventional mechanical lithotr

BACKGROUND AND AIMS: Per-oral pancreatoscopy (POP) permits direct evaluation of the pancreatic duct for the visualization and sampling of neoplastic lesions and treatment of pancreatic duct stones by using intraductal lithotripsy techniques. Pancreatic laser endotherapy of mucosa has been described in animal models for tumor ablation, but human experience is lacking. We describe 3 unique and challenging clinical situations that benefited from pancreatic laser dissection and ablation.
CASE DESCRIPTION: Case 1 was a 75-year-old woman with presumed divisum-associated chronic pancreatitis who had

BACKGROUND: Laser lithotripsy is central to the endoscopic management of renal and ureteral stones. Thulium:YAG (Tm:YAG) laser has emerged as a promising platform, although clinical evidence on its safety, effectiveness, and real-world performance remains limited. The aim of this article is to evaluate the clinical performance, effectiveness, and safety of Tm:YAG laser lithotripsy in patients undergoing URS/ULT or RIRS at a high-volume center.
METHODS: Patients undergoing URS/ULT or RIRS for urolithiasis with the Tm:YAG laser were prospectively enrolled at a single high-volume institution bet

Introduction: Transurethral lithotripsy (TUL) is an appropriate treatment for ureteral stones and is usually used for stones in the middle and lower part of the ureter. Different devices such as the Holmium laser, the stone basket, and the stone cone exist to prevent any fragments from retropulsion during TUL. The present study aims to compare the advantages and disadvantages of the Holmium laser, the stone basket, and the stone cone. Methods: A retrospective study was conducted from September 2016 to January 2018 comparing various TUL methods in 88 subjects with proximal ureteral calculi. The

Renal cysts and renal stones are common diseases in urology, but it is less common for both to coexist in a patient's kidney. In some unsophisticated urology clinics, separate elective treatments for renal cysts and renal stones are usually chosen, reducing the surgery risk but increasing the financial burden for patients. In this paper, we report a case of percutaneous nephrolithotomy for managing renal stones and endoscopy of renal cysts. The patient was a 61-year-old woman with back pain for more than 10 days. She was preoperatively diagnosed with left kidney stones and bilateral renal cyst

Introduction: Flexible ureteroscopy (FURS) using the flexible and navigable suction ureteral access sheath (FANS) is a novel technique for treatment of kidney stones. We aimed to compare outcomes of FURS with FANS in the sitting vs standing position. Patients and Methods: We analyzed adult patients from 21 centers who underwent FURS with FANS, divided according to whether the surgeon operated in a sitting or standing position. Baseline demographics, operative parameters, and 30-day outcomes were compared. Multivariable logistic regression was used to identify potential predictive factors for z

BACKGROUND: Laser lithotripsy using a thulium fiber laser (TFL) has become an effective treatment option for small renal stones with low complication rates. TFL has a higher absorption coefficient, smaller fibers, and better pulse rate capability.
METHODS: We conducted a systematic review and meta-analysis to evaluate the published evidence regarding TFL's lithotripsy performance in retrograde intrarenal surgery (RIRS), for which we primarily assessed the outcomes of stone-free rate, operation time, and complications. We searched different databases from inception to April 2023. We assessed t
