Hepatic angiosarcoma presenting as an acute intraabdominal hemorrhage treated with transarterial chemoembolization.

Primary malignant neoplasms of the liver are some of the most uncommon malignancies in many parts of the world. They include hepatocellular carcinoma and stromal tumors such as hepatic angiosarcoma. It is a lethal tumor with life expectancy of less than six months. Once discovered, it is often too late for surgical intervention. Like other vascular tumors of the liver and spleen, intraperitoneal hemorrhage is a well-documented finding of angiosarcoma which can be lethal if not diagnosed and treated immediately. As in our case, intraperitoneal hemorrhage from primary tumor rupture was the only

Fine-needle aspiration diagnosis of metastatic urothelial carcinoma: a review.

Urothelial carcinoma (UC) is the most common malignant neoplasm of the urinary tract. Metastases of UC are most common in the regional lymph nodes, lungs, liver, bone, and adrenal glands. Fine-needle aspiration cytology diagnosis of such metastases can be difficult, particularly in the setting of incomplete clinical history or when multiple primary neoplasms may be present. This review focuses on the cytologic features helpful in differentiating UC from its potential mimics, as well as ancillary studies that may be helpful in the distinction.

Challenges in Diagnosis: Primary Hepatic Lymphoma Presenting as a Space-Occupying Liver Lesion.

Non-Hodgkin lymphoma is a heterogeneous group of lymphoid neoplasms, the incidence of which has increased over the past 3 decades. Primary non-Hodgkin lymphoma of the liver is a very rare malignancy. We outline a case describing primary hepatic lymphoma in an 85-year-old woman with a history of breast cancer who presented with generalized weakness and nonspecific symptoms. The patient had normal liver function, serum alpha fetoprotein level, and hemoglobin. A computed tomography scan of the chest, abdomen, and pelvis showed numerous low-attenuation lesions scattered throughout the liver. This

Hepatic Sinusoidal Disorders.

Hepatic sinusoids are highly specialized microcirculatory conduits within the hepatic lobules that facilitate liver functions. The sinusoids can be affected by various disorders, including sinusoidal dilatation, sinusoidal obstruction syndrome (SOS), sinusoidal cellular infiltration, perisinusoidal infiltration, and endothelial neoplasms, such as hemangioendothelioma and angiosarcoma. While these disorders, particularly SOS and neoplasms, can be life threatening, their clinical manifestation is often nonspecific. Patients may present with right upper quadrant pain, jaundice, hepatomegaly, asci

[Portal vein thrombosis as the main symptom of unclassified JAK2-positive myeloproliferative neoplasm--case report].

We report a case of a patient with a diagnosis of myeloproliferative neoplasm, unclassifiable, manifested only portal vein thrombosis and followed by cirrhosis of the liver. 37-year-old patient, previously healthy, without congenital thrombophilia, without prior thrombosis, with normal peripheral blood morphology were signs of extensive portal vein system, with massive collateral circulation. Patient did not meet the criteria for diagnosis of any of the classic myeloproliferative neoplasms. Bone marrow examination revealed hyperplasia and presence of single polymorphic megakaryocytes. Positive

[Biliary obstruction due to adenocarcinoma of the ampulla of Vater ten years after orthotopic liver transplantation].

The main causes of death in patients with liver transplants are derived from the graft itself. Given the increased survival in this group of patients, the rate of de novo neoplasms in other locations is increasing. We present the case of a 60-year-old man who underwent orthotopic liver transplantation 10 years earlier for terminal cirrhosis caused by hepatitis B. Imaging studies showed dilation of the intra- and extra-hepatic bile ducts with no evidence of lithiasis, anastomotic stenosis, or other biliary complications that are common in patients with liver transplants. The definitive diagnosi

Waldenstrom Macroglobulinemia Leading to Cholestatic Liver Injury and Rapid Progression to Liver Failure.

Waldenstrom macroglobulinemia (WM) is an indolent B-cell neoplasm with generally good prognosis, and while hepatic involvement in WM has been documented in the literature, it is rarely clinically significant. We present a case of a 77-year-old man with WM who presented with a cholestatic liver injury and rapid progression to decompensated liver disease. The patient ultimately transitioned to comfort care and died from liver failure. This case not only highlights the rarity of this poor outcome from WM, but also underscores the importance of early recognition of infiltrative malignancy as a pot

Studies evaluating the utility of N-methyl-N-nitrosourea as a positive control in carcinogenicity studies in the p53+/- mouse.

Studies conducted under the auspices of International Life Sciences Institute (ILSI) have suggested that an alternative mouse carcinogenicity study may be substituted for the traditional 2-year mouse bioassay typically conducted to support the development of drug candidates. The purpose of this study was to characterize the carcinogenic potential of N-methyl-N-nitrosourea (MNU), a DNA alkylating agent, in p53+/- knockout mice to determine its suitability as a positive control agent in an alternative carcinogenicity model. p53+/- knockout mice were administered a single oral dose of 90 mg/kg an

Liver re-transplantation for donor-derived neuroendocrine tumor: A case report.

BACKGROUND: Donor-origin cancer is a well-recognized but rare complication after liver transplantation (LT). The rise in the use of extended criteria donors due to the current shortage of organs increases the risk. Data on donor-origin neuroendocrine neoplasms (NENs) and the most appropriate treatment are scarce. Here, we report a case of a patient who developed a NEN confined to the liver after LT and was treated with liver re-transplantation (re-LT). CASE SUMMARY: A 49-year-old man with no other medical co-morbidities underwent LT in 2013 for alcoholic liver cirrhosis. The donor was a 73-ye

Surgical management of liver tumors.

Two percent of pediatric malignancies arise primarily in the liver; roughly 60% of these cancers are hepatoblastoma (HB). Despite the rarity of these cases, international collaborative efforts have led to the consistent histological classification and staging systems, which facilitate ongoing clinical trials. Other primary liver malignancies seen in children include hepatocellular carcinoma (HCC) with or without underlying liver disease, fibrolamellar carcinoma (FLC), undifferentiated embryonal sarcoma of the liver (UESL), and hepatocellular neoplasm not otherwise specified (HCN-NOS). This rev

A case of acute liver failure caused by Budd-Chiari syndrome salvaged by brain-dead donor liver transplantation.

A 24-year-old man was admitted to our hospital with abdominal distension. He was found to have acute liver failure and diagnosed with Budd-Chiari syndrome based on angiography and liver biopsy. Liver transplantation was deemed necessary when angiography showed extensive thrombotic occlusion of the hepatic veins and liver biopsy revealed submassive hepatic necrosis. The patient was found to have the JAK2V617F mutation, indicating a myeloproliferative neoplasm as the background disease. He developed hepatic encephalopathy but remained conscious on on-line hemodiafiltration. Brain-dead donor live

Living Donor Liver Transplantation for Vasoactive Intestinal Peptide-Secreting Tumors with Watery Diarrhea, Hypokalemia, and Achlorhydria Syndrome Resulting from Transformation of a Nonfunctional Pancreatic Neuroendocrine Tumor with Multiple Liver Metastases: A Case Report.

INTRODUCTION: Pancreatic neuroendocrine neoplasms (PNENs) are a rare subtype of neuroendocrine neoplasms (NENs), some of which cause characteristic clinical syndromes due to hormone hypersecretion. Vasoactive intestinal peptide-secreting tumors (VIPomas) produce vasoactive intestinal peptide (VIP) and are associated with watery diarrhea, hypokalemia, and achlorhydria (WDHA) syndrome, often leading to severe treatment-refractory symptoms. Although liver transplantation (LT) is regarded as a promising option for unresectable, liver-limited NENs, studies focusing on VIPoma remain limited, as do r

Lack of promoting effect of clonazepam on the development of N-nitrosodiethylamine-initiated hepatocellular tumors in mice is correlated with its inability to inhibit cell-to-cell communication in mouse hepatocytes.

The tumor-promoting ability of clonazepam (CZP), a widely used benzodiazepine anticonvulsant, was investigated in an in vivo mouse liver tumor promotion assay and an in vitro mouse hepatocyte intercellular communication assay. The development of preneoplastic hepatocellular foci of cellular alteration and hepatocellular neoplasms was studied in male B6C3F1 mice initiated, at 5 weeks of age, with a single i.p. injection of N-nitrosodiethylamine (NDEA; 90 mg/kg body weight) in tricaprylin, followed by administration of either phenobarbital (PB; 0.05%) or CZP (0.068% or 0.136%) in diet beginning

Primary Presacral Neuroendocrine Tumor Presenting as Multiple Liver Metastasis: A Case Report.

BACKGROUND Various neoplasms, including neuroendocrine neoplasms (NENs), can arise from the presacral space. Most presacral lesions are detected due to symptoms arising from tumor growth. However, diagnosing small, asymptomatic presacral tumors is challenging because of their unique location. CASE REPORT A 63-year-old woman with chronic hepatitis C underwent follow-up after achieving a sustained virological response. Abdominal ultrasonography revealed multiple new hyperechoic masses in the liver. Physical and laboratory examinations, including tumor marker analysis, yielded unremarkable result

[Development of obstructive jaundice after hemorrhage into a benign, non-parasitic liver cyst].

Benign, non-parasitic liver cysts are usually asymptomatic and found incidentally by abdominal ultrasound or CT scan. We present the case of a 68-year-old patient who showed obstructive jaundice due to a hemorrhagic liver cyst compressing the choledochal duct. Due to the location of the cyst in the porta hepatis with partial compression of central liver vessels and the meanwhile organized hematoma, operative therapy was preferred in order to prevent secondary complications such as portal vein thrombosis and to exclude a malignant neoplasm.

[Causes of death of alcoholics].

An analysis of the representative material revealed a high rate of mortality among alcoholics, which was much higher than the similar rate among sex- and age-matched control subjects. The leading causes of death were accidents, injuries, poisoning, cardiovascular diseases and neoplasms. Other causes of mortality include suicide, liver cirrhosis and lung tuberculosis.

Sarcoma of follicular dendritic cells with features of sinus lining cells--a new subtype of reticulum cell sarcoma?

Dendritic cell neoplasms of the World Health Organization classification comprise Langerhans cell histiocytosis, Langerhans cell sarcoma, interdigitating dendritic cell sarcoma, follicular dendritic cell sarcoma, and dendritic cell sarcoma, not otherwise specified. Several studies based on immunohistochemical and ultrastructural analysis tried to further clarify the origin of these neoplasms which are thought to derive from mesenchymal or bone marrow precursors. Lymphatic vessel endothelium hyaluronan receptor-1 (LYVE-1) was recently described as a marker for lymphatic endothelium which is exp

Adrenal gland: MR imaging.

The authors investigated the utility of magnetic resonance (MR) imaging in identifying the normal adrenal gland in 100 patients as well as in distinguishing adrenal adenomas (n = 12) from malignant neoplasms (n = 14). The left adrenal gland was seen in 99 of 100 cases and the right in 91 of 100 cases. The adrenals were most easily seen with T1-weighted spin-echo pulse sequences. The ratio of the intensity of the adrenal mass to that of fat at 2,100/90 (repetition time msec/echo time msec) was most helpful in distinguishing adrenal adenomas from malignant neoplasms. In contrast to other studies

Production of intestinal and other tumors by 1, 2-dimethylhydrazine dihydrochloride in mice. I. A light and transmission electron microscopic study of colonic neoplasms.

Single or ten weekly subcutaneous injection(s) of 1, 2-dimethylhydrazine dihydrochloride were administered separately to Swiss mice. The repeated application gave rise mainly to high incidences of tumors in the large intestine. These neoplasms occurred most frequently in the colorectal area and in cecum adjacent to ileum. Light microscopically, these lesions were classified as polypoid adenomas and adenocarcinomas. Most of the adenocarcinomas were highly invasive, although they metastasized rarely. The fine structure of the malignant cells exhibited features typical of columnar absorptive cell
