Basing upon the experience of soviet medicine during the Great Patriotic War, organization of medical security of the Troops and Navy personnel in the post-war period, especially during the war in Afghanistan and local military conflicts and analysis of modern tendencies in military medicine development in the armies of foreign countries the authors formulate the principles of military-medical doctrine of the Armed Forces. Substance use disorder treatment carve outs in Medicaid managed care.

INTRODUCTION: Medicaid managed care organizations (MCO) play a major role in addressing the nation's epidemic of drug overdose and mortality by administering substance use disorder (SUD) treatment benefits for over 50 million Americans. While it is known that some Medicaid MCO plans delegate responsibility for managing SUD treatment benefits to an outside "carve out" entity, the extent and structure of such carve out arrangements are unknown. This is an important gap in knowledge, given that carve outs have been linked to reductions in rates of SUD treatment receipt in several studies. To addr

Survey of dietetic provision for patients with diabetes.

AIMS: To survey dietitians involved in diabetes care regarding the provisions for patients with diabetes. A Healthcare Finance Model for the United States: A Conceptual Workflow for Episode-Based Pricing, Risk Sharing, and Value-Based Accountability Using Bundled Payment as the Core Mechanism.

The United States healthcare system is increasingly moving toward bundled payments as a model for value-based accountability. This approach functions as a healthcare finance model in which the episode of care serves as the central unit of payment design. Rather than reimbursing individual services separately, it organizes costs, risk, incentives, accountability, and quality measurement around the full clinical episode, making the episode the core framework for both financial and care delivery decisions. Under traditional fee-for-service reimbursement, services are paid separately, which can fr

Prevalence and associated factors of metabolic syndrome among Omani adults with mental illnesses: a cross-sectional study.

Individuals with mental illnesses are at increased risk of developing metabolic syndrome (MetS), a cluster of cardiovascular and metabolic abnormalities. Despite growing evidence globally, there is a lack of research in the Gulf region, particularly in Oman. This study is the first to assess the prevalence and associated factors of MetS among Omani adults with mental illnesses. A descriptive cross-sectional design was employed. Three diagnostic criteria were used to assess MetS: the International Diabetes Federation (IDF) criteria, the modified Adult Treatment Panel III (ATP III), and the Join

All patients underwent whole organ pancreas transplantation with bladder drainage and received quadruple immunosuppression with OKT3 induction. The two groups were well-matched; outliers were excluded (four in 1991 and five in 1995), and no attempt was mad

Challenges of liver transplantation programs in low- and middle-income countries: An experience from Sri Lanka.

Liver diseases lead to 1.3 million deaths per year around the world, the majority of which are secondary to cirrhosis. In the management of liver diseases in chronic and acute conditions, liver transplant (LT) plays a major role in improving the survival and quality of life of patients. LT programmes require the technical capabilities in performing the pre-transplant evaluation, transplant surgery and post-transplant care supported by adequate infrastructure and a set of trained teams. Globally, there were 28,000 deceased donor LTs, and 14,000 living donor LTs were performed in 2021. In the So

Building Safety Through Orientation: Current Practices of Client and Patient Safety Onboarding in Finland.

UNLABELLED: Comprehensive orientation is essential for ensuring client and patient safety, strengthening safety culture, and supporting workforce competence in social and health care. Although orientation is recognized as an important patient safety intervention, little is known about how comprehensively client and patient safety topics are incorporated into orientation programs. This study evaluated the extent to which client and patient safety topics were incorporated into orientation programs across Finnish wellbeing services counties. DATA: A cross-sectional electronic survey was conducte

Effectiveness of fracture liaison services in osteoporosis.

BACKGROUND: In response to the gradual decline in the number of prescriptions for anti-osteoporosis medication (AOM) following fragility fractures, fracture liaison services (FLSs) have been set up around the world with the aim of filling this treatment gap. Several studies have already reported the benefits of such organizations, particularly in reducing fracture risk, mortality rates and healthcare costs, and literature on FLSs has increased at a steady pace over time. METHODS: A narrative review was conducted on the latest available findings on the effectiveness of FLSs. Various approaches

Early Impact of a Dedicated Trauma Surgery Unit on Outcomes in a Malaysian Tertiary Center.

INTRODUCTION: Trauma remains a major cause of mortality worldwide, particularly in low- and middle-income countries. Organized trauma systems and dedicated trauma services have been associated with improved outcomes; however, data from Southeast Asia remain limited. This study evaluated the impact of establishing a dedicated Trauma Surgery Unit (TSU) at a Malaysian tertiary referral center. METHODS: A single-center observational study was conducted at Hospital Tuanku Ja'afar Seremban, Malaysia, comparing trauma patients managed before TSU establishment (April-June 2024) and after implementati

Consultation-liaison psychiatry in general practice.

OBJECTIVE: This paper describes the characteristics of 303 consecutive referrals, over a 12-month period, to a consultation-liaison (C-L) psychiatry service provided to eight group general practices in Newcastle, Australia. METHOD: A purpose designed service audit form was used throughout the evaluation period to collect information about demographic characteristics, reasons for referral, service contacts, psychiatric diagnoses and clinical management. In addition, patients were invited to participate in a separate, prospective outcome evaluation study, which involved structured interviews an

Engaging the third estate: the Transplant Growth and Management Collaborative.

The Organ Donation and Transplantation Collaboratives that occurred within the United States from 2004 to 2008 helped contribute to a significant increase in organ donors and transplants across the country. Centers were needed to accommodate and maintain this increase in capacity to perform successful transplantations for candidates on the waiting list. The Transplant Growth and Management Collaborative was created to help fulfill this new performance level expectation. In 2007 the US Department of Health and Human Services, Health Resources and Services Administration published a best-practic

Disputes over coverage of emergency department services: a study of two health maintenance organizations.

STUDY OBJECTIVE: We describe the characteristics and outcomes of enrollee-health plan disputes over insurance coverage for emergency department (ED) services at 2 large health maintenance organizations (HMOs) that apply the prudent layperson standard. METHODS: We abstracted information from a stratified random sample of approximately 3,500 appeals of coverage denials lodged by privately insured enrollees between 1998 and 2000 at 2 of the nation's largest HMOs (hereafter referred to as Plan 1 and Plan 2). We describe appeals involving ED services in terms of the timing of visits, patient age,
