INTRODUCTION: Aligning dental arches in orthognathic treatment after undergoing LeFort I osteotomy or bilateral sagittal split osteotomy (BSSO) relies on occlusal splints, which do not provide precise control over the condylar head.
AIM OF THE STUDY: To clinically and radiographically assess the accuracy of computer-assisted design and manufactured (CAD/CAM) surgical guide for condylar positioning in a BSSO.
MATERIALS AND METHODS: The study recruited 8 patients with a non-syndromic dentofacial deformity who underwent a BSSO. Surgery was planned according to CAD/CAM technology. After osteotom

An abnormal maxillomandibular ridge relationship frequently hinders oral implant treatment in patients with jaw deformities. Here, we describe a patient who was experiencing difficulty using dentures due to multiple maxillary tooth loss and mandibular prognathism. Treatment comprising sagittal splitting ramus osteotomy and alveolar ridge augmentation using bone grafts harvested from the mandibular ramus followed by implant treatment yielded good outcomes. The patient was a 47-year-old woman presenting with an unstable upper partial denture. Although prior prosthetic treatment for mandibular pr

The aim of this study was to compare the fracture patterns after sagittal split osteotomy according to Obwegeser/Dal Pont (ODP) and Hunsuck/Epker (HE), as well as to investigate the relationship between lateral bone cut ending or angle and the incidence of unfavorable/bad splits. Postoperative cone-beam computed tomograms of 124 splits according to ODP and 60 according to HE were analyzed. ODP led to 75.8% and HE led to 60% lingual fractures with mandibular foramen contact. Horizontal fractures were found in 9.7% and 6.7%, respectively, and unfavorable/bad splits were found in 11.3% and 10%, r

We report a case involving a young female patient with severe mandibular retrognathism accompanied by mandibular condylar deformity that was effectively treated with Le Fort I osteotomy and two genioplasty procedures. At 9 years and 9 months of age, she was diagnosed with Angle Class III malocclusion, a skeletal Class II jaw relationship, an anterior crossbite, congenital absence of some teeth, and a left-sided cleft lip and palate. Although the anterior crossbite and narrow maxillary arch were corrected by interceptive orthodontic treatment, severe mandibular hypogrowth resulted in unexpected

PURPOSE: In order to minimize post-operative relapse after mandibular setback using bilateral sagittal split ramus osteotomy, we have recently tried a technique, i.e. intentional ostectomy of the posterior part of the distal segment. The aim of this study was to evaluate the effects of this technique on the frequencies and extent of post-operative relapse.
PATIENTS: This study was based on 61 cases of mandibular prognathism. The traditional sagittal split was performed in 24 cases (average age: 22.0+/-3.8) as a control group and the technique of additional distal ostectomy was used in the oth

UNLABELLED: Among the osteotomies performed in orthognathic surgery, the sagittal osteotomy of the mandibular ramus (SOMR) is the most common, allowing a great range of movements and stable internal fixation (SIF), therefore eliminating the need of maxillomandibular block in the postoperative period.
OBJECTIVES: The purpose of this study was to evaluate the biomechanical resistance of three national systems used for SIF in SOMR in sheep mandibles.
MATERIAL AND METHODS: The study was performed in 30 sheep hemi-mandibles randomly divided into 3 experimental groups, each containing 10 hemi-mand

Background/Objectives: Surgical extraction of impacted mandibular third molars is frequently associated with postoperative morbidity, including swelling, trismus, and pain. However, the extent to which osteotomy contributes to these outcomes remains unclear. The aim of this study was to evaluate osteotomy as an intraoperative determinant of early postoperative morbidity following mandibular third molar extraction. Methods: This study represents a secondary analysis of data obtained from a randomized clinical trial. Patients undergoing surgical removal of impacted mandibular third molars were c

RATIONALE: Orthognathic surgery cases are inherently challenging to treat with lingual appliances due to the complexities of orthodontic management and difficulties in achieving intermaxillary fixation during surgery. This challenge is further amplified in cases involving asymmetric space closure, such as those with a missing molar on one side and a premolar on the other, a scenario not yet documented in the literature. This case report presents the orthodontic-orthognathic management of an adult patient requiring space closure of asymmetric missing lower teeth.
PATIENT CONCERNS: A 30-year-ol

PURPOSE: Although the bilateral sagittal split osteotomy (BSSO) is a routinely performed procedure, exact control of the lingual fracture line remains problematic. The purpose of this study was to determine the various lingual splitting patterns in cadaveric human mandibles after a BSSO and the possible influence of the mandibular canal and mylohyoid groove on the lingual fracture line.
MATERIALS AND METHODS: The investigators designed and implemented a case series to compare different lingual fracture lines. A standardized SSO was performed on 40 cadaveric hemimandibles using elevators and s

The insufficient fibular height after mandibular reconstruction using vascularized single-barrel free fibular flap is a Gordian knot for implantation. Numerous studies have demonstrated that sandwich osteotomy is an effective technique for bone augmentation,so it is worth investigating whether it can achieve the same result used for transplanted fibula.A total of 18 patients were retrospectively enrolled. All patients underwent single-barrel free fibula flap repair for segmental mandibular defects but the fibular height was not sufficient for subsequent implantation, so sandwich osteotomy was
