An acute mastoiditis can occasionally present as a swelling in front of the ear. An inflammation of the air cells of the mastoid cavity can spread, via the cells in the root of the zygomatic arch, to the soft tissues of the cheek. Unfamiliarity with this underlying cause of a swollen cheek can lead to delay of proper treatment with potential harm to the patient. This case presents such an uncommon form of mastoiditis in a 3-year-old boy; the failure to recognize the disease initially led to extensive osteomyelitis of the temporal bone. CT scanning was important in establishing the cause of the…

A retrospective analysis of 29 cases of squamous cell carcinoma of the external and middle ear and 15 cases of glomus jugulare tumors of the middle ear was performed to evaluate the incidence of osteoradionecrosis of the temporal bone following surgery and/or radiotherapy. Osteoradionecrosis occurred in 42% (8/19) of cases of squamous cell carcinoma which were treated with radical mastoidectomy, lateral or subtotal temporal bone resection leaving an open cavity and followed by an average of 5,840 rad postoperatively. No osteoradionecrosis, wound breakdown or otorrhea occurred in another group …

One hundred years ago Robert Koch discovered tuberculous bacilli in tuberculous tissue. Since then the tuberculous disease has shown decreasing frequency, due to the specific antibiotics (Streptomycin since 1944) and to the BCG-Vaccination (Calmette and Guerin, 1921; vaccinations since 1948). This leads to difficulties in diagnosing the disease, especially in extrapulmonary forms (present in about 1:7 cases). One must consider also that about 40% of new diseases occur in the first 5 years of life and 80% in the first 14 years [9]. We report on two children with tuberculous otitis/mastoiditis i…

Fifty-four children with acute mastoiditis were managed at the Los Angeles County-University of Southern California Medical Center from 1972 through 1982. Our criteria for the diagnosis of acute mastoiditis are acute or subacute otitis media, postauricular swelling and erythema, protrusion of the auricle, and clouding of mastoid air cells on radiographs. Thirty-one (57%) recovered with conservative therapy consisting of early myringotomy and intravenous antibiotic, usually ampicillin. Twenty-three patients were managed surgically. The indication for surgery in each case was the clinical diagno…

It has been demonstrated that in most ears with middle ear effusion the mastoid air cells may be involved as well. The mucosal changes and the secretion extends into all cavities of the ear including the mastoid air cells. In most ears insertin of a ventilating tube through the tympanic membrane is adequate for proper aeration of the middle ear as well as of the mastoid air cell system. The draining usually stops and the mucosa gradually changes into its normal condition. In a small percentage of these, the ear will continue to drain and the mastoid will not clear up. Antibiotics and decongest…

Three unusual cases of mastoid abscess are described. Case histories and the clinical, laboratory, radiological, surgical, and histopathological findings in the patients are reported. Operculated eggs measuring about 60 mu times 40 mu were seen in pus obtained from two cases, but no adult worm was recovered. The findings suggest that the infections could have been caused by either Paragonimus species or Poikilorchis (Achillurbania) species.

Report of two cases with "acute mastoiditis" which was due to secondary malignant disease in the mastoid as shown postoperatively. One was the metastisis of an embryonic lung tumor previously diagnosed as histologically benign, the other one was the first sign of acute myeloid leukemia.

OBJECTIVE: To determine whether the subperiosteal abscess (SPA) volume in acute coalescent mastoiditis (ACM) as measured in imaging studies, should influence the decision-making process between performing mastoidectomy versus needle aspiration or incision and drainage (I&D) as the initial treatment of SPA.
SETTING: Single tertiary referral center.
PATIENTS AND METHODS: The records of all pediatric patients admitted with ACM between 1/2012 and 12/2023 were retrospectively reviewed. Baseline abscess volumes were measured on dedicated segmentation software for analyzing imaging findings. SPA vo…

BACKGROUND: Acute mastoiditis (AM) is the most common complication of acute otitis media (AOM) and could lead to serious complications if not diagnosed early and treated appropriately. Nowadays, there is no definitive consensus about the diagnostic algorithm and the optimal therapeutic management for patients with AM. The purpose of this study is to analyze the management of children admitted for AM and complicated AM (CAM) in a referral children's hospital, evaluating differences in clinical presentation and management to outline a diagnostic and therapeutic pathway. Moreover, the incidence o…

BACKGROUND: Tuberculosis (TB) otitis media is an uncommon site of extrapulmonary TB and can primarily present as a complicated TB mastoiditis (TBM). This complication is rare in children, even in TB endemic areas but necessitates early identification as delays can lead to severe morbidities. We describe the clinical characteristics as a case series to raise awareness of the condition, and highlight fundamentals related to diagnosis and management.
METHODS: A retrospective chart review of clinical and radiological information of five children with TBM seen at the Red Cross War Memorial Childre…
