Improving cardiovascular health of underserved populations in the community with Life's Simple 7.

PURPOSE: The purpose of this nurse practitioner (NP) led initiative was to improve the cardiovascular health of two underserved populations in the community using the American Heart Association (AHA) Life's Simple 7 and My Life Check (MLC) tools. DATA SOURCES: Two inner city community sites were targeted: (a) a senior center servicing African American (AA) older adults, and (b) a residential facility servicing homeless women. Preprogram health data (blood pressure, cholesterol, blood glucose levels, body mass index, and health behaviors) were collected to calculate MLC scores. Postprogram hea

Childhood stunting and caries increment in permanent teeth: a three and a half year longitudinal study in Peru.

BACKGROUND: Malnutrition has been consistently associated with caries in primary teeth, although an effect on permanent teeth has not been established because of the few longitudinal studies. AIM: To explore the association between stunting and caries increment in permanent teeth over 3.5 years. DESIGN: In 2003, 121 children aged 7-9 years were randomly selected from nine underserved communities in Lima (Peru). Parents provided demographic information and a food diary for their children. Clinical examinations included assessments of height, weight, oral hygiene, and dental caries. Stunting w

Evaluation and delivery of domestic animal health services in remote communities in the Northwest Territories: A case study of status and needs.

Domestic animal health services are supplied to communities in Canada's Northwest Territories (NT) in diverse ways, including private veterinary practices in 2 of 33 communities, and by mail-order, fly-in, free clinics, and a government-coordinated lay vaccinator program in some of the other 31 communities. We evaluated delivery, needs, and potential uptake of domestic animal health services in the Sahtu Settlement Area, NT by offering free clinics for 225 dogs in 2008 and 2009; and administered questionnaires to 42 dog owners and 67 students in 2008. Owners indicated that 20% of dogs were neu

Priorities and practice in tropical paediatrics.

More than 14 million children under 5 years of age die annually in the Third World, mainly due to diarrhoea, pneumonia, malaria and immunizable diseases. The problems of poverty, malnutrition, poor sanitation, illiteracy and high fertility that traditionally are associated with underdevelopment are now being compounded by social disruption due to rapid changes in lifestyle, new diseases such as AIDS and Third World debt. A vital part of the solution is provision of basic medical and education services to all, with emphasis on female literacy and improving the status of women. Key elements in p

The PRAMS study (Perinatal Redesign for Accessing Mental Health Services): a research protocol.

BACKGROUND: Perinatal mental health (PMH) problems affect 10-20% of women and birthing people during pregnancy and the postnatal year, costing the UK an estimated £8.1 billion annually. Underserved groups - including women and birthing people from minority ethnic groups, deprived areas, and those facing multiple disadvantages - experience the greatest inequalities in access and outcomes. Despite national investment, many fall between primary care (general practice, NHS Talking Therapies) and specialist PMH services, with limited guidance on bridging this gap. AIM: To co-design an experience-b

Persistent hepatitis burden in a geographically isolated and medically underserved population: a community-based study.

Ulleung County is a geographically isolated and medically underserved region in South Korea with a persistently high incidence of liver cancer. Using a community-based approach, this study aimed to estimate the prevalence of hepatitis B virus (HBV) and hepatitis C virus (HCV) infections and to identify associated risk factors. As part of the Ulleung Liver Cancer Prevention and Management Project, an interagency workgroup implemented community-based screening and health behavior surveys. Between 2019 and 2022, a total of 1,792 residents aged ≥ 40 years voluntarily participated, and data from 1,

Participation and Follow-Up Compliance in Cervical Cancer Screening According to Delivery Strategy for HPV Self-Collection: A Community Trial in Underserved Colombian Areas.

Low- and middle-income countries (LMIC) continue to bear a high burden of cervical cancer (CC). Women's participation in CC screening is challenging in LMICs, but self-collection of HPV tests has shown increased participation in controlled trials. However, data from real-world studies and data on follow-up compliance after positive screening test results are scant. A two-arm community trial was conducted in four municipalities from two different territories in Colombia (HPV self- and clinician-collected tests). A comparison of participation and follow-up compliance was done between arms, both

"Pushed to Their Limits": Health Care Provider Perspectives on Barriers and Facilitators to Implementing a Mind-Body and Activity Program for Older Adults With Chronic Pain in a Community Clinic for the Underserved.

Older adults from underserved backgrounds experience chronic pain at a rate of 60% to 75%. Pharmacological treatments have limited efficacy and involve considerable risks. Mind-body interventions hold promise to improve pain outcomes but are typically not implemented in community clinics in which they are needed most, thus contributing to health disparities in chronic pain treatment. We conducted qualitative focus groups and interviews with 20 providers (eg, primary care doctors, nurses, administrators). We sought their perspectives on barriers and facilitators to implementing an evidence base

Comparison of diagnostic accuracy of digital chest X-ray images between PACS and WhatsApp Messenger in resource poor setting.

PURPOSE: Many underserved remote locations without specialists would benefit from the ability to quickly and easily share images of radiographs with trained radiologists using WhatsApp messenger. However, there is limited evidence on the role of WhatsApp messenger for sharing chest x-ray (CXR) images to aid diagnosis and management. The objective of the study was to determine the diagnostic accuracy and inter-observer agreement of WhatsApp messenger images of digital CXR compared to viewing on Picture Archiving and Communication System (PACS) monitor. METHODS: Two pulmonologists reported 400

Mobile Mammography Participation Among Medically Underserved Women: A Systematic Review.

INTRODUCTION: Although breast cancer deaths have declined, the mortality rate among women from medically underserved communities is disproportionally high. Screening mammography is the most effective tool for detecting breast cancer in its early stages, yet many women from medically underserved communities do not have adequate access to screening mammograms. Mobile mammography may be able to bridge this gap by providing screening mammograms at no cost or low cost and delivering services to women in their own neighborhoods, thus eliminating cost and transportation barriers. The objective of thi

Substance use, risk of dependence, counseling and treatment among adult health center patients.

Health centers provide primary care to 20 million underserved patients. We examined the prevalence of substance use and risk of dependence among health center patients, and identified factors associated with desire for counseling/treatment and discussions about substance use with a doctor. National data on 3,949 adults came from the 2009 Health Center Patient Survey. Forty percent of patients reported past-year binge drinking, 14% of patients had used any drug in the past three months, and 13% of these recent users were at high risk of dependence. Eighty-four percent of patients who desired su

Stakeholder perceptions of a nurse led walk-in centre.

BACKGROUND: As many countries face primary care medical workforce shortages and find it difficult to provide timely and affordable care they seek to find new ways of delivering first point of contact health care through developing new service models. In common with other areas of rural and regional Australia, the Australian Capital Territory (ACT) is currently experiencing a general practitioner (GP) workforce shortage which impacts significantly on the ability of patients to access GP led primary care services. The introduction of a nurse led primary care Walk-in Centre in the ACT aimed to fu

Individual and health facility factors and the risk for obstructed labour and its adverse outcomes in south-western Uganda.

BACKGROUND: Obstructed labour is still a major cause of maternal morbidity and mortality and of adverse outcome for newborns in low-income countries. The aim of this study was to investigate the role of individual and health facility factors and the risk for obstructed labour and its adverse outcomes in south-western Uganda. METHODS: A review was performed on 12,463 obstetric records for the year 2006 from six hospitals located in south-western Uganda and 11,180 women records were analysed. Multivariate logistic regression analyses were applied to control for probable confounders. RESULTS: P

Quality assurance issues raised by proposed limited-service rural hospitals.

This article explores quality assurance issues that are likely to arise related to limited-service rural hospitals, an institutional alternative to existing rural hospitals. In exploring these issues, we use as an example, the Montana Medical Assistance Facility, a limited-service rural hospital model about to be implemented by the Health Care Financing Administration as a demonstration. While such medical assistance facilities will need to meet licensure and certification requirements, it is not reasonable to expect them to meet regulations that are designed for large hospitals. Also, because

Adoption of self-measured blood pressure monitoring in underserved communities: Program evaluation in primary care.

RATIONALE: Self-measured blood pressure (SMBP) monitoring is crucial for hypertension management, yet its adoption, particularly among disadvantaged populations, remains low. 'Love Your Heart' is a quality improvement program providing free standard SMBP devices to hypertensive patients, aiming to enhance adoption and assess its impact on blood pressure control. AIMS AND OBJECTIVES: This study evaluates the 'Love Your Heart' program's implementation success through assessing adoption rates and exploring changes in systolic blood pressure (SBP) among participants. We aim to understand factors

Establishing a SARS-CoV-2 (COVID-19) Drive-Through Collection Site: A Community-Based Participatory Research Partnership With a Federally Qualified Health Center.

The COVID-19 pandemic has disproportionately affected underserved and minority populations in the United States. This is partially attributable to limited access to diagnostic testing from deeply rooted structural inequities precipitating higher infection and mortality rates. We describe the process of establishing a drive-through collection site by leveraging an academic-community partnership between a medical institution and a federally qualified health center in Minnesota. Over 10 weeks, 2006 COVID-19 tests were provided to a socioeconomically disadvantaged population of racial/ethnic minor

Should men be exempted from vaccination against human papillomavirus? Health disparities regarding HPV: the example of sexual minorities in Poland.

INTRODUCTION: Social campaigns concerning vaccinations against human papillomavirus (HPV) in Poland are mainly addressed to women. In addition to cervical cancer, anal, penile, and oropharyngeal cancers can be caused by the virus, which clearly affects men as well. HPV vaccinations are voluntary and mostly not refunded in Poland. METHODS: A survey was published on social media's group gathering males and contained questions concerning epidemiological data, knowledge about HPV, and opinions of HPV vaccination. A questionnaire was enriched with educational note regarding HPV-dependent cancers a

Giant mucinous cystadenoma: a case report.

INTRODUCTION: Giant ovarian cysts are rarely described in the literature, owing to the availability of advanced imaging technologies in developed countries leading to early treatment. In resource-limited settings, various factors lead to late presentation. CASE PRESENTATION: We present a case of a 48-year-old black African woman with a giant mucinous cystadenoma who presented to a tertiary hospital with massive abdominal distention 5 years after being referred from a district hospital for the same problem. Surgical management resulted in fatal complications. CONCLUSIONS: The surgical managem

A direct-to-patient telemedicine abortion service in Australia: Retrospective analysis of the first 18 months.

BACKGROUND: In 2015, the Tabbot Foundation launched a nationwide direct-to-patient telemedicine service to enable women to obtain medical abortion without visiting an abortion provider. AIMS: We aimed to describe results from the first 18 months of this service. MATERIALS AND METHODS: To have an abortion through the Foundation, a woman obtained screening tests locally and had a telephone consultation with a Foundation doctor. If she was eligible, mifepristone, misoprostol and other medications were sent to her by mail. After taking the drugs, the woman obtained follow-up tests at local facil

A colorectal cancer screening program in an underserved, ethnically diverse population in Chicago, IL.

The purpose of this project was to describe the structure and outcomes of a colorectal cancer screening program at CommunityHealth, the largest free health clinic in Illinois. We conducted a retrospective observational study using administrative clinical data from 2006 to 2011. A total of 4,026 patients were eligible for colorectal cancer screening of which 2,418 (60.0 %) completed fecal occult blood testing (FOBT). Subsequently, 1,657 patients had negative FOBT results and 1,365 patients underwent on-site flexible sigmoidoscopy. Over 90 % of patients had never had a prior screening examinatio
