Improving access, efficacy, and cost effectiveness of alcohol rehabilitation for poor patients.

Numerous alcoholics and alcohol abusers unknown to other agencies enter hospitals for treatment of alcohol-related illnesses. Two well-constructed studies focusing on poor clients enrolled in alcohol rehabilitation programs do not duplicate the long-term medical cost savings typical for higher income populations. Study limitations are indicated. Alcohol rehabilitation might avert or reduce potentially even higher long-term costs for poor clients as well as a host of social costs. It is important to demonstrate and improve the accessibility, efficacy, and cost effectiveness of alcohol treatment

Optimizing Tobacco-Free Workplace Programs: Applying Rapid Qualitative Analysis to Adapt Interventions for Texas Healthcare Centers Serving Rural and Medically Underserved Patients.

BACKGROUND: Tobacco use is disproportionately high in rural areas, contributing to elevated cancer mortality, yet it often goes untreated due to limited access to care, high poverty and uninsured rates, and co-occurring substance use disorders (SUDs). This study explored the utility of using rapid qualitative analysis (RQA) to guide the adaptation of a tobacco-free workplace program (TFWP) in Texas healthcare centers serving adults with SUDs in medically underserved areas. METHODS: From September-December 2023 and May-July 2024, we conducted 11 pre-implementation, virtual semi-structured grou

Trends and predictors of leaving before medically advised in US emergency departments from 2016 to 2021.

BACKGROUND: Leaving before medically advised (BMA) is a significant issue in the US healthcare system, leading to adverse health outcomes and increased costs. Despite previous research, multi-year studies using up-to-date nationwide emergency department (ED) data, are limited. This study examines factors associated with leaving BMA from EDs and trends over time, before and during the COVID-19 pandemic. METHODS: We conducted a pooled, cross-sectional study using data from the National Hospital Ambulatory Medical Care Survey (NHAMCS) from 2016 to 2021. Multivariable logistic regression and inte

Financial Hardship From Medical Bills Among Adults With Chronic Liver Diseases: National Estimates From the United States.

BACKGROUND AND AIMS: Chronic liver diseases (CLD) affect approximately 2% of the U.S. population and are associated with substantial burden of hospitalization and costs. We estimated the national burden and consequences of financial hardship from medical bills in individuals with CLD. APPROACH AND RESULTS: Using the National Health Interview Survey from 2014 to 2018, we identified individuals with self-reported CLD. We used complex weighted survey analysis to obtain national estimates of financial hardship from medical bills and other financial toxicity measures (eg, cost-related medication n

Describing the Uniquely Vulnerable Patients Served by an Outpatient Non-Oncologic Palliative Clinic.

CONTEXT: Outpatient nononcologic palliative care is a growing field with an evolving identity. OBJECTIVES: The objective of this study was to assess patient demographics and clinical utilization of an outpatient nononcologic palliative care clinic. METHODS: We collected and analyzed data from the first two years of an urban, nononcologic palliative care clinic within a quaternary academic center. RESULTS: The diagnoses with highest clinic utilization were heart failure (46.2%) and end-stage lung disease (16.6%). The medical complexity of these patients was high, with geriatric patients comp

A syringe prescription program to prevent infectious disease and improve health of injection drug users.

Injection drug users (IDUs) are at increased risk for many health problems, including acquisition of human immunodeficiency virus (HIV) and hepatitis B and C. These risks are compounded by barriers in obtaining legal, sterile syringes and in accessing necessary medical care. In 1999, we established the first-ever syringe prescription program in Providence, Rhode Island, to provide legal access to sterile syringes, reduce HIV risk behaviors, and encourage entry into medical care. Physicians provided free medical care, counseling, disease testing, vaccination, community referrals, and prescripti

Catastrophic health expenditure in patients with Guillain Barre syndrome: Retrospective study.

INTRODUCTION: Catastrophic health expenditure (CHE) is a reliable measure of the financial unpreparedness of the studied population to meet unexpected health issues. The alarming proportion of patients who incur CHE in the wake of an acute neurological illness like Guillain Barre Syndrome (GBS) is of serious concern in a country like India where a large majority of households are uninsured. METHODOLOGY: Medical records of patients diagnosed with at a tertiary care centre in Delhi were analysed retrospectively to determine the rate of CHE. Clinical details and other contributory variables were

Pharmaceutical assistance programs to support smoking cessation medication access.

OBJECTIVES: Rural smokers are more likely to be uninsured and live in poverty, which may pose significant cost barriers to accessing smoking cessation medications. As part of a randomized clinical trial, we provided support to connect low-income smokers with the use of pharmaceutical assistance programs (PAPs) to improve medication access. METHODS: Study participants were rural smokers enrolled in a randomized clinical trial testing in-office telemedicine versus telephone-based approaches to deliver counseling sessions. For potentially qualified participants, we developed a system to connect

Uplifting the Social Determinants of Health of Adults Exiting Carceral Settings May Aid Health Care Access.

This article investigates health care access (i.e., health insurance coverage, usual source of care, doctor visit) among adults exiting carceral settings. Between 2022 and 2024, adults aged 18 and over (N = 202) enrolled in two health studies in San Diego, California. Surveys collected demographic and health care access data. Logistic regression models assessed factors independently associated with health care access. Overall, 27% of participants were uninsured, 66% had Medicaid insurance, and 21% lacked a usual place of care. In logistic regression models, adults released from custody within
