Anatomical variant of the meniscus related to posterior junction: a case report.

BACKGROUND: There are several reports on anatomical differences of the meniscus. However, there are only a few reports on abnormalities in both menisci and anatomical differences in anterior cruciate ligament insertions. CASE PRESENTATION: This is a case report of a 36-year-old Hispanic man presenting symptoms, including knee pain, locking, and effusion, with an anatomical abnormality of the menisci corresponding to the fusion of the posterior horns of the menisci in tandem with the insertion of the posterior meniscus fibers in the anterior cruciate ligament. CONCLUSIONS: This is the first s…

Reconstruction of the Popliteomeniscal Fascicles for Treatment of Recurrent Subluxation of the Lateral Meniscus.

Recurrent subluxation of the lateral meniscus is characterized by episodes of mechanical locking of the knee joint. To completely preclude the posterior segment of the lateral meniscus from undergoing anterior dislocation during deep knee flexion, the structures to which it is attached need to be relatively taut. The posterosuperior popliteomeniscal fascicle retains its tension during deep knee flexion; therefore, reconstruction of the posterosuperior and anteroinferior popliteomeniscal fascicles was performed with an autograft harvested from the iliotibial band. This technique provides stabil…

Association between medial meniscus extrusion and spontaneous osteonecrosis of the knee.

AIM: Medial meniscus tear has been proposed as a potential etiology of spontaneous osteonecrosis of the knee (SONK). Disruption of collagen fibers within the meniscus causes meniscal extrusion, which results in alteration in load distribution in the knee. Our purpose was to determine whether the extent of medial meniscus extrusion correlates with the severity of SONK in the medial femoral condyle. METHODS: Radiological stage of SONK and femorotibial angle (FTA) were determined on knee radiographs. Ellipsoid volume of SONK lesion and meniscal pathology (degeneration, tear and extrusion) were e…

Meniscus Shape and Wetting Competition of a Drop between a Cone and a Plane.

The formation of a liquid bridge between a cone and a plane is related to dip-pen nanolithography. The meniscus shape and rupture process of a liquid meniscus between a cone and a plane are investigated by Surface Evolver, many-body dissipative particle dynamics, and macroscopic experiments. Dependent on the cone geometry, cone-plane separation, and wetting properties of cone and plane, three types of menisci can be observed before rupture and two types of wetting competition outcomes are seen after breakup. It is interesting to find that after rupture, the bulk of the liquid bridge volume is …

Advances and Prospects in Tissue-Engineered Meniscal Scaffolds for Meniscus Regeneration.

The meniscus plays a crucial role in maintaining knee joint homoeostasis. Meniscal lesions are relatively common in the knee joint and are typically categorized into various types. However, it is difficult for inner avascular meniscal lesions to self-heal. Untreated meniscal lesions lead to meniscal extrusions in the long-term and gradually trigger the development of knee osteoarthritis (OA). The relationship between meniscal lesions and knee OA is complex. Partial meniscectomy, which is the primary method to treat a meniscal injury, only relieves short-term pain; however, it does not prevent …

Mechanoreceptors in the Anterior Horn of the Equine Medial Meniscus: an Immunohistochemical Approach.

Lameness due to stifle and especially meniscal lesions is frequent in equine species. In humans, mechanoreceptors involved in proprioceptive function are well studied. Given the high incidence of meniscal injuries in horses, and the lack of information concerning them in equine menisci, our objective was to study these corpuscles in six healthy anterior horns of the equine medial meniscus, which is the most common localisation reported for equine meniscal injuries. Immunohistochemical stainings were performed using antibodies against high molecular weight neurofilaments and glial fibrillary ac…

Meniscus repair and regeneration: review on current methods and research potential.

Meniscus regeneration is an unsolved clinical challenge. Despite the wide acceptance of the degenerative consequences of meniscectomy, no surgical procedure has succeeded to date in regenerating a functional and long-lasting meniscal fibrocartilage. Research proposed a number of experimental approaches encompassing all the typical strategies of regenerative medicine: cell-free scaffolds, gene therapy, intra-articular delivery of progenitor cells, biological glues for enhanced bonding of reparable tears, partial and total tissue engineered meniscus replacement. None of these approaches has been…

Root tear of the meniscus: biomechanical evaluation of an arthroscopic refixation technique.

BACKGROUND: Aim of the current study was to evaluate the structural properties of an arthroscopic refixation technique for meniscal root tears. HYPOTHESIS: Arthroscopic two suture root tear refixation technique using a transtibial approach restores the structural properties of the intact meniscus attachment more closely than a one suture technique. METHODS: Lateral root tear refixation was performed in a porcine knee model. Using a material testing machine, structural properties were determined after a cyclic loading protocol comparing transtibial tunnel reconstruction using one or a double …

Arthroscopic resection of a bucket handle tear of the meniscus: a technical note.

Arthroscopic resection of bucket handle tears of the meniscus is difficult owing to the obstructed view of the posterior horn. It often requires an additional portal for access or additional tools. We describe a technique involving conventional tools including grasper, shaver, and basket forceps. No additional portal is required. The basket forceps are used to cut the anterior attachment of the tear, and the bucket handle is shaved to a suitable size. The grasper is then used to hold the remaining meniscus and move it to the posterior space. The posterior corner of the meniscus is then turned …

The ineffectiveness of fibrin glue and cyanoacrylate on fixation of meniscus transplants in rabbits.

Our aim was to evaluate whether a fibrin glue and octyl-cyanoacrylate can promote fixation of meniscal allograft in rabbits. The medial menisci of 18 rabbits were frozen and stored at -73 degrees C (30 days) and then was allotransplantation and fixed by: GSu (n=6) soft tissue; GFi (n=6) fibrin glue; GCy (n=6) cyanoacrylate. They were evaluated by daily surgery recovery score (4 weeks), gross inspection and cells density on scaffold. A severe inflammatory response with caseous necrosis from the inside of the joint on through the approach incision led us to an early sacrifice (16th day) of all a

MR imaging of the meniscus: review, current trends, and clinical implications.

MR imaging is the preferred imaging modality for evaluating the meniscus. Overall, when strict criteria are followed, it is accurate in diagnosing meniscal tears in patients who have not had prior meniscal surgery. However, an accurate interpretation requires a through knowledge of the normal meniscal anatomy, common meniscal variants, and common diagnostic pitfalls. In this article, the author emphasizes the importance of describing meniscal tears properly and discusses treatment options. Diagnosing a recurrent tear is more complicated in patients who have had prior partial meniscal resection…

Degenerative meniscus tears and mobility impairment in women with knee osteoarthritis.

OBJECTIVE: Meniscus tears are often presumed to be associated with a traumatic event, but they can also occur as a result of the cartilage degeneration process in osteoarthritis (OA). The aim of this paper is to describe the prevalence and clinical correlates of degenerative meniscus tears in women with knee OA. METHOD: The subjects were women screened for a double-blind, sham-exercise controlled clinical trial for women over 40 years of age with OA in at least one knee, according to American College of Rheumatology (ACR) clinical criteria. The presence of meniscus tears was assessed via a 3T…

Distraction forces on repaired bucket-handle lesions in the medial meniscus.

BACKGROUND: Numerous studies have investigated the biomechanical properties of meniscal repair techniques. One of the most commonly discussed parameters is the failure load in the axis of insertion, although little is known about the distraction forces actually occurring at repaired bucket-handle lesions. HYPOTHESIS: There are clinically relevant distraction forces on repaired meniscus bucket-handle lesions. STUDY DESIGN: Controlled laboratory study. METHODS: Meniscus bucket-handle lesions were created and repaired in human cadaveric knees with a vertical suture made from a braided steel wi…

Is there an association between articular cartilage changes and degenerative meniscus tears?

PURPOSE: The objective of the present study was to evaluate whether horizontal cleavage and complex meniscus tears, which are degenerative tears, are associated with an increased incidence of cartilage damage, in comparison with patients having other patterns of meniscal injury. TYPE OF STUDY: Case series study. METHODS: Data were collected prospectively from 497 consecutive knee arthroscopies carried out from 1997 to 2001. Patient data (age, sex, duration of symptoms, injuries, and possible mechanism of injury), operative details (types and number of portals, equipment used), intra-articula…

[Meniscus transplantation as an option in case of painful arthrosis following meniscectomy].

The menisci play a role as shock absorbers and distribute the pressure uniformly over the joint cartilage. Other functions are: the nutrition of the joint cartilage, secondary stability and proprioception. In case of a torn meniscus, one should first attempt to suture the meniscus, followed by arthroscopic partial meniscectomy if suturing is impossible. In a large proportion of patients, arthrosis with pain and loss of function of the knee develops several years after the meniscectomy. In order to alleviate the symptoms of such arthrosis, a trial of meniscus transplantation was undertaken. In …

The anterior horn of the medical meniscus. An anatomic study of its insertion.

A morphologic study of 48 cadaveric knees was performed to more accurately define the osseous and soft tissue anatomy of the insertion of the anterior horn of the medial meniscus. Soft tissue relationships of the anterior horn of the medial meniscus to the anterior cruciate ligament and the lateral meniscus were examined. Four tibial insertion locations of the medial meniscus were identifiable by bony landmarks. Type I insertions were located in the flat intercondylar region of the tibial plateau; type II occurred on the downward slope from the medial articular plateau to the intercondylar reg…

Lateral pterygoid muscle and its relationship to the meniscus of the temporomandibular joint.

OBJECTIVE: The theory of internal derangement of the temporomandibular joint involves the anterior displacement of the meniscus, which is thought to be the result of hyperactivity of the lateral pterygoid muscle. There is, however, a great deal of confusion with respect to the attachment of this muscle to the meniscus. This anatomic study reexamines the meniscal attachment of the lateral pterygoid muscle, in particular, the upper head of this muscle. STUDY DESIGN: The anatomic relationship between the lateral pterygoid muscle and the meniscus was examined in the postmortem specimens of 25 mal…

[Relevance of proton spin tomographic meniscus diagnosis in correlation with arthroscopy].

Arthroscopy of the menisci is considered the gold standard by which all noninvasive imaging procedures of the knee are measured. In a prospective study we evaluated the use of MRI in 50 patients in whom a disorder of the meniscus was suspected clinically; this was followed by an arthroscopic examination by an experienced arthroscopist. The MR studies were performed after clinical evaluation and were interpreted by an experienced radiologist, who had no knowledge of the clinical findings. The accuracy of the diagnosis from MRI was 78% for the medial meniscus (sensitivity 79% and specificity 78%…
