Do patients with young onset Alzheimer's disease deteriorate faster than those with late onset Alzheimer's disease? A review of the literature.

BACKGROUND: Young onset Alzheimer's disease (YOAD; onset before 65 years of age) is thought to have a more rapid course and increased rate of progression compared to late onset Alzheimer's disease (LOAD). This assumption appears partly due to important clinical, structural, neuropathological, and neurochemical differences suggesting YOAD is a separate entity to LOAD. The aim in this review was to systematically identify and examine appropriate studies comparing rate of cognitive decline between patients with YOAD and patients with LOAD. METHODS: A computer-based literature search was initiall

Assessing capacity to consent to sexual relations: a guide for nurses.

The right to marry and found a family is a fundamental freedom bestowed on adults under the European Convention of Human Rights (Council of Europe 1950). Engaging in sexual activity is an essential component of that right and should be free from state interference. Community nurses, however, often express concern over the possibility that adults with learning disabilities or other impairments of the mind cannot give a real consent to participate in sexual relations and so should be prevented from doing so in their own best interests. In this article Richard Griffith and Cassam Tengnah review t

[Surgery for unruptured anterior cerebral artery aneurysms].

In Japan, a large number of surgical treatments for unruptured intracranial aneurysms have been performed. On the other hands, it is known that surgical treatment for unruptured intracranial aneurysms is the most frequent source of lawsuits in the neurosurgical field. Neurosurgeons have the duty to disclose all risks and consequences of a proposed surgical procedure to each patient. Moreover, precise knowledge of surgical risks would be helpful in preventing complications from occurring. However, it is almost impossible that a single surgeon or an institute have experiences with all possible s

[Testament's ability in Balkan endemic nephropathy].

INTRODUCTION: Testament is a solemn, authentic instrument in writing, by which a person declares his or her will as to disposal of his or her estate, and it has a psychopathological, lawful and ethical importance to a person, family and society. The aim of the study was to assess if the ability to make a testament was more damaged in patients with Balkan Endemic Nephropathy (BEN) than in patients with other diseases that resulted in Chronic Renal Failure in Bosnia and Herzegovina in the period from the 1st January 2001 to 31st December 2006. MATERIAL AND METHODS: The 753 respondents were divi

Sleepwalking disorder and mens rea: a review and case report. Maricopa County Superior Court.

This paper examines complications of sleepwalking disorder (DSM-IV 307.46), an arousal disorder or parasomnia, in relationship to mens rea, or culpable mental state necessary to a finding of criminal responsibility. The legal history of criminal intent and insanity is reviewed. A case of indecent exposure is discussed in a man with a history of closed head injuries and sleepwalking disorder who was found standing naked in the middle of a busy urban thoroughfare in the wee hours of the morning and arrested. On psychiatric evaluation, the defendant was found to have a long-standing sleepwalking

Training skills in the psychiatrically disabled: learning coping and competence.

Social skills training methods represent a major strategy for psychiatric rehabilitation. Building skills in patients with schizophrenic and other major mental disorders is based on the assumption that coping and competence can override stress and vulnerability in reducing relapses and improving psychosocial functioning. For maximum efficiency, skills training needs to incorporate procedures and principles of human learning and information processing. Several models for skills training have been designed and evaluated, each of which has proved to be effective in raising the social competence o

False belief and the refusal of medical treatment.

May a doctor treat a patient, despite that patient's refusal, when in his professional opinion treatment is necessary? This is the dilemma which must from time to time confront most physicians. An examination of the validity of such a refusal is provided by the present authors who use the case history of a patient refusing treatment, for cancer as well as for a fractured hip, to evaluate the grounds for intervention in such circumstances. In such a situation the patient is said to have a 'false belief' and it is the doctor's duty to try to change that belief in the patient's interest. The fals

Effects of a six-week basketball-specific individual skills development program on mental toughness and sport competition anxiety.

INTRODUCTION: This quasi-experimental study aimed to investigate the impact of a six-week basketball-specific individual skills development program on mental toughness and competitive anxiety among young professional male basketball players competing in organized leagues. Mental toughness and anxiety are critical psychological factors that influence athletic performance, particularly in high-pressure sports like basketball. The study sought to bridge the gap in existing research by examining how individualized training programs can simultaneously enhance mental resilience and reduce anxiety le

Early caregiving quality predicts consistency of competent functioning from middle childhood to adolescence following early psychosocial deprivation.

Adverse developmental outcomes for some children following institutional care are well established. Removal from institutional care and placement into families can promote recovery. However, little is known about how positive outcomes are sustained across adolescence among children with histories of severe deprivation. The present study examined the caregiving conditions that are associated with attaining and maintaining competent functioning (i.e., outcomes within typical levels) from middle childhood to adolescence following exposure to early institutional care. The participants included chi

Amnesia and crime.

Amnesia for serious offenses has important legal implications, particularly regarding its relevance in the contexts of competency to stand trial and criminal responsibility. Forensic psychiatrists and other mental health professionals are often required to provide expert testimony regarding amnesia in defendants. However, the diagnosis of amnesia presents a challenge, as claims of memory impairment may stem from organic disease, dissociative amnesia, amnesia due to a psychotic episode, or malingered amnesia. We review the theoretical, clinical, and legal perspectives on amnesia in relation to

The interplay of actual and perceived motor competencies, physical activity and well-being: a child-centered approach.

BACKGROUND: Motor development models highlight the interplay between actual motor competence, perceived motor competence and physical activity in children. In studies, mental health parameters, such as well-being, are mostly considered as an outcome of PA or motor development, but they can also be the prerequisite for participation in PA. While variable centered studies have explored mostly correlations among these factors, there is a need to adopt person-centered approaches to identify distinct profiles based on these variables. Understanding these profiles can support targeted interventions

Benchmarking Language Models for Clinical Safety: A Primer for Mental Health Professionals.

BACKGROUND: Millions of people use language models to discuss mental health concerns, including suicidal ideation, but limited frameworks exist for evaluating whether these systems respond safely. Benchmarking, the practice of administering standardized assessments to language models, offers direct parallels to clinical competency evaluation, yet few clinicians are involved in designing, validating, or interpreting these assessments. AIMS: To introduce mental health professionals to benchmarking language models by administering a validated clinical instrument and demonstrating how configurati

The mental health and well-being of internally displaced female Yazidis in the Kurdistan Region of Iraq: a realist review of psychosocial interventions and the impact of COVID-19.

BACKGROUND: Yazidis in the Kurdistan Region of Iraq have been exposed to recurrent traumatic experiences associated with genocide and gender-based violence (GBV). In 2014, ISIS perpetrated another genocide against the Yazidi community of Sinjar. Women and girls were held captive, raped and beaten. Many have been forced into displacement. Rates of post-traumatic stress disorder (PTSD) and suicide are high. Limited research has evaluated interventions delivered to this population. METHODS: This review explores how the global evidence on psychosocial interventions for female survivors of conflic

Characterizing mechanisms of a theory-underpinning mHealth multicomponent supportive program for adult children caregivers of community-dwelling frail older adults (Hi-Care): a randomized clinical trial.

BACKGROUND AND OBJECTIVES: Family caregivers of community-dwelling frail older adults often experience substantial stress and poor mental health. This study aimed to examine whether and how a Stress Process Model (SPM)-based mHealth multicomponent supportive program (Hi-Care) promoted adult children caregivers' mental well-being. RESEARCH DESIGN AND METHODS: A 3-month randomized controlled trial (n = 136) was conducted among the Hi-Care intervention group versus usual care. Caregivers' mental outcomes (depressive symptoms and anxiety symptoms), potential mediators (mastery and competence), an

Mental health challenges and substance use in student-athletes: prevalence and impact.

BACKGROUND: Mental health significantly impacts athletes' daily functioning and performance. Some coping techniques, such as substance abuse, can lead to addictive behaviors detrimental to sport participation. PURPOSE: This study aimed to identify the prevalence of anxiety, depression, and substance use in varsity student-athletes, examine their associations, and determine if academic and athletic factors (e.g., competition level, current sport season) are linked to these issues. METHODS: An anonymous survey assessed mental health (anxiety and depression) and substance use in varsity athlete

Evaluation of an interaction-skills training for reducing the burden of family caregivers of patients with severe mental illness: a pre-posttest design.

BACKGROUND: Family members who care for patients with severe mental illness experience emotional distress and report a higher incidence of mental illness than those in the general population. They report feeling inadequately prepared to provide the necessary practical and emotional support for these patients. The MAT training, an Interaction-Skills Training program (IST) for caregivers, was developed to meet those needs. This study used a single-arm pretest-posttest design to examine the impact of the training on caregivers' sense of competence (self-efficacy) and burden. METHODS: One hundred

Internally directed cognition and mindfulness: an integrative perspective derived from predictive and reactive control systems theory.

In the present paper, we will apply the predictive and reactive control systems (PARCS) theory as a framework that integrates competing theories of neural substrates of awareness by describing the "default mode network" (DMN) and anterior insula (AI) as parts of two different behavioral and homeostatic control systems. The DMN, a network that becomes active at rest when there is no external stimulation or task to perform, has been implicated in self-reflective awareness and prospection. By contrast, the AI is associated with awareness and task-related attention. This has led to competing theor
