Pollicization for congenital thumb aplasia using the second dorsal metacarpal artery as the vascular pedicle: case report.

The author presents the case of a child with bilateral thumb hypoplasia. Pollicization of the left index finger with grade 4 thumb hypoplasia was attempted and abandoned, apparently because of anomalous vascular anatomy. A secondary procedure identified a substantial second dorsal metacarpal artery that connected with a palmar-ulnar digital vessel at the level of the metacarpal neck. Successful pollicization was performed using the second dorsal metacarpal artery and its venae comitantes as the vascular pedicle.

Transient talipes equinus deformity of bilateral lower limbs following malignant hyperthermia: a case report and review of literature.

Malignant hyperthermia (MH) is a potentially fatal disease triggered by general anaesthesia. Four cases of compartment syndrome complicating MH have been reported. We report here a case of transient talipes equinus deformity of bilateral lower limbs, a condition similar to compartment syndrome, following MH in a previously healthy pediatric patient. MRI revealed high intensity in the bilateral soleus muscles but not in the gastrocnemius muscles. We discuss the possible mechanisms of the deformity of the lower limbs after the MH crisis.

Type 1B external fixation of a metacarpal fracture in an alpaca.

External skeletal fixation can be used in small ruminants as a successful, economic, alternative to internal fixation. In this case report, a type 1B frame was used on an alpaca to salvage a comminuted metacarpal fracture after failed internal fixation. Positioning the frame on the caudal aspect of the limb allowed full carpal flexion for kneeling and hence normal use of the limb throughout the recovery period. Complete healing occurred without further complications.

Complex bilateral polysyndactyly featuring a triplet of delta phalanges in a syndactylised digit.

The delta phalanx is a rare congenital skeletal anomaly. An abnormal C-shaped epiphysis is usually responsible for a progressive angular digital deformity observed either in hands or feet. Solitary delta phalanges are usually described. We report a case of bilateral congenital hand malformations featuring a triplet of delta phalanges affecting a single digit on one hand, together with a concealed central polydactyly on the other.

The results of operative treatment of fractures of the thumb metacarpal base.

The results of operative treatment of 21 patients with intraarticular fractures of the thumb metacarpal bone are presented. In 14 patients closed reduction was performed followed by percutaneous fixation with one or two K-wires through the metacarpal shaft to the trapezium. In five delayed cases, in which closed reduction was unsuccessful, open reduction and K-wire pinning was performed. After operation the hand and wrist were immobilized for four weeks. In two patients with Rolando-type fractures, an external fixator was used after closed reduction. Patients were followed-up for six months to

Long first metacarpal in monozygotic twins with probable Baller-Gerold syndrome.

We report on a pair of monozygotic twins with probable Baller-Gerold syndrome (BGS). Twin A had severe coronal craniosynostosis. Twin B had right radioulnar and ipsilateral first metacarpal hypoplasia. Both had bilateral fifth finger clinodactyly. Assuming that the twins were truly monozygotic, a single genetic disorder (i.e., BGS) could explain the variable expression. Together the twins have the typical anomalies of BGS. The diagnosis was supported by the metacarpophalangeal profile (MPP) which confirmed hypoplasia of the first right metacarpal in Twin A and bilateral fifth finger brachymeso

Metacarpal lengthening after traumatic amputation of the thumb.

In nine patients of median age 34 years who had sustained an amputation of the thumb at a median 24 (5 to 131) months previously, we lengthened the first metacarpal by 30 (17 to 36) mm. Seven amputations had been through the proximal phalanx and two through the metacarpal. The first two patients had autogenous grafting at a second stage, but the other seven had callotasis alone. In these patients the external fixators were removed at a median 189 (115 to 271) days after osteotomy. In six cases the adductor pollicis tendon was transferred proximally and the first web deepened. There was late fr

Distal metacarpal bone mineral density by dual energy X-ray absorptiometry (DEXA) scan. Methodological investigation and application in rheumatoid arthritis.

Dual energy X-ray absorptiometry scanning was performed along the axis of the third metacarpal bone of the non-dominant hand and including metacarpal bones 2, 3, 4 and 5. The Bone Mineral Density (BMD) was calculated for the distal 1/4 of each metacarpal bone. Ten patients with seropositive, erosive rheumatoid arthritis (RA) and 10 healthy, sex- and age-matched persons were investigated twice. The average BMD in RA patients was 73.6% of the value found in normals. The coefficient of variation on double determinations (in patients and controls) was 0.9-3.0%. We suggest that dual energy X-ray ab

An unusual association of the windblown hand with upper limb hypertrophy.

An eight-year-old boy presented with a congenital windblown hand deformity associated with partial hypertrophy of that arm. Many of the classical features of the windblown hand syndrome described in previous papers were absent. In this case, the primary cause of the ulnar-drift deformity was an anomalous epiphysis at the base of the second metacarpal. Surgical correction was readily obtained by osteotomy at the base of the metacarpal combined with soft tissue re-alignment.

Transitional cell carcinoma of the renal pelvis metastatic to the metacarpal. A case report correlating cytologic and histologic findings.

This report describes the cytologic, radiologic and histologic findings in a 76-year-old male who presented with a pathologic fracture of the first metacarpal bone as the result of metastatic transitional cell carcinoma. The primary neoplasm was sited in the right renal pelvis. Metastases were also detected in the liver and confirmed cytologically. Problems encountered with the cytologic diagnosis are explained by correlation with the histologic findings. The case also illustrates the importance of clinicopathologic correlation when interpreting fine needle aspiration biopsies.

Metacarpophalangeal pattern profiles: Q-score for ages from birth to 7 years.

The metacarpophalangeal pattern profile (MCPP) analysis is a widely used method of standardization of bone length measurements to quantify congenital skeletal dysplasia of the hands. A main source of inaccuracy is the prominent scatter in the values of the standard deviation (S.D.) of the reference length, which is used in the calculation of the standardized values (Z-scores). Other sources of error, especially when bone length measurements of younger children are evaluated, are the rather large variability in the age of appearance of the epiphyses of the different bones, and the use of tabula

[An unusual displacement fracture of the 1st metacarpal bone. Apropos of a case reviewed after 3 years].

An unusual fracture of the base of the first metacarpal with in contrast with Bennett's fracture displacement of the thumb and avulsion of the Abductor Pollicis Longus tendon is described after a "Rugby" trauma. Immediate repair and stabilization of the injured structures followed by specific rehabilitation, produced a satisfactory result with normal function of the thumb three years after the injury.
