Peak inspiratory flow rate measurement in inhaler technique optimisation across inhaler devices.

BACKGROUND: Peak inspiratory flow rate (PIFR) is an important physiological parameter to assess during inhaler technique optimisation (ITO) and inhaler device selection, providing insight into patients' ability to generate appropriate inspiratory flow for effective medication delivery. OBJECTIVES: To determine the proportion of patients achieving optimal PIFR ranges before and after ITO across inhaler devices using the In-Check G-16 DIAL, and assess the relationship between PIFR ranges and clinical outcomes (self-reported exacerbation frequency, ACQ-6 and CAT scores). METHODS: 586 adult inha

Recent developments in respiratory care pharmacology.

Bronchoactive inhaled aerosol drugs target the respiratory tract directly and seek to minimize systemic exposure and reduce side effects. Common delivery devices such as the metered dose inhaler, the small volume nebulizer, or the dry powder inhaler each deliver approximately the same fraction of dose (10%) to the lungs, although their dose amounts are not equivalent. Major respiratory drug groups are reviewed, and include the beta-adrenergic and anticholinergic bronchodilators, mucolytic agents, corticosteroids, mediator antagonists, anti-infective agents, and exogenous surfactants. New agent

Assessment of Emergency Spacers Versus Traditional Spacers, An In-Vitro Model for Aerosol Delivery.

Spacers, when used with pressurized metered dose inhalers (pMDIs), enhance aerosol drug delivery and address coordination challenges during inhalation. This study aimed to compare the efficacy of emergency spacers with traditional spacers in delivering salbutamol aerosol from pMDIs. The total emitted dose (TED) and particle size distribution of salbutamol were determined using an Andersen MKII cascade impactor. The study evaluated pMDI alone and with various spacers, including traditional antistatic spacers (Able, Tips-Haler, Aerochamber Plus Flow Vu, Atomizer Chamber) and emergency spacers (P…

Comparison of adverse events associated with different spacers used with non-extrafine beclometasone dipropionate for asthma.

Co-prescription of Aerochamber® spacer with non-extrafine beclometasone diproprionate (non-EF BDP) is common but unlicensed. We report a comparison of inhaled corticosteroid (ICS)-related adverse events between patients co-prescribed Aerochamber compared to the licensed Volumatic® spacer. We utilised two historical cohorts: questionnaire-based and electronic medical record (EMR)-based, to assess patient-reported and EMR-recorded adverse events in patients with asthma prescribed non-EF BDP. Marginal effect estimate (MEE) was calculated to determine non-inferiority of Aerochamber compared to Vol

The pharmacological management of asthma-chronic obstructive pulmonary disease overlap syndrome (ACOS).

Introduction: Asthma-chronic obstructive pulmonary disease overlap syndrome (ACOS) is a disease phenotype that shares T helper lymphocyte cell Th1/neutrophilic/non-Type-2 Inflammation pathways thought to be key in COPD and Th2/eosinophilic/Type-2 inflammatory pathways of asthma. The pharmacology of treating ACOS is challenging in severe circumstances.Areas covered: This review evaluates the stepwise treatment of ACOS using pharmacological treatments used in both COPD and asthma. The most common medications involve the same inhalers used to treat COPD and asthma patients. Advanced stepwise ther

Measurements of electrodynamic effects on the deposition of MDI and DPI aerosols in a replica cast of human oral-pharyngeal-laryngeal airways.

Metered dose inhalers (MDIs) and dry powder inhalers (DPIs) are popular drug delivery devices used in the treatment of respiratory diseases such as asthma and chronic obstructive pulmonary disease (COPD). Integrated effects of electrostatic charges and aerodynamic sizes on the deposition of MDI and DPI particles in a replica cast of human oral-pharyngeal-laryngeal (OPL) airways were examined. Experimental aerosols were generated from commercially available MDI and DPI devices. They are the trademarked brands of the same pharmaceutical company, and contain the same amounts of different drugs. I…

Impact of Preliminary Bronchodilator Dose in Chronic Obstructive Pulmonary Disease Patients With Suboptimal Peak Inspiratory Flow.

PURPOSE: Suboptimal peak inspiratory flow rate (PIFR) is highly prevalent in patients with chronic obstructive pulmonary disease (COPD) owing to the mismatch of their PIFR with the corresponding inhaler-device resistance. This study aimed to investigate the impact of a preliminary dose of pressurized metered dose inhalers (pMDIs) on patients with COPD with suboptimal PIFR using Diskus dry powder inhalers (DPIs). METHODS: A prospective, randomized, case-control study included 24 patients with COPD. PIFR was measured using the In-Check Dial G16 with low-to-medium resistance. Spirodoc was used t…

Twelve hours' bronchodilating effect of inhaled formoterol in children with asthma: a double-blind cross-over study versus salbutamol.

Thirteen children, mean age 10.9 yr, with perennial asthma, were studied with respect to the duration of the bronchodilating effect of formoterol, a new long-acting beta 2 agonists for inhalation. The duration of action of formoterol metered dose aerosol (12 micrograms) was compared with salbutamol metered dose aerosol (200 micrograms) in a double-blind cross-over study. Formoterol was found to have significantly better bronchodilating effect 8 hr (P less than 0.01) and 12 hr (P less than 0.05) after inhalation of the drug. Formoterol (24 micrograms) was given single-blindly on the third trial

Salbutamol Transport and Deposition in the Upper and Lower Airway with Different Devices in Cats: A Computational Fluid Dynamics Approach.

Pressurized metered-dose inhalers (pMDI) with or without spacers are commonly used for the treatment of feline inflammatory airway disease. During traditional airways treatments, a substantial amount of drugs are wasted upstream of their target. To study the efficiency of commonly used devices in the transport of inhaled salbutamol, different computational models based on two healthy adult client-owned cats were developed. Computed tomographic images from one cat were used to generate a three-dimensional geometry, and two masks (spherical and conical shapes) and two spacers (10 and 20 cm) comp

Respiratory failure due to morbid obesity in a patient with Prader-Willi syndrome: an experience of long-term mechanical ventilation.

Prader-Willi syndrome (PWS) is characterized by obesity, mild mental retardation or learning disability, and behavior problems, especially in association with food and eating. A 19 year-old man, 150 cm, 140 kg (body mass index [BMI], 62.2 kg.m(-2)), whose condition had been diagnosed as PWS, received 41-day mechanical ventilation because of respiratory failure, chiefly due to morbid obesity. Because the patient frequently developed bronchoconstriction, metered-dose inhalers of a corticosteroid (beclomethasone dipropionate) and a beta2 agonist (salbutamol) were needed. To achieve adequate sedat

Comparison of output particle size distributions from pressurized aerosols formulated as solutions or suspensions.

The delivery of particles as small as possible (preferably less than 5 microns) to the respiratory tract should be the aim of those formulating metered dose inhalers (MDIs). This may be facilitated by the formulation of solution, rather than suspension-type, pressurized aerosol units. Two series of MDIs were compared; one contained suspended micronized disodium fluorescein (0.1%, w/v), while the other contained the same concentration of dissolved salicylic acid. Either oleic acid, L-alpha-phosphatidylcholine, or sorbitan trioleate was incorporated at 0.15% (w/v) as suspending agent (disodium f…
