Itraconazole treatment of coccidioidomycosis. NAIAD Mycoses Study Group.

PURPOSE: The purpose of this study was to assess the tolerance and efficacy of itraconazole in the treatment of coccidioidomycosis. PATIENTS AND METHODS: Fifty-one patients with nonmeningeal coccidioidomycosis were considered for treatment with intraconazole. Forty-nine patients who met study criteria were treated with itraconazole given orally in doses of 100 to 400 mg/day for periods up to 39 months. Of these patients, 12 had osteoarticular disease, 23 had chronic pulmonary disease, and 14 had skin or soft tissue disease. Clinical response was evaluated using a scoring system accounting for

Epidemiologic risk factors associated with canine systemic mycoses.

A prevalence study was designed to evaluate host and seasonal risk factors associated with three systemic mycoses in dogs: blastomycosis, coccidioidomycosis, and histoplasmosis. Histoplasmosis was the most commonly diagnosed of the three diseases. All three of the mycoses occurred more frequently in male than female dogs. Compared to dogs less than 2 years of age, those 4--7 years of age were at increased risk of developing coccidioidomycosis whereas dogs 2--7 years of age were at increased risk of developing histoplasmosis. Three breeds, i.e., Pointer, Weimaraner and Brittany Spaniel, had inc…

Siblings in the ICU: Keeping Endemic Mycoses on the Differential.

Blastomycosis is a rare fungal infection caused by the inhalation of Blastomyces dermatitidis spores. Infection with this fungus can impact nearly every organ system, though pulmonary disease is the most common. Presentations of pulmonary blastomycosis are highly variable, ranging from clinically asymptomatic to severe respiratory failure requiring intensive care. This case series describes the clinical presentation, diagnostic challenges, management, and outcomes of two siblings with severe pulmonary blastomycosis that progressed to pediatric acute respiratory distress syndrome requiring mech

Subcutaneous mycoses - The hidden mould, a case series.

Subcutaneous mycoses are a heterogenous group of infections common in subtropical and tropical regions. These infections have an insidious onset and slowly evolve as the pathogen survives and adapts to the host tissue environment. Common subcutaneous fungal infections include Sporotrichosis, Chromoblastomycosis, Mycetoma, Subcutaneous Zygomycosis, Subcutaneous Phaeohyphomycosis, Lobomycosis and Rhinosporidiosis. Even with rapid urbanization, the incidence of subcutaneous mycoses is on the rise. Confirmation by histopathological examination is necessary.

Complications of invasive mycoses in organ transplant recipients.

Opportunistic mycoses remain a significant complication in organ recipients. Areas covered: This review is an evidence-based presentation of current state-of-knowledge and our perspective on recent developments in the field Expert commentary: Invasive fungal infections are associated with reduced allograft and patient survival, increase in healthcare resource utilization, and newly appreciated but largely unrecognized immunologic sequelae, such as immune reconstitution syndrome. Given adverse outcomes associated with established infections, prophylaxis is a widely used strategy for the prevent…

[Epidemiology of Visceral Mycoses in Autopsy Cases in 2011].

To identify recent trends in the frequency of mycoses in autopsy cases, we performed an epidemiological analysis using the data reported in the "Annual Report of Autopsy Cases in Japan" from 2011. 12,339 cases were autopsied, of which 608 (4.9 %) were found to have fungal infections. Of these, 411 cases (67.6 %) were male, the median age was 67, and 353 cases were severe (58.1 %). Aspergillus spp. was the most predominant causative agent among those infected with one pathogen. These data corroborate our previous data from reports in 2007 and 2009. For the first time since 1989, we observed a c…

Skin and nail mycoses in patients with diabetic foot.

Diabetes mellitus affects all socioeconomic and age groups and its incidence is rapidly increasing worldwide. The diabetic foot complication represents one of the most complex and serious complications in these patients. Fungal infections can also contribute to the severity of the diabetic foot. The aim of the present study was to evaluate the prevalence of foot skin and toenail mycosis in a group of 75 patients with diabetic foot complication and in a matched control group. Diabetic patients showed onychomycosis in 53.3% and foot skin mycosis in 46.7% of the cases, with a prevalence of both f

[Selected aspects of modern epidemiology of ENT-mycoses].

The prevalence of mycosis affecting ears, nose, and throat was analysed based on the literature data and the results of original observations of 3964 patients who applied for medical aid to the counseling department of our centre during the period from 2005 to 2007. The study revealed a relative increase in the occurrence of ear, nose, and throat mycosis in the structure of chronic inflammatory pathology. Specifically, the frequency of mycotic otitis rose to 25.2%, mycotic infestation of the pharynx in patients with pharyngitis and tonsillitis to 28.7%, mycotic infestation of the larynx associ

[Imported mycoses in Japan: their present status and problems].

Imported mycoses in Japan, particularly coccidioidomycosis and histoplasmosis, have been on the increase in the past 15 years, and more than 40 patients have been reported to suffer from each of these diseases. In spite of their high infectivity and virulence, imported mycoses have received little attention by physicians. Concerning coccidioidomycosis, in particular, physicians should keep abreast of updated and detailed information, and should maintain close contact with clinical microbiologists. In this review, the general profile of coccidioidomycosis, including its diagnosis and treatment,…

Management of mycoses in surgical patients -- review of the literature.

Fungal infections have been recognized as major cause of morbidity and mortality in neutropenic and non-neutropenic surgical intensive care patients. The incidence of Candida has increased: it is now the fourth most often isolated pathogen in bloodstream infections. The incidence of Aspergillus infection in transplant patients is highest in heart and lung transplants: 19-26%. Most invasive fungal infections in surgical patients are caused by Candida spp. and Aspergillus spp., less by Cryptococcus spp. and may be classified as local or organ-related, as (chronic) disseminated, and as fungemia.

Voriconazole -- better chances for patients with invasive mycoses.

The past two decades have witnessed an increase in serious fungal infections, without corresponding growth in available antifungal agents. Voriconazole (VRC) is a novel triazole antifungal, recently approved in Europe for treatment of serious infections caused by Aspergillus, Fusarium, Scedosporium, and resistant Candida species. Voriconazole has in vitro activity against yeasts and yeast-like fungi similar, or superior to, fluconazole (FLC), itraconazole (ITC) and amphotericin B (AMB). Candida albicans is generally the most susceptible yeast (VRC MIC subset90 of 0.06 microg/ml); C. krusei oft

Butenafine: an update of its use in superficial mycoses.

Butenafine is a synthetic benzylamine antifungal agent that may be fungicidal against susceptible organisms, e.g., dermatophytes. Butenafine may be effective and safe in the treatment of interdigital tinea pedis (apply twice daily for 1 week or once daily for 4 weeks), tinea corporis/tinea cruris (apply twice daily for 2 weeks), and pityriasis versicolor (apply once daily for 2 weeks). The effectiveness of the drug persists for at least 4 weeks following the discontinuation of therapy suggesting that there is some retention of the drug in the skin following termination of active therapy.

Therapeutic potential of terbinafine in subcutaneous and systemic mycoses.

Mycoses vary widely in severity, and may present as superficial, subcutaneous and/or systemic infection. Effective treatments for most superficial mycoses now exist, but new agents with convenient dosing regimens and a low level of adverse events are still needed to reduce morbidity and mortality from serious subcutaneous and systemic fungal infections. In vitro, terbinafine exhibits a broad spectrum of activity against the pathogenic fungi responsible for deep mycoses. Clinical data, while not abundant, suggest that this in vitro activity of terbinafine is reflected in its in vivo efficacy. T…

Clinical pharmacokinetics of fluconazole in superficial and systemic mycoses.

The bis triazole agent fluconazole is used widely in the treatment of superficial and deep mycoses. A single oral dose of fluconazole 150 mg gives a mean long term clinical cure rate of 84 +/- 5% and is considered a valuable alternative to other topical antifungal drugs for vaginal candidiasis. A clinical cure rate of 90.4% for oropharyngeal candidiasis was obtained with 100mg daily for a minimum of 14 days; however, as for the other azoles the rate of relapse was large (40%) in immunocompromised patients. A daily dose of 100mg for at last 3 weeks gave satisfying outcomes for oesophageal candi…

Diagnosis of systemic mycoses by specific immunohistochemical tests.

Immunohistochemistry has proved to be a powerful tool for the accurate diagnosis of a number of important mycoses in humans and animals, such as aspergillosis, candidosis, cryptococcosis, blastomycosis, coccidioidomycosis, histoplasmosis capsulati and duboisii, paracoccidioidomycosis, fusariosis, pseudallescheriosis (scedosporiosis), sporotrichosis, trichosporonosis, penicilliosis, and zygomycosis (mucormycosis). These techniques are also applicable to pneumocystosis and to non-mycotic infections caused by algae such as protothecosis. Apart from the specificity of immunohistochemistry, the app…

Fungi responsible for skin mycoses in Turin (Italy).

There has been a recent increase in the frequency of skin mycoses, coupled with changes in the epidemiology and distribution of the species responsible. Periodic epidemiological analyses of these disease are thus required to ensure their efficacious control. Hair fragments, skin scrapings, specimens from vesicles and blisters and nail parings were seeded on Petri dishes loaded with Sabouraud or Mycosel agar, supplemented with chloramphenicol and with chloramphenicol plus cycloheximide respectively. Parts of each specimen were also mounted in 10% potassium hydroxide for examination under the mi…

Treatment of experimental systemic mycoses with BRL 49594A.

BRL 49594A (BRL) is a water soluble analogue of amphotericin B which has been developed as a polyene with efficacy similar to amphotericin B but with much reduced toxicity. BRL was prepared in 5% glucose, and was used to treat mice experimentally infected with Aspergillus fumigatus, Cryptococcus neoformans, or Histoplasma capsulatum. In the models in which BRL and amphotericin B were compared, BRL was well tolerated but was less effective than a similar regimen of amphotericin B. However, the ability to give much larger doses of BRL than tolerated with amphotericin B suggest that this drug cou
