Children Receiving Hematopoietic Stem Cell Transplantation are at Increased Risk of Onychocryptosis Requiring Surgical Management.

PURPOSE: At our institution, a high proportion of children with onychocryptosis (ingrown toenail) requiring surgical intervention were noted to have a history of hematopoietic stem cell transplantation (HSCT). We analyzed the characteristics of patients who underwent surgical intervention for onychocryptosis and examined our institutional HSCT database to determine if an association exists between onychocryptosis and HSCT. MATERIALS AND METHODS: Surgical cases for onychocryptosis performed from 2000 to 2012 were identified. Nine demographic, clinical, and perioperative variables for both pati

Simple operative management of ingrown toenail using bipolar diathermy.

Ingrowing toenails (IGTN) cause significant discomfort for children and adults alike. Where conservative treatment fails, a surgical approach is usually adopted. Many surgical procedures have been described with varying complexity and outcome. We report a novel, simple technique which involves wedge excision of the ingrowing nail, and bipolar diathermy of the nail bed. Three-hundred and fifty-three procedures were carried out on 302 patients during the study period. Our re-operation rate for recurrence is 9.9%, which compares favourably with other techniques, and which lends itself to utilisat

Surgical treatment of in-growing toenails performed by senior house officers: are they good enough?

BACKGROUND: Although the surgical treatment of in-growing toenails is a common procedure, the success of ingrown toenail surgery is extremely variable and recurrences frequently impair the quality of lift of those who have this condition. In most hospitals this procedure is commonly performed by junior surgical trainees who may have little experience. AIM: We proposed to find out if the success of this procedure is operator-dependent by comparing the results of standard of toenail surgery performed by basic surgical trainees (BSTs) in our hospitals with already published data. METHODS: A ret

Decompression for the management of onychocryptosis.

BACKGROUND: There are many therapeutic alternatives for the management of onychocryptosis. Generally in severe cases, surgical approaches are preferred. OBJECTIVE: The aim of this study was to introduce the new surgical method that treats onychocryptosis without inducing a permanent injury to the matrix. MATERIALS AND METHODS: Twenty-two patients with 30 cases of onychocryptosis were treated by unilateral soft tissue resection combined with partial nail plate avulsion technique without permanent matricectomy. RESULTS: Complete recovery was attained in 25 (83.3%) of cases. No complication wa

Pathology of mitochondria in MELAS syndrome: an ultrastructural study.

BACKGROUND: Onychocryptosis, commonly referred to as ingrown nails, has many therapeutic alternatives for its management. Although mild cases can be treated conservatively, in severe cases, surgical treatment is preferred. Silicone gel sheeting is found to be effective in the treatment of hypertrophic scars and keloids. OBJECTIVE: To document the effectiveness of silicone gel sheeting in the management of patients with onychocryptosis and in the prevention of the recurrences by breaking the devil's circle, which usually took place after the surgical procedures used in the treatment of the ony

A Novel Use of Umbilical Perinatal Graft in Subungual Exostosis Resection.

Nail pathologies have a broad range of origin and may sometimes be complicated in presentation or clinical course, specifically when the pathology remains recalcitrant after treatment. In this case report we discuss a pathologic disorder that was initially misdiagnosed as a pyogenic granuloma surrounding an ingrown nail but was later found to be a benign neoplastic bone growth, Dupuytren exostosis, also known as a subungual exostosis. Operative treatment was deemed appropriate for the patient, and the exostosis was resected, leaving a soft-tissue void at the distal toe. The remaining void was

Safety of phenol vapor inhalation during performance of chemical matrixectomy to treat ingrown toenails.

BACKGROUND: Phenol is commonly used in chemical matrixectomies to treat ingrown toenails. Although many studies have documented the safety of phenol matrixectomy for the patient, few studies have investigated the procedure's safety for the physician and auxiliary health care personnel. OBJECTIVE: This study was undertaken to evaluate the safety of phenol vapor inhalation for health care personnel during performance of chemical matrixectomy. METHODS: The environmental concentrations of phenol in the respiration zones of the physician and an assistant were measured during an in-office surgical

[Modification of surgical treatment of ingrowing toenail].

The authors present their own modification of Emmert's operation used by treatment of ingrowing toenail. The modification itself consists in special technique of the suture of operation wound which prevents further nail incarnation. In second part the results of treatment of 51 patients are presented and discussed in context of other treatment modes. Patients treated by modified technique were treated approximately 14.06 +/- 5.44 days and needed approximately 3.79 +/- 1.04 checkups, others, treated by standard methods it was approximately 19.54 +/- 1.98 days and 4.73 +/- 0.62 checkups. Disorders of the great toe in dancers.

Although often neglected in the consideration of dancing and sports injuries, the great toe is as important as the other large joints of the lower limb. Many injuries to the great toe are associated with technical faults occurring during dancing, and these should be corrected or the injury will recur. It is also important that when a dancer presents with an injury elsewhere, the possibility of faults at the great toe be considered.

[Turrett's exostoses: about 35 casesk].

We conducted a retrospective study of 35 patients with subungual exostosis of the hallux, also known as Turrett's exostosis, in the Department of Orthopedics and Traumatology at the Senior Military Hospital of Instruction of Tunis over the period between 1995 and 2015. We here summarize the outcomes of patients treated for this disease. The average age of patients was 29 years, with a sex ratio of 1.7. The median consultation time was six months. This delay in consultation was caused by a diagnostic error due to clinical picture resemblance with ingrown nail. Diagnosis was always confirmed by

Ingrowing toenails in children and adolescents: is nail avulsion superior to nonoperative treatment?

INTRODUCTION: Ingrowing toenail (IGTN) or onychocryptosis is not uncommon in children and adolescents. However, there is a dearth of evidence in the literature on the management of IGTN in this age group. This study aimed to compare the results of nonoperative treatment for IGTN with that of operative treatment among children and adolescents. METHODS: All children and adolescents who were treated for IGTN at our institution between 2010 and 2014 were included for this retrospective study. Demographic data, treatment prescribed and outcome at six months after presentation were analysed. RESUL

Trauma to the nail unit including occupational sports injuries.

Trauma to the nail unit is one of the most common causes of onychodystrophy and may be acute (a single overwhelming injury) or chronic (repeated minor injury). Immediate effects of acute trauma include splinter hemorrhages, subungual hematomas, and nail shedding. Delayed deformities include splits and ridges, pterygium, hook nails, pigment bands, and ectopic nail. Also discussed are the results of chronic trauma, including self-inflicted injury and repeated indirect injury.

Platelet gel for the surgical treatment of onychocryptosis.

BACKGROUND: The growth factors derived from platelets contained in platelet-rich plasma comprise a series of molecules that favor the production of collagen with the proliferation of fibroblasts and new blood vessels. These substances exert their effect on the cells, acting in all the stages of cicatrization, especially in hemostasis and early fibroplasia. METHODS: Thirty-five patients (70 feet) were selected, operated on both sides for ingrown hallux nails, and subjected to two different experimental conditions in a crossover clinical trial with positive control of treatment. Two main variab

Clinical quiz. Rubinstein-Taybi syndrome (synonyms: broad thumbs and great toes, characteristic facies, and mental retardation -- broad thumb-hallux syndrome).

A 28-year-old mentally retarded, institutionalised woman was referred to us for evaluation of multiple plantar warts and ingrown nails of both great toes. The patient was born to unrelated parents of North African origin and had one brother and three half-brothers, all of whom were healthy. Physical examination revealed short stature, slight obesity, facial abnormalities (Fig. 1), short and broad thumbs and big toes (Figs. 2 and 3). A keloid was found on the right forearm, that had developed after surgical correction of a fracture (Fig. 4). Ill-defined hyperpigmented macules were observed on t

[Lasers in dermatologic surgery].

The authors review their experiences with the use of carbon-dioxide (CO2) lasers in dermatological surgery in a group of 3000 patients, with a total number of 3920 tumorous skin lesions, during a three-year period. The word LASER is an acronym for L-ight A-mplification by S-timulated E-mission of R-adiation. It must be pointed out that it is electromagnetic radiation, not X-irradiation. In regard to the spectrum laser light is between infrared and ultraviolet light, mainly in the visible spectrum, so its application does not produce new generations of iatrogenic malignancies as in the case of

Treatment of ingrown toenails in the pediatric population.

BACKGROUND/PURPOSE: Nail avulsion plus chemical matrixectomy (CM) using NaOH as an alternative to surgical matrixectomy (SM) has recently been used in the treatment of ingrown toenails (IGTNs) in adults. No studies exist to dictate the most effective and safe treatment method in the pediatric population. METHODS: A retrospective review of pediatric IGTNs treated at 2 institutions for 6 years was done, looking at presentation, treatment modality, SM vs CM, and outcomes. RESULTS: Eight hundred forty-eight IGTNs in 518 patients were reviewed with an average age of 12.5 years. Twenty-three perce
