Sugar rationing during the first 1000 days and early onset cancer: a natural experiment.

BACKGROUND: The "first 1000 days" of life is a critical window for metabolic programming, while the long-term oncological consequences of nutritional exposures during this period remain understudied. OBJECTIVES: We aimed to evaluate whether restricted sugar intake in utero and during early childhood reduces risk of early onset cancer diagnosis and mortality in adulthood, utilizing a natural experiment. METHODS: We analyzed 63,819 United Kingdom Biobank participants born between October 1951 and March 1956, spanning the end of United Kingdom sugar rationing (September 1953). Leveraging a quas

Long-term survivors of gastric cancer: a California population-based study.

PURPOSE: In the United States, gastric cancer is rapidly fatal with a 25% 5-year survival. Of the few patients who survive, little is known about their demographic, clinical, and tumor characteristics. PATIENTS AND METHODS: Data regarding all cases of gastric and gastroesophageal junction (GEJ) adenocarcinoma diagnosed in California between 1988 and 2005 were obtained from the California Cancer Registry, a member of the National Cancer Institute's Surveillance, Epidemiology, and End Results (SEER) program. A Cox proportional hazards model was constructed to understand the independent relation

Variations in Persistent Use of Low-Value Breast Cancer Surgery.

IMPORTANCE: Through the Choosing Wisely campaign, surgical specialties identified 4 low-value breast cancer operations. Preliminary data suggest varying rates of deimplementation and have identified patient-level and clinician-level determinants of continued overuse. However, little information exists about facility-level variation or determinants of differential deimplementation. OBJECTIVE: To identify variation and determinants of persistent use of low-value breast cancer surgical care. DESIGN, SETTING, AND PARTICIPANTS: Retrospective cohort study in which reliability-adjusted facility rat

Trends in cancer during pregnancy and adverse neonatal outcome: a nationwide population-based study in Taiwan.

BACKGROUND: Cancer has remained a leading cause of death worldwide. Although relatively rare during pregnancy, cancer poses significant risks to both maternal and fetal health. With the continuous rise in maternal age, the incidence of cancer during pregnancy is expected to increase. In this nationwide, population-based epidemiological study, we aimed to assess temporal trends in the incidence of cancer during pregnancy and evaluate its association with adverse neonatal outcomes. METHODS: We analyzed nationwide data from the Taiwan National Health Insurance Research Database, Taiwan Cancer Reg

Melanoma incidence rates among whites in the U.S. Military.

BACKGROUND: The U.S. Military and general populations may differ in the exposure to sunlight and other risk factors for melanoma and therefore the incidence rates of melanoma may be different in these two populations. However, few studies have compared melanoma incidence rates and trends over time between the military and the general population. METHODS: Melanoma incidence rates from 1990 to 2004 among white active-duty military personnel and the general U.S. population were compared using data from the Department of Defense Automated Central Tumor Registry and the National Cancer Institute S

Changes in incidence of HPV-related cancers in South Africa (2011-21): a cross-sectional analysis of the South African National Cancer Registry.

BACKGROUND: Understanding human papillomavirus (HPV)-related cancer epidemiology in South Africa is crucial for informing cancer prevention in this high-burden country. We aimed to describe HPV-related cancer incidence in South Africa between 2011 and 2021. METHODS: For this cross-sectional study, we obtained data on cancer incidence from the South African National Cancer Registry and population estimates from Statistics South Africa. We calculated age-standardised incidence per 100 000 person-years for cervical carcinoma, and vulvar, vaginal, penile, oropharyngeal, and anal squamous cell car

Patterns and Differences in Lung Cancer Treatment: United States, 2015-2020.

BACKGROUND: Treatment for lung cancer can improve prognosis, but 5-year survival remains low at 26%. An examination of treatment using data with higher population coverage, and among a broader number of treatment modalities and individual characteristics, would provide greater insight into differences in lung cancer treatment. RESEARCH QUESTION: Among adults diagnosed with lung cancer, how does reported receipt of lung cancer treatment differ by sociodemographic characteristics? STUDY DESIGN AND METHODS: We used 2015-2020 National Program of Cancer Registry data covering 89% of the US popula

Estimating breast cancer-specific and other-cause mortality in clinical trial and population-based cancer registry cohorts.

BACKGROUND: To compute net cancer-specific survival rates using population data sources (eg, the National Cancer Institute's Surveillance, Epidemiology, and End Results [SEER] Program), 2 approaches primarily are used: relative survival (observed survival adjusted for life expectancy) and cause-specific survival based on death certificates. The authors of this report evaluated the performance of these estimates relative to a third approach based on detailed clinical follow-up history. METHODS: By using data from Cancer Cooperative Group clinical trials in breast cancer, the authors estimated

Induced abortion and breast cancer risk.

Results from case-control studies suggest that induced abortion may be associated with a small increase in risk of breast cancer. While risk estimates from cohort studies have generally not observed such an association, these studies have had limited information regarding abortion and possible confounding variables. Therefore, we conducted a study among a cohort of post-menopausal women from whom detailed information regarding pregnancy outcomes as well as risk factors for breast cancer had been collected. The study sample included 37,247 Iowa Women's Health Study participants, 55-64 years of

Breast and cervical cancer screening in a semi-urban African setting: evidence from Franceville, Gabon (2015-2021).

BACKGROUND: Breast and cervical cancers are among the most frequent malignancies in women worldwide and represent a growing public health concern in sub-Saharan Africa (SSA). In Gabon, national screening programs have been established, but their effectiveness remains poorly documented, particularly outside the capital city, Libreville. This study, therefore, aimed to describe the screening outcomes and associated factors for breast abnormalities and suspected cervical lesions at the Centre Hospitalier Universitaire Amissa Bongo (CHUAB) in Franceville, Gabon. METHODS: We conducted a retrospect

[Geomapping of 10-year coverage rate of cervical cancer screening in Sweden - a basis for targeting population-level interventions to improve screening].

A high coverage rate of cervical cancer screening is necessary to prevent cervical cancer. Women who have not been tested in the last 10 years are considered as long-term non-attenders from the screening program. A recent report from the Regional Cancer Centers in Sweden addressed geographic disparities in the 10-year coverage rate among 33-62-year-old women residing in Sweden in 2023, and compared the results with those in a previous report from 2020. The analytic method employed is referred to as geomapping, based on geo-coded data to 5984 neighborhoods (Statistics Sweden's Demographic Stati

Impact of Social Disparities on 5-Year Survival Rates in Pediatric Hematologic Malignancies.

BACKGROUND: Socioeconomic disparities exist in pediatric patients with hematologic malignancies, leading to suboptimal survival rates. Social determinants of health impact health outcomes, and in children, they may not only lead to worse survival outcomes but carry over into late effects in adult life. The social deprivation index (SDI) is a composite score using geographic county data to measure social determinants of health. Using the SDI, the purpose of the present study is to stratify survival outcomes in pediatric patients with hematologic malignancies based on area deprivation. METHODS:

Modeling the Cost-Effectiveness of Alternative Upper Age Limits for Breast Cancer Screening in England and Wales.

BACKGROUND: Currently in the United Kingdom, the National Health Service (NHS) Breast Screening Programme invites all women for triennial mammography between the ages of 47 and 73 years (the extension to 47-50 and 70-73 years is currently examined as part of a randomized controlled trial). The benefits and harms of screening in women 70 years and older, however, are less well documented. OBJECTIVES: The aim of this study was to examine whether extending screening to women older than 70 years would represent a cost-effective use of NHS resources and to identify the upper age limit at which scr

Epidemiology of Cervical Cancer in Brazil: Age-Specific Trends From 2000 to 2018.

PURPOSE: Cervical cancer (CC) remains a significant public health challenge in low- and middle-income countries (LMICs), including Brazil. While global incidence has declined, age-specific variations may mask emerging trends among younger women. This study aimed to evaluate trends and age-specific changes in CC incidence in Brazil between 2000 and 2018. MATERIALS AND METHODS: Incidence data and age at diagnosis were obtained from Brazilian Population-Based Cancer Registries (PBCRs) for the period 2000-2018. Temporal trends were evaluated using Joinpoint regression to estimate annual percent c
