Transfer of the extensor indicis proprius branch of posterior interosseous nerve to reconstruct ulnar nerve and median nerve injured proximally: an anatomical study.

Proximal or middle lesions of the ulnar or median nerves are responsible for extensive loss of hand motor function. This occurs even when the most meticulous microsurgical techniques or nerve grafts are used. Previous studies had proposed that nerve transfer was more effective than nerve grafting for nerve repair. Our hypothesis is that transfer of the posterior interosseous nerve, which contains mainly motor fibers, to the ulnar or median nerve can innervate the intrinsic muscles of hands. The present study sought to investigate the feasibility of reconstruction of the deep branch of the ulna…

The Impacts of Marijuana Usage on Nerve Transfer Outcomes: A Retrospective Cohort Study.

BACKGROUND AND OBJECTIVES: Marijuana is often used by patients with peripheral nerve injuries for pain management, but its impact on nerve transfer surgery outcomes remains unexplored. METHODS: This retrospective cohort study reviewed the records of patients age 18 years and older who underwent radial-to-axillary or ulnar fascicle to biceps fascicle (Oberlin) nerve transfers at a single institution from 2014 to 2023. Data collected included demographics, surgical details, injury characteristics, marijuana use, and Medical Research Council strength scores from preoperative evaluations to 24 mo…

Distal Nerve Transfers for Foot Drop: A Systematic Review and Meta-Analysis.

BACKGROUND: Foot drop from peroneal nerve palsy causes steppage gait and functional disability. Tendon transfer is reliable but sacrifices native biomechanics. Distal nerve transfer (DNT) reinnervates dorsiflexors, potentially restoring more physiologic function. This study aimed to systematically review and meta-analyze the clinical outcomes of DNT for foot drop and to identify key prognostic and technical factors that inform contemporary reconstructive decision-making. METHODS: Following PRISMA, we searched MEDLINE, EMBASE, and Cochrane (through September 2025) for clinical studies of DNT t…

Flail Arm Reconstruction After Failed Nerve Transfers: A Case Report.

CASE: A 53-year-old, right-hand-dominant gentleman suffered a traumatic right scapulothoracic dissociation with severe soft tissue damage and complete right arm paralysis following a high-speed all-terrain vehicle accident. Subsequent burns led to digital amputations. He underwent elbow disarticulation with humeral shortening osteotomy, glenohumeral arthrodesis, and a vascularized radial osteocutaneous flap for clavicular reconstruction. Postoperatively, he achieved stable limb positioning and functional improvement with a prosthesis. CONCLUSION: This case underscores the importance of a mult

Nerve Transfers for Treatment of Upper Extremity Paralysis in Acute Flaccid Myelitis.

Acute flaccid myelitis (AFM) is a rare, debilitating neurological disease resulting in pure motor deficits in school-aged children. Unfortunately, 90% of patients do not experience complete spontaneous recovery, and medical therapy has not proven efficacious. Herein, we report the course of 2 patients with AFM who underwent upper extremity nerve transfers to treat persistent upper extremity paralysis after failing roughly 6.5 months of conservative treatment. Objective comparisons were made between preoperative and postoperative examinations using the Active Movement Scale and Mallet Classific…

Nerves transfers for functional hand recovery in traumatic lower brachial plexopathy.

BACKGROUND: Distal nerve transfers are an innovative modality for the treatment of C8-T1 brachial plexus lesions. The purpose of this case series is to report the authors' results with hand restoration function by nerve transfer in patients with lower brachial plexus injury. METHODS: Three consecutive nerve transfers were performed in a series of 11 patients to restore hand function after injury to the lower brachial plexus: brachialis motor branch to anterior interosseous nerve (AIN) and supinator branch to the posterior interosseous nerve (PIN) in a first surgical procedure, and AIN to pron…

Median to radial nerve transfers for restoration of wrist, finger, and thumb extension.

Radial nerve injury results in loss of wrist, finger, and thumb extension. Traditionally, radial nerve palsies that fail to recover spontaneously have been reconstructed with tendon transfers or nerve grafts. Nerve transfers are a novel approach to the surgical management of Sunderland grade IV and V radial nerve injuries. We describe our technique for median to radial nerve transfers. In this procedure, the flexor digitorum superficialis nerve is transferred to the extensor carpi radialis brevis nerve for wrist extension, and the flexor carpi radialis nerve is transferred to the posterior int…

Functional outcome of nerve transfers for traumatic global brachial plexus avulsion.

BACKGROUND: The treatment of global brachial plexus avulsion is a demanding field of hand and upper extremity surgery. The recent development of functional and quality-of-life (QOL) assessment tools has improved quantifying these functional outcomes after surgery. OBJECTIVE: We sought to combine Medical Research Council (MRC) grading with the Disability of the Arm, Shoulder, and Hand (DASH) questionnaires and Numerical Rating Scale (NRS) for pain to evaluate the functional outcome of patients who suffered complete brachial plexus avulsion before and after nerve transfers. METHODS: The author…

Use of the masseter motor nerve in facial animation with free muscle transfer.

Facial paralysis is either congenital or acquired, and of varying severity, which leads to an asymmetrical or absent facial expression. It is an important disability both from the aesthetic and functional points of view. Between 2003 and 2008, at the Department of Maxillofacial Surgery, University of Parma, Italy, 21 patients with facial paralysis had their faces reanimated with a gracilis transplant reinnervated by the masseter motor nerve. All free-muscle transplants survived the transfer, and no flap was lost. Facial symmetry at rest and while smiling was excellent or good in most cases, an

Surgical outcomes following nerve transfers in upper brachial plexus injuries.

BACKGROUND: Brachial plexus injuries represent devastating injuries with a poor prognosis. Neurolysis, nerve repair, nerve grafts, nerve transfer, functioning free-muscle transfer and pedicle muscle transfer are the main surgical procedures for treating these injuries. Among these, nerve transfer or neurotization is mainly indicated in root avulsion injury. MATERIALS AND METHODS: We analysed the results of various neurotization techniques in 20 patients (age group 20-41 years, mean 25.7 years) in terms of denervation time, recovery time and functional results. The inclusion criteria for the s…

Nerve transfers in brachial plexus birth palsies: indications, techniques, and outcomes.

The advent of nerve transfers has greatly increased surgical options for children who have brachial plexus birth palsies. Nerve transfers have considerable advantages, including easier surgical techniques, avoidance of neuroma resection, and direct motor and sensory reinnervation. Therefore, any functioning nerve fibers within the neuroma are preserved. Furthermore, a carefully selected donor nerve results in little or no clinical deficit. However, some disadvantages and unanswered questions remain. Because of a lack of head-to-head comparison between nerve transfers and nerve grafting, the wi…

Improved myoelectric prosthesis control accomplished using multiple nerve transfers.

BACKGROUND: The control of shoulder-level disarticulation prostheses is significantly more difficult than that of prostheses for more distal amputations. Amputees have significant difficulties coordinating the separate functions of prosthetic shoulder, elbow, wrist, and hand/hook components. The user must lock one joint at a particular position in space before subsequently moving a different joint. METHODS: A patient with bilateral humeral disarticulations after an electrical injury underwent a novel nerve transfer procedure designed to improve the control of a myoelectric prosthesis. The med…

Selective laryngeal reinnervation with separate phrenic and ansa cervicalis nerve transfers.

OBJECTIVE: To perform selective reinnervation of the laryngeal abductor and adductor muscle groups after injury to the recurrent laryngeal nerve, recovering laryngeal function without impairment by synkinesis. DESIGN: Ten cats underwent the surgical procedure. To reinnervate the posterior cricoarytenoid muscle (abductor), a phrenic nerve graft was anastomosed to the main trunk of the recurrent laryngeal nerve. The adductor branch was severed, and the proximal stump was buried in the posterior cricoarytenoid muscle. The sternohyoid branch of the ansa cervicalis was anastomosed to the distal st

Ultrasonographic Median Nerve Changes After Repeated Wheelchair Transfers in Persons With Paraplegia: Relationship With Subject Characteristics and Transfer Skills.

BACKGROUND: Wheelchair users with spinal cord injuries are susceptible to peripheral neuropathies from overuse, yet no studies have established a relationship between median neuropathy and wheelchair transfers. A more thorough understanding of how transfers and technique contribute to pathologic conditions may guide interventions that curtail its development. OBJECTIVE: To evaluate the effects of repeated transfers on ultrasound markers for carpal tunnel syndrome (CTS) in people with spinal cord injuries and to relate changes to subject characteristics and transfer skills. DESIGN: Cross-sect

High Median Nerve Reconstruction with Vascularized Ulnar Nerve and Extensor Carpi Radialis Brevis Motor Branch to Anterior Interosseous Nerve Transfers.

Combined high median and ulnar nerve injuries present significant challenges due to the complexity of restoring sensory and motor functions with limited donor sites for tendon or nerve transfers. Traditional techniques such as cable grafting often fail to achieve satisfactory functional recovery, particularly in cases involving proximal injuries and large nerve gaps. We report a case of a 21-year-old man who sustained a gunshot wound to the right medial upper arm, injuring the median and ulnar nerves. Three months postinjury, the patient exhibited upper extremity weakness, with 0 of 5 finger a…
