Parry-romberg syndrome: a linear variant of scleroderma with discoid lupus erythematosus on scalp - an association.

Parry-Romberg syndrome is a rare neurocutaneous syndrome characterized by progressive shrinkage and degeneration of the tissues beneath the skin, usually on one side of the face. It is associated with neurological disorders such as trigeminal neuralgia, facial paresthesia, headache, and focal epilepsy. Concomitant occurrence of discoid lupus erythematosus (DLE) and morphea in the same skin lesion is exceptional, defined as overlap syndrome with two or more different connective tissue disease concurrently or consecutively. A 32-year-old female developed DLE on a long-standing lesion of sclerode

Ocular features in a patient presenting with a rare combination of multiple phakomatoses.

Phakomatoses are a group of congenital disorders characterised by hamartomatous lesions of the skin as well as the central and peripheral nervous systems. The presence of naevus flammeus or port-wine stain is a characteristic feature of many such disorders including Sturge-Weber syndrome (SWS), Klippel-Trenaunay syndrome (KTS) and Phakomatosis pigmentovascularis (PPV).We describe the ocular findings in a patient with coexisting PPV, SWS and KTS.

Is the prophylactic use of antiepileptic drugs in Sturge-Weber syndrome justified?

Sturge-Weber syndrome (SWS) is a rare neurocutaneous syndrome. Epilepsy is one of its most common features and is usually the first neurological manifestation. Adequate control of epilepsy assumes major importance in SWS as the early onset of seizures and severe seizures are associated with developmental regression and hemiparesis. Recent advances in neuroimaging have helped in making preclinical and early diagnosis in SWS. The author proposes treating newly diagnosed patients with severe SWS with antiepileptic drugs (AEDs) before the onset of epilepsy. This may help prevent the development of

The genetics of malignant melanomas.

A review of the hereditary aspects of the malignant melanomas showed causal heterogeneity and similar pathogenesis based on the dysregulation of the paracrine/autocrine growth mechanisms. The genetically different malignant melanomas have a range of recurrence risks from 1% for the nonfamilial, solitary, malignant melanoma to a risk exceeding 70% for the syndromic melanomas of neurocutaneous melanosis and the nine types of xeroderma pigmentosum. A recurrence risk of 6% is relevant to the members of dysplastic nevus syndrome families without malignant melanomas and the risk increases in excess

Segmental Pallor and Hair Loss in a Patient With Ross Syndrome: Case Report.

A 30-year-old male sought medical consultation due to sudden anisocoria. Clinical examination confirmed Adie's tonic pupil. Subsequently, the presence of segmental anhidrosis with compensatory hyperhidrosis and areflexia established the diagnosis of Ross syndrome. In a 15-year follow-up, uncommon manifestations developed in the facial territory affected by compensatory hyperhidrosis: unilateral facial pallor and alopecia following a midline-defined pattern. This case emphasizes the importance of recognizing neurocutaneous manifestations in dermatology practice and suggests that Ross syndrome m

Standard fractionation low-dose proton radiotherapy for diffuse choroidal hemangiomas in pediatric Sturge-Weber syndrome.

Sturge-Weber syndrome is a nonhereditary congenital neurocutaneous syndrome characterized by leptomeningeal angiomatosis, facial nevus flammeus, and diffuse choroidal hemangioma, which when complicated by total retinal detachment, portend a poor prognosis. Management is often limited to salvage external beam irradiation. We present a modified proton therapy technique for young children with total bullous retinal detachments that uses standard fractionation low-dose proton radiotherapy to decrease the risk of radiation complications. Treatment techniques for young children who cannot cooperate

Successful treatment of angiofibromata of tuberous sclerosis complex with rapamycin.

Tuberous sclerosis complex (TSC) is an autosomal dominant neurocutaneous syndrome manifesting as hamartomatous growths in multiple organs. We present two cases of patients with TSC and associated facial angiofibromata treated with topical and oral rapamycin and discuss the role for rapamycin in the treatment of these disfiguring lesions. Our patients demonstrated decreased numbers of angiofibromata and less redness with this treatment. This is the first published report on the use of topical rapamycin for the treatment of angiofibromata.

Bathing trunks nevus: case report of giant congenital melanocytic nevus.

Bathing trunks nevi, a subtype of giant congenital melanocytic nevi (CMN), are skin tumors that present by 2 years of age and occur in a low percentage of all births. We report a case of bathing trunks nevus that was initially suspected to be melanoma, and describe the history, pathophysiology, and treatment options for CMN. We also discuss the risk for neurocutaneous melanosis (NCM), which is a rare syndrome in patients with giant CMN.

Therapeutic Strategies in Neurocutaneous Melanocytosis.

Neurocutaneous melanocytosis (NCM) is a rare, congenital condition primarily affecting children, characterized by large or giant congenital melanocytic nevi (L/GCMN) on the skin and pigmented lesions in the brain. The presence of pigmented lesions in the brain often leads to neurological symptoms like headaches, seizures, and hydrocephalus, typically manifesting before age two. Melanocytic lesions in the brain can range from benign melanocytosis to malignant melanoma. NCM is nearly always fatal if symptomatic, with a high risk of malignant transformation. Patients with larger skin nevi with ne

The risk of glaucoma associated with phacomatosis cesioflammea and phacomatosis cesioflammeo-marmorata.

BACKGROUND: The ocular features of phacomatosis pigmentovascularis (PPV) have rarely been reported, and glaucoma is the leading cause of blindness in patients with this condition. To protect vision in these patients, it is important to identify glaucoma as early as possible. OBJECTIVES: To systematically report the systemic and ocular manifestations of phacomatosis cesioflammea and phacomatosis cesioflammeo-marmorata, and to investigate a glaucoma risk scoring system. MATERIALS & METHODS: In this prospective study, patients with PPV from 2014 to 2021 were included. Clinical information was c

[Melanocytic Tumors].

Primary melanocytic neoplasms of the central nervous system(CNS)presumably arise from leptomeningeal melanocytes that are derived from the neural crest. Melanocytic neoplasms associated with neurocutaneous melanosis likely derive from melanocyte precursor cells that reach the CNS after somatic mutations, mostly, of the NRAS. They should be distinguished from other melanotic tumors involving the CNS, including metastatic melanoma and other primary tumors that undergo melanization, such as melanocytic schwannomas, medulloblastomas, paragangliomas, and various gliomas, because these lesions requi

Infantile and congenital hemangiomas.

Infantile hemangiomas (IHs) are the most common benign tumors of infancy. They typically appear after birth and undergo a period of rapid growth, followed by a gradual period of involution. Although the majority of IHs do not requirement treatment, oral propranolol is the first-line therapy for lesions that are at risk for life-threatening complications, functional impairment, ulceration, or permanent disfigurement. Rarely, IHs can be associated with structural anomalies. Congenital hemangiomas (CHs) are a distinct clinical entity, caused by a point mutation in GNAQ or GNA11. These lesions are

Neuro-ophthalmic manifestations of the phakomatoses.

PURPOSE OF REVIEW: The phakomatoses are a group of inherited disorders with variable clinical manifestations that are characterized by brain, cutaneous, ocular and other distinct lesions in multiple organs. Correctly recognizing the neuro-ophthalmic signs and symptoms can lead to early diagnosis and treatment. The group is composed of neurofibromatosis (type 1 and 2), tuberous sclerosis complex, von Hippel-Lindau, ataxia-telangiectasia and Sturge-Weber syndromes. However, more than 60 syndromes have been described in the medical literature. This review provides an update on the diagnosis and m

Segmental hypomelanosis and hypermelanosis arranged in a checkerboard pattern are distinct naevi: flag-like hypomelanotic naevus and flag-like hypermelanotic naevus.

The categorization of congenital hypo- or hyperpigmented skin lesions following a segmental pattern has been a long-lasting matter of debate and have been reported under various and often incorrect terms. To reassess published hypomelanotic and hypermelanotic lesions that did not follow Blaschko lines nor a phylloid pattern of mosaicism, we carried out an extensive and critical review of the worldwide literature. Seventy-four retrieved cases consisted of lateralized hypomelanotic lesions arranged in a flag-like pattern or appearing as large patches of grossly oval or angulated shape and sharp,

Tuberous sclerosis complex: The critical role of the interventional radiologist in management.

Tuberous sclerosis complex (TSC) is an autosomal dominant neurocutaneous syndrome that is characterised by hamartomas in multiple organs, the characteristic imaging features of which are illustrated in this case report. Angiomyolipoma (AML) is the most common renal manifestation of TSC, which may present with life-threatening haemorrhage at the time of diagnosis. Interventional management with selective renal embolisation is currently the treatment of choice for the safe and effective management of ruptured renal AML.

Neurocutaneous melanomatosis with a rapidly deteriorating course.

Neurocutaneous melanosis is a rare congenital syndrome characterized by large or multiple congenital melanocytic nevi and benign or malignant pigment cell tumors of the leptomeninges. The prognosis is extremely poor for symptomatic patients, even in the absence of malignant melanoma. We present serial MR imaging findings in the brain and spine of a child with congenital giant hairy nevi who developed progressive leptomeningeal melanomatosis and whose neurologic condition rapidly deteriorated.
