Evolution of flash visual evoked potentials to monitor visual pathway integrity during tumor resection: illustrative cases and literature review.

Flash visual evoked potentials (fVEPs) provide a means to interrogate visual system functioning intraoperatively during tumor resection in which the optic pathway is at risk for injury. Due to technical limitations, fVEPs have remained underutilized in the armamentarium of intraoperative neurophysiological monitoring (IONM) techniques. Here we review the evolution of fVEPs as an IONM technique with emphasis on the enabling technological and intraoperative improvements. A combined approach with electroretinography (ERG) has enhanced feasibility of fVEP neuromonitoring as a practical application…

Intraoperative neuromonitoring during resection of cranial meningiomas and its effect on the surgical workflow.

PURPOSE: Resection of meningiomas adjacent to the central sulcus entails a high rate of morbidity. Explored for intra-axial lesion resection, intraoperative neuromonitoring intraoperative neuromonitoring (IONM) has been shown to decrease neurological deficits. The use of IONM is relatively uncommon and is not considered routine practice in the removal of extra-axial lesions. We sought to characterize IONM's impact on the surgical workflow in supratentorial meningiomas. METHODS: We retrospectively analyzed a prospectively collected database, searching cases in which IONM was used for resection

Recent advances in traumatic brain injury.

Traumatic brain injury (TBI) is the most common cause of death and disability in those aged under 40 years in the UK. Higher rates of morbidity and mortality are seen in low-income and middle-income countries making it a global health challenge. There has been a secular trend towards reduced incidence of severe TBI in the first world, driven by public health interventions such as seatbelt legislation, helmet use, and workplace health and safety regulations. This has paralleled improved outcomes following TBI delivered in a large part by the widespread establishment of specialised neurointensiv

Caveats of Cooling: Available Evidence and Ongoing Investigations of Therapeutic Hypothermia.

Therapeutic hypothermia (TH) mitigates the long-term effects of neuronal excitotoxicity and cell death seen in hypoxic-ischemic encephalopathy (HIE). It remains the most evidence-based therapy for HIE, but it is not without clinical controversy. The literature abounds with questions, such as "When should we start cooling-as early as the delivery room?" "Given the efficacy of TH for moderate to severe HIE when started within 6 hours of birth, can we expand the therapy to infants with mild HIE?" "What should the target temperature be?" "What is the optimal duration of treatment?" "Is early disco

[Intraoperative monitoring of nerve roots in spinal surgery].

INTRODUCTION: The use of intraoperative monitoring to determine the functional integrity of the nervous system has grown steadily over the last 15 years. The development of new instrumentation systems in spinal surgery and a greater incidence of complications have led to the need to introduce new methods of monitoring. AIMS AND DEVELOPMENT: The purpose of this study is to analyse the neurophysiological techniques that are currently available and applied in order to prevent injury to nerve roots and the fibres of the cauda equina, to assess their indications and to determine their limitations,

[Identification of the cochlear nerve in surgical removal of vestibular schwannomas].

Review of methods which allow intraoperative identifying and monitoring of cochlear and facial nerves in resection of vestibular schwannomas is presented. We describe a case of successful identification and functional preservation of facial and cochlear nerves in a patient with acoustic neuroma. Detailed description of neurophysiological and neurosurgical aspects of surgical technique intended to preserve auditory functions is given. According to some authors, application of intraoperative neurophysiological techniques in surgery of vestibular schwannomas allows to preserve not only anatomical

EEG Signal Decomposition Evidence for a Role of Perceptual Processes during Conflict-related Behavioral Adjustments in Middle Frontal Regions.

Conflict monitoring processes are central to cope with fluctuating environmental demands. However, the efficacy of these processes depends on previous trial history/experience, which is reflected in the "congruency sequence effect" (CSE). Several theoretical accounts have been put forward to explain this effect. Some accounts stress the role of perceptual processes in the emergence of the CSE. As yet, it is elusive how these perceptual processes are implemented on a neural level. We examined this question using a newly developed moving dots flanker task. We combine decomposition methods of EEG

Spinal cord monitoring.

Spinal cord surgery carries the risk of spinal cord or nerve root injury. Neurophysiologic monitoring decreases risk of injury by continuous assessment of spinal cord and nerve root function throughout surgery. Techniques include somatosensory evoked potentials (SEPs), transcranial electrical motor evoked potentials (MEPs), and electromyography (EMG). Baseline neurophysiologic data are obtained prior to incision. Real-time signal changes are identified in time to correct compromised neural function. Such monitoring improves postoperative neurologic functional outcomes. Challenges in neurophysi

Neurophysiologic assessment in the management of spinal dysraphism.

Neurophysiologic techniques provide a valuable addition to the armamentarium of tools for the evaluation of sensory and motor function in the pediatric spinal cord. These techniques include median, radial, and ulnar nerve evoked potentials from the upper extremity; common peroneal and tibial nerve evoked potentials from the lower extremity; dermatomal potentials; and compound muscle action potentials and compound nerve action potentials. The techniques that evaluate the sensory system have been used extensively and effectively as research tools, as adjuncts to diagnostic evaluation, and for in

Smart Wearable EEG Devices: A Review of Lightweight, Multi-Sensor Systems for Sleep and Everyday Neurophysiology.

Wearable electroencephalography (EEG) is rapidly evolving toward lightweight, user-friendly systems that enable brain monitoring in naturalistic settings. Traditional multi-channel, gel-based systems provide broad scalp coverage and high signal fidelity but are impractical for unsupervised or long-term use. This review focuses on the emerging generation of smart wearable EEG devices that are easy to wear, require minimal setup, and typically integrate additional physiological sensors such as photoplethysmography (PPG), temperature, or motion sensors. We review wearable EEG systems across four

Is Electroencephalographic Burst-Suppression Good, Bad, or Indifferent for Brain Health? Context Matters.

Intraoperative burst-suppression (BSP) has long been postulated to be associated with an increased risk of postoperative delirium (POD) based on data from human experimental studies and meta-analyses. However, the effects of BSP on POD are not consistently demonstrated in large-scale randomized controlled trials. At a minimum, this warrants an explanation of the discrepancies between these results, and at a maximum, prompts the questioning of the utility of intraoperative electroencephalographic (EEG) monitoring for POD altogether. Here, we provide a narrative review of the mechanisms of BSP g

Anesthetic Management of Narcolepsy Patients During Surgery: A Systematic Review.

BACKGROUND: Narcolepsy is a rare sleep disorder characterized by excessive daytime sleepiness, sleep paralysis, and/or hypnagogic/hypnopompic hallucinations, and in some cases cataplexy. The response to anesthetic medications and possible interactions in narcolepsy patients is unclear in the perioperative period. In this systematic review, we aim to evaluate the current evidence on the perioperative outcomes and anesthetic considerations in narcolepsy patients. METHODS: Electronic literature search of Medline, Medline in-process, Embase, Cochrane Database of Systematic Reviews databases, inte

Intraoperative recording of the bulbocavernosus reflex.

: The bulbocavernosus reflex (BCR) is mediated by the sacral somatic afferent/efferent periphery as well as the sacral cord. Unfortunately, the reflex has suffered from a partly deserved reputation as difficult to implement. However, recent stratagems have improved the test's reliability. Multipulse stimulation (enhanced by double trains as required) and exacting recording technique can yield positive and remarkably reproducible results in patients of all ages and either sex. In this review, we document a 94% baseline BCR acquisition rate among 100 consecutive cases in one institution. Accepta

A mouthful - airway matters in intraoperative neuromonitoring in auditory brainstem implant surgery for the pediatric patient: a case series.

BACKGROUND: The Auditory brainstem implant (ABI) is a new surgical option for hearing impaired children. Intraoperative neurophysiology monitoring includes brainstem mapping of cranial nerve (CN) IX, X, XI, XII and their motor nuclei, and corticobulbar tract motor-evoked potential. These require laryngeal electrodes and intra-oral pins, posing a challenge to airway management especially in the pediatric airway, where specialized electromyogram (EMG) tracheal tubes are not available. Challenges include determining the optimum position on the endotracheal tube (ETT) in which to place laryngeal e

Monitoring cardiac and ascending aortic procedures.

Although cardiac and aortic operations have been successfully performed for more than 60 years, the risk of neurologic complications remains high. In particular, the rate of stroke with cardiac operations continues to be significant in the 1%-5% range. Similarly, the risk of stroke with aortic operations remains in the range of 7%-10% despite many years of improving techniques. Because of this persistently high risk, the use of intra-operative neurophysiologic monitoring (IONM) has the potential of improving outcomes. This chapter provides an overview of cardiac/aortic arch procedures from the

Unilateral External Anal Sphincter MEP Monitoring in a Case of Conus Medullaris Tumor.

The use of intraoperative neurophysiological monitoring is a helpful tool during surgeries of conus medullaris tumors and helps in reducing the risk of post-operative functional compromise like paraparesis, sensory deficits, or urological dysfunctions. External anal sphincter (EAS) motor evoked potential (MEP) monitoring is usually done for monitoring sacral nerve roots and to prevent post-operative neurological deficits. Here we describe a case where unilateral motor cortical stimulation aided us to differentiate the laterality of TcMEP responses in EAS.

Clinical Utility of Far-Field Potentials in Amyotrophic Lateral Sclerosis.

INTRODUCTION/AIMS: Far field potentials (FFP) have been proposed as a reliable neurophysiological prognostic biomarker in amyotrophic lateral sclerosis (ALS). This study evaluates the diagnostic utility of ulnar nerve FFP in ALS. METHODS: Comprehensive peripheral neurophysiological assessments were conducted in 62 ALS and 43 ALS-mimicking disorder participants. The ulnar nerve was stimulated at the wrist, recording motor responses over the abductor digit minimi (ADM) muscle. Conventional compound muscle action potentials (CMAP), FFP, and near field potential amplitudes were recorded, alongsid

Paradigms of intraoperative neuromonitoring in paediatric thyroid surgery.

The larynx of children and adolescents is still in the developmental phase and the anatomical structure is still very small and sensitive. The higher malignancy and faster progression of some paediatric thyroid cancers make surgery more difficult. Intraoperative neuromonitoring (IONM) is frequently used in thyroid surgery as an effective means of securing the recurrent laryngeal nerve (RLN). Little information is available on the clinical efficacy of IONM in paediatric surgery. In addition, classic IONM techniques such as reinforced tracheal tube models with integrated surface electrodes are n
